A smartwatch is a capable general-purpose tracker, but the right device depends on the health question you need to answer. For glucose, that may mean a continuous glucose monitor (CGM) used for an appropriate purpose; for blood pressure, a validated arm-cuff monitor; and for other conditions, a device chosen with a clinician. A sensor reading or wellness score is not automatically a medical result.
Start with the question, not the gadget
Choose a device by the measure you need, the decision its reading will inform, and the person who will interpret it. A watch can be useful for broad activity and sleep tracking, but a device designed around one measurement may be a better fit when that measurement matters clinically.

- What do you need to measure? Glucose, blood pressure, oxygen, heart rate, sleep, and activity call for different tools.
- What is the intended use? A wellness tracker supports general health awareness; a medical device is intended for a defined monitoring or care use. Do not treat the former as a diagnosis.
- How often do you need a reading? Some devices provide continuous or frequent updates; others take a spot measurement. One pattern may not substitute for the other.
- Can you use and maintain it? Consider fit, availability, compatible phones or systems, replacement supplies, prescriptions, insurance, and whether a care team needs to help interpret results.
A 2024 practical guide to remote-monitoring device selection recommends weighing intended use, technical performance, monitoring capability, feasibility, availability, and cost. HHS likewise describes device choice as dependent on the condition and care context, with options that include blood-pressure devices, glucose meters and CGMs, oxygen monitors, sleep monitors, and wearables. HHS remote patient monitoring guidance explains the care context.

The FDA warned on February 21, 2024, that no smartwatch or smart ring had been authorized, cleared, or approved to measure or estimate blood glucose on its own without piercing the skin. The agency advised consumers not to buy or use devices making that claim: an inaccurate glucose result can prompt a harmful medication decision. The warning applies regardless of brand.

A watch or phone app that displays data from a separate authorized sensor is a different arrangement. The FDA’s sensor-based digital health technology list includes examples such as Dexcom Stelo and Dexcom G7 entries, but authorization for one product and intended use does not transfer to another. FDA says listed devices met applicable premarket requirements and had focused review of safety and effectiveness for their intended use; the list is not comprehensive and is periodically updated.
What a CGM measures—and what it does not
A CGM is a medical device attached to the arm or stomach. Its sensor measures glucose in interstitial fluid through a tiny tube below the skin, with readings updating every few minutes. Because interstitial-fluid readings can differ from blood glucose, some users may still need occasional fingerstick checks; ask the care team how often. CDC notes sensors commonly need replacement every 7 to 14 days. Insurance coverage depends on circumstances, a prescription may be required, and uncovered use can cost thousands of dollars a year. See the CDC overview of continuous glucose monitoring.
CGMs are not automatically right for everyone. Check the device’s intended population and indication, access requirements, ongoing supply costs, and whether a clinician should guide use. For example, Dexcom’s Stelo is indicated for adults 18 and older who are not on insulin; its page says users are not intended to take medical action based on output without consulting a qualified health professional. Stelo’s official site provides its current indication and use information.

For blood pressure, be cautious about cuffless readings
Cuffless devices—including watches, rings, patches, and fingertip monitors—may be convenient, but convenience is not evidence of clinical accuracy. In a December 11, 2025 statement, the American Heart Association said these devices are not proven accurate enough to diagnose high blood pressure or guide treatment decisions. Reliability may vary with exercise, sleep, daily activity, medication, arm position, skin color, and calibration timing. Its statement writing group says they should not currently be used to diagnose, track, or treat high blood pressure. Read the AHA statement summary.
If you need blood-pressure readings, consider an automatic upper-arm cuff monitor rather than assuming a watch can replace one. Confirm that the specific model is validated for its intended use and that the cuff fits; a clinician can advise on how and when to measure. Nearly half of U.S. adults—about 122 million—have high blood pressure, defined by the AHA as at least 130/80 mm Hg, which makes reliable measurement consequential, not a reason to trust an unproven sensor.

For general activity, heart-rate trends, and sleep habits, a smartwatch or fitness tracker can be useful if you understand that its wellness features do not necessarily establish a diagnosis. A device’s sensor list alone is not proof that every output is accurate or appropriate for medical decisions.

Wearable data can also be valuable in research without turning a consumer watch into a diagnostic instrument. NIH described a study of 40 adults who wore a commercially available smartwatch over many months. Combined with standard laboratory blood tests, more than 250,000 bodily measurements per person per day could reveal early infection signs through heart-rate changes. That finding describes research potential, not evidence that a watch independently diagnoses infection. See the NIH summary.

- Name the measure and decision. Be specific: “I need blood-pressure readings my clinician can use” is different from “I want a wellness trend.”
- Check intended use and authorization. Confirm the exact model, metric, population, and use—not merely the brand or product family. FDA’s list is a helpful reference, not a complete catalog.
- Match the measurement pattern. Decide whether you need continuous monitoring, frequent updates, or a periodic spot check, and ask whether that pattern suits the care question.
- Account for real-world use. Check comfort, fit, accessibility, compatibility, sensor or cuff replacement, prescription needs, and coverage. Include the recurring cost, not just the purchase price.
- Plan how results will be acted on. Ask a clinician whether readings should guide treatment, be confirmed another way, or simply be tracked for discussion. Do not change medication based on an unvalidated wellness reading.
GAO distinguishes medical wearables such as CGMs and prescription-based watches from wellness devices such as fitness trackers and some smart rings, which may track vital signs but are not intended to diagnose, treat, or prevent disease. That distinction is a useful guardrail: select for the actual care need, and do not ask a wellness gadget to do a medical device’s job. GAO’s 2026 report on wearable devices discusses the categories.
