Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteFor most teams starting a brain-tumor segmentation project, use nnU-Net as the baseline. It automates much of the pipeline configuration, giving you a reproducible starting point without requiring you to design every preprocessing and training choice yourself. Choose a custom 3D U-Net when a specific architecture or deployment constraint justifies that control—and compare it fairly against nnU-Net.
These are not strictly competing model types: a 3D U-Net is an architecture, while nnU-Net is a self-configuring segmentation method that can generate a 3D U-Net-like network.

What is the difference between 3D U-Net and nnU-Net?
A 3D U-Net is an encoder-decoder neural network that processes volumetric images and uses skip connections between its encoder and decoder. It is an architecture: by itself, the name does not specify the full data-processing and training workflow.
nnU-Net is a broader, self-configuring segmentation pipeline. It selects and configures components such as preprocessing, network architecture, training, and postprocessing for a dataset. In the BraTS 2020 study by Isensee and coauthors, nnU-Net generated a plain 3D U-Net-like network. That means a comparison between “3D U-Net” and “nnU-Net” may really compare a manually assembled pipeline with an automated one—not two entirely different architectures.

Results depend on more than the architecture name. Preprocessing, patch size, augmentation, target labels, ensembling, and postprocessing can all affect performance. Attribute a result to the complete tested pipeline.
How do they compare in practice?
| Consideration | nnU-Net | Custom 3D U-Net |
|---|---|---|
| Setup | Automates much of the pipeline configuration for the dataset. | Requires the team to choose and configure the model and workflow components. |
| Control | Provides a configured pipeline, with less need to specify every component manually. | Offers direct control over architecture and workflow choices. |
| Best reason to choose it | You need a credible baseline or want to reduce manual pipeline design. | A concrete model-size, deployment, or architecture hypothesis calls for custom choices. |
| Compute needs | Depend on the data and chosen configuration; the cited guide does not establish a minimum GPU or memory requirement. | Depend on the model and workflow you build; measure on the intended data and hardware. |
| What a performance result represents | The tested nnU-Net configuration and any additions, such as task-specific tuning or ensembling. | The tested custom model and its associated preprocessing, training, and postprocessing. |
What does the BraTS 2020 result show—and not show?
Isensee and coauthors reported that their nnU-Net approach placed first in the BraTS 2020 challenge. Its test-set ensemble results were Dice 88.95 for whole tumor, 85.06 for tumor core, and 82.03 for enhancing tumor. The corresponding HD95 results were 8.498, 17.337, and 17.805. These figures describe the authors’ final tuned ensemble, not an unmodified nnU-Net baseline or a universal head-to-head win over custom 3D U-Nets.
The study described 369 training cases and 125 validation cases. Validation labels were withheld from participants, and evaluation was conducted through the challenge’s online platform. The reported 3D configuration used a 128×128×128 input patch. Each figure belongs to that historical BraTS 2020 setup; the patch size is not a general requirement, and these data do not establish current hardware needs.

Challenge rankings also need context. The paper notes that selecting a model by mean-aggregated Dice or HD95 can produce a different result from selecting it under the challenge’s ranking procedure. The authors also say their experiments were not extensive enough to isolate which modifications caused the improvements. The result supports nnU-Net as a strong starting point, but it does not prove that nnU-Net always beats a custom 3D U-Net.
How should you compare the methods?
Run a custom 3D U-Net when you have a testable reason to change the model or workflow—for example, a target model size or deployment constraint, or a specific architecture hypothesis. Treat that as a project-specific choice, not an assumption that customization will improve accuracy.

Check BraTS labels and evaluation regions before training
BraTS evaluates regions that partially overlap: whole tumor, tumor core, and enhancing tumor. The annotated classes are edema, non-enhancing tumor/necrosis, and enhancing tumor. A model trained to predict annotated classes is not automatically being evaluated on exactly the same targets as a model trained directly on regions.
MIC-DKFZ’s nnU-Net documentation describes region-based training, which can target evaluation regions directly and convert region predictions back into label maps. The conversion order matters: place encompassing regions, such as whole tumor, before their subregions because later labels overwrite earlier ones. Check the dataset’s label meanings, target regions, and conversion order before comparing scores; a mismatch can invalidate an otherwise careful model comparison.

What should you measure for your project?
Neither a challenge patch size nor a generic hardware claim tells you whether a configuration will fit your setup. Measure runtime and memory with the actual modality count, image spacing, patch size, batch size, and intended inference workflow. NVIDIA’s nnU-Net for PyTorch guide documents a workflow that includes cloning the code, building a Docker image, preprocessing data in 2D or 3D mode, running inference, and evaluating predictions when labels are available. It does not establish a minimum GPU, memory requirement, or training duration.
Before settling on a method, check that its outputs match the intended label definitions, run it on held-out cases, and review errors by region. Record the configuration and evaluation procedure so that a later comparison can be reproduced.
Do benchmark results establish clinical readiness?
No. The BraTS results are research benchmark evidence; they do not establish that either method is approved for clinical use or can replace expert interpretation. A clinical deployment claim requires appropriate external validation and governance evidence beyond challenge performance.
A 2021 study of anatomical context added to a 3D U-Net on BraTS 2020 found no statistically significant overall Dice improvement from context masks or probability maps, but did report improvement for whole-tumor segmentation in its reduced-modality scenario. That study-specific result is a reminder that adding inputs or complexity does not guarantee an improvement; it does not settle the value of every contextual method.
