Crucially, the forms are front-end demos: submissions are not collected until you connect a backend or form service.

Choose by the shape of the questions and the moment you will ask them. The template descriptions below reflect the publisher’s stated features; they are not evidence that one design produces better completion rates than another.

Customer and product feedback
- V01 — Customer satisfaction: A post-delivery form pairs a 1–5 star rating made with native radio inputs with a 0–10 recommendation scale, optional comments, and a thank-you state. It suits order follow-ups, post-purchase pages, and general customer feedback.
- V02 — Conditional NPS follow-up: Respondents choose a score from 0 to 10, then see a follow-up whose wording, color, and label change for detractors, passives, and promoters. Consider it for in-app prompts, SaaS dashboards, or relationship surveys.
- V04 — Product feedback: An emoji reaction scale, feature checkboxes, notes, and optional screenshot upload support beta and in-app feedback. The upload demo includes file-type and size checks and a preview.
Longer or structured evaluations
- V03 — Multistep survey: Four pages include progress, back and next navigation, validation, retained answers, and a review page with edit links. Its side panel becomes a compact row on phones. It is suited to longer research or onboarding questionnaires.
- V05 — Event feedback matrix: Session rows are rated against shared rating columns, with a “Didn’t attend” option. On mobile, rows become labelled cards. Use it for conferences, workshops, and course sessions with several items to rate.
- V06 — Employee pulse: Five Likert statements appear with anonymity enabled by default. Optional identity fields are revealed when a respondent switches anonymity off, and an answered-item counter tracks progress. It is aimed at HR pulse checks, retrospectives, and engagement surveys.
- V09 — Course evaluation: Four range sliders have live value badges and spoken
aria-valuetext, alongside open questions and a completion recap. It fits course and training feedback.
Short polls and intercepts
- V07 — Instant poll: One question is followed by result bars and counts. The displayed counts are simulated in the script; this demo does not aggregate real votes. It is intended for blogs, newsletters, and community pages.
- V08 — Conversational survey: Respondents answer one question at a time, with keyboard shortcuts, a progress display, and up/down navigation. It is suited to short onboarding, lead qualification, or informal check-ins.
- V10 — Exit-intent feedback: A checkout feedback card offers reason checkboxes and an optional note, with focus-return behavior. It can be adapted for checkout, pricing, cancellation, or abandoned-cart research.
Do these survey forms need Bootstrap or jQuery?
No. Colorlib describes the templates as plain HTML, CSS, and vanilla JavaScript, without a required jQuery, Bootstrap file, or plugin. They can run on their own or be integrated into Bootstrap 4, 5, or 6 projects. That compatibility does not mean Bootstrap is bundled: if the surrounding site uses Bootstrap styling, you may need to adapt classes and layout to match its version and design.

Some implementations include mobile or accessibility-oriented details, such as V03’s compact phone navigation, V05’s labelled mobile cards, radio groups in V01 and V02, keyboard controls in V08, and spoken slider values in V09. These are publisher-described features, not independently verified accessibility or usability results. Check the markup and test your adapted form with your own devices and assistive technologies.

Where do the answers go?
They do not go anywhere by default. Colorlib describes the templates as front-end only: clicking through a demo or seeing a thank-you state does not show that a response has been stored. V07’s bars and counts are simulated rather than a live tally of submitted votes.
To collect answers, connect the form to a backend or a form-handling service, then validate submitted data on the server. For screenshot uploads, the server-side flow also needs to receive and handle the file; client-side previews and type or size checks alone do not store it securely. Test the complete submission path, including validation errors and the confirmation shown after a successful response.

What does “free” mean for use and attribution?
Colorlib states that its snippets use the Creative Commons Attribution 3.0 license (CC BY 3.0), allowing personal and commercial use while requiring the credit link to be kept. If you redistribute or substantially adapt a downloaded template, check the notice in the actual file as well as the published terms. A separate project’s license does not replace the terms for these downloads.
Where to find the templates and alternatives
Colorlib’s roundup, published September 25, 2026, describes all ten designs and their intended uses: 10 Free HTML Survey Form Templates. If you want a separate vanilla HTML/CSS example with an optional dynamic handling approach, FormBold maintains an HTML survey form example; it is a different project, with its own terms and handling setup. For Bootstrap-specific component examples, MDBootstrap provides a separate collection of Bootstrap 5 survey examples, including single-choice, ranking, and range patterns.
