It includes simple message forms as well as fuller contact pages and lead forms with options such as service, budget, company, phone, or callback time. Choose by the information you need and the layout that fits your page—but plan to connect a backend or form service separately: the downloaded design does not send email by itself.
What the 43 templates cover
The collection ranges from compact forms for general enquiries to larger contact-page layouts with details such as maps, social links, and structured inquiry fields. Its first 20 entries are labeled Contact Form V01 through V20; other entries include Colorlib contact-form designs, Appointment Form V10, and Pricing Table V11 with a sales form. Each entry is presented with a live demo and a free download, according to Colorlib’s article, published September 28, 2026.

For a general message
A three- or four-field form is often enough when the goal is simply to let someone send a question. Avoid asking for details you do not need: every extra field adds friction for the person filling it out.

For qualifying service or agency leads
If you need to understand a prospect’s project before replying, look at V14, V19, and Contact Forms 3–5. Colorlib highlights these for budget or service choices. A company field, phone number, or callback-time option can also help route a more structured enquiry, but include such fields only when they support a real follow-up process.

For a more designed contact page
Layouts vary in how they arrange the form and supporting information: options include a standalone card, a split layout with contact details, a photo or gradient backdrop, map placement, and a popup. Decide whether the page needs those elements before choosing a design, since maps and imagery add implementation and maintenance work beyond the form itself.
How to choose a template
- Define the enquiry. Decide whether visitors are sending an open-ended message or providing details for a quote, appointment, or sales conversation.
- Choose the minimum useful fields. Match the form to that purpose. Use a short name, email, and message form for general enquiries; add service, budget, company, phone, or callback choices only when you will use the answers.
- Pick the page layout. Choose a compact card, split contact page, background treatment, map, or popup based on the surrounding page design and content.
- Check the implementation fit. Consider whether you need to adapt the form to an existing site, add an image or map, or connect a separate submission handler.
- Test the downloaded files in your project. Colorlib describes its main-list templates as HTML, CSS, and a small amount of vanilla JavaScript, and claims they work on their own or in Bootstrap 4, 5, or 6 projects. Those are the publisher’s compatibility claims, not independently tested results here.
What you need to make a form work
A downloaded template provides the front end: the fields and their visual presentation. To receive submissions or send email, connect its form to a server-side handler, a serverless function, or a hosted form service. Colorlib’s article names a PHP mail script and services such as Formspree, Basin, and Web3Forms as examples; it does not compare their current pricing, features, privacy terms, or availability. Choose and configure a submission method for your own requirements rather than assuming one is included with the template.
Rank #2

- Comes with secure packaging
- It can be a gift option
Colorlib puts the limitation plainly in its FAQ: “The templates cover the front end only.” In practice, inspect the form’s markup and configure its submission destination and handling method. Then test a real submission and verify that it reaches the intended inbox or system; seeing a successful-looking button interaction in the browser does not establish that delivery is working.

Validation, styling, and accessibility checks
Colorlib recommends browser-level HTML validation attributes such as required, type="email", and pattern. JavaScript can add inline error messages and prevent a full-page round trip, but validation in the browser is not a substitute for checking submitted data on the receiving side.
- Make sure each input has a corresponding, clearly associated label.
- Test empty required fields, malformed email addresses, and any pattern-constrained values.
- Load the CSS from your actual site while testing. Existing site styles can change how form fields appear or behave.
- Test keyboard navigation, error visibility, and the complete submission flow, not just the static demo.
License and reuse
Colorlib states that the templates use the CC BY 3.0 license for personal and commercial use and asks users to retain the credit link. Check the license notice in the specific downloaded files before removing attribution or redistributing a template; the roundup’s statement should not replace checking the terms attached to the files you actually use.
