In late 2024, users reported that ChatGPT sometimes stopped, returned an error, or failed to produce the name “David Mayer.” OpenAI told The Guardian that one of its tools had mistakenly flagged the name. The newspaper reported on December 3, 2024, that the issue had been fixed. That is a dated account of the incident—not confirmation of how every ChatGPT model or interface behaves today.
What happened when users entered the name?
In late November and early December 2024, people reported that ChatGPT responses could stop after beginning to produce “David Mayer” or fail with messages such as “something seems to have gone wrong” and “I’m unable to produce a response.” A contemporaneous OpenAI Developer Community discussion captures users asking why the failure happened, but it documents their reports rather than establishing the technical cause.
What explanation did OpenAI give?
On December 3, 2024, The Guardian quoted an OpenAI spokesperson as saying: “One of our tools mistakenly flagged this name and prevented it from appearing in responses, which it shouldn’t have. We’re working on a fix.” The Guardian reported the same day that the issue had been fixed and ChatGPT was responding to queries using the name. Read The Guardian’s report.
That statement supports a mistaken flag as the company’s explanation. OpenAI did not identify the tool or describe its code, architecture, or interaction with the model, so the precise mechanism remains undisclosed.

Was the name blocked because of a privacy request?
The incident prompted speculation that someone named David Mayer had asked for the name to be removed. The Guardian reported that David Mayer de Rothschild denied making such a request and said he had never had contact with ChatGPT. OpenAI declined to say whether the glitch was connected to a right-to-be-forgotten request. The available reporting does not establish a privacy request as the cause.
A separate December 2024 article about similar name-related failures offered broader context, not a technical account of this particular incident. Harvard Law professor Jonathan L. Zittrain said OpenAI told him a very small number of names could be omitted because of privacy requests or to avoid persistent AI hallucinations. He also reported that OpenAI told The New York Times “David Mayer” had been added mistakenly. Those comments do not reveal how the Mayer failure worked. Read the related report.

What is known, and what remains unverified?
- Reported problem: Users encountered name-triggered failures in late 2024.
- Company explanation: OpenAI said a tool mistakenly flagged the name and prevented it from appearing in responses.
- Reported resolution: The Guardian said on December 3, 2024, that the issue had been fixed.
- Unconfirmed claims: The cited reporting does not establish a deliberate censorship scheme, a hard-coded blacklist, or a privacy request as the cause.
- Present-day behavior: The historical reporting does not show whether every current model, interface, region, or account responds normally.
The community discussion is useful for understanding what users saw and wondered about; it is not evidence of a specific filter design. Claims about intentional blocking or the exact implementation go beyond what the cited sources establish.

The Guardian reported the issue fixed on December 3, 2024. That does not establish current behavior across all ChatGPT versions and interfaces, and there is no live test here to confirm it. If you encounter a similar failure now, the old incident alone cannot tell you whether it has recurred or whether a separate problem is responsible.
