An AI agent’s final message is an account of what it says it did, not proof that every step succeeded. Check the session trace for the relevant run, follow the recorded turns and tool events, and verify important changes against the system that was meant to change. A trace can show what the platform recorded; it does not, by itself, prove an external action committed.
Start with the claim, then find the run
Turn a broad statement such as “I updated the account” into a checkable question: which account, what field or state should have changed, and when? Then locate the session or run that the agent’s account refers to. Confirm its identity and time before drawing conclusions; a trace from a different run cannot substantiate the claim.

OpenAI’s API observability documentation describes session views and trace export, while its tracing guide explains the recorded events within a trace. These are product-specific capabilities, not a guarantee that every agent platform exposes the same views or captures every event. OpenAI API observability; OpenAI API tracing.
Follow the recorded events, not just the summary
Read the trace in sequence and compare it with the agent’s account. Look for the model’s relevant outputs, tool calls, recorded tool results, delegated work, and any visible status or failure. The tracing guide describes event details such as inputs, outputs, duration, and status. Availability and exact fields depend on the platform and its configuration.

- Expected step absent: The trace does not show the action the account describes. That weakens the account, though missing instrumentation or incomplete capture may prevent a firm conclusion.
- Call present, error or failure visible: The trace records an attempted step that did not report success. Check whether a later retry or alternate path appears before deciding what happened.
- Call and apparent success result present: The trace supports that the platform recorded a call and a result. It still does not independently establish that the external system reached the intended state.
- Delegated work involved: Follow the recorded delegated activity where available, rather than assuming the parent agent’s summary fully represents what the delegate did.
Keep the distinction clear: a tool-call event is evidence about the recorded interaction, not independent proof of an external side effect. This follows from what tracing records; the trace should not be treated as a tamper-proof or complete audit record.
Verify consequential changes in the affected system
For an action that matters—such as changing a record or sending an instruction—check the resulting state in the system that was meant to receive it. Use an authoritative read-back, transaction record, or other independent evidence available for that system. Match the result to the specific target and expected change; a success-looking tool response alone may not confirm that the intended change persisted.

If no independent check is available, state the limit plainly: the trace records an attempted action or reported result, but the resulting external state could not be confirmed. Do not turn uncertainty into a claim of completion.
Account for gaps in observability
Traces depend on the agent platform, instrumentation, and settings. A missing event can mean the action did not occur, but it can also mean the event was not captured or the trace is unavailable. Record whether tracing was enabled, whether the relevant run is present, and whether the logs expose enough detail to check the claim.

For the OpenAI Agents SDK specifically, its documentation says, “Tracing is enabled by default.” It also documents ways to disable tracing and says tracing is unavailable for organizations using OpenAI APIs under a Zero Data Retention policy. Other cloud platforms describe their own agent telemetry surfaces. Google Cloud documents telemetry for investigating agent behavior and troubleshooting; AWS CloudWatch describes agent spans with child spans for model and tool calls. A practical verification checklist
- Define the claim: Identify the exact action and expected outcome.
- Match the session: Find the trace for the run under review and confirm it is the right one.
- Inspect the sequence: Check relevant model outputs, tool calls, results, delegated steps, and visible errors or statuses.
- Compare account to evidence: Note missing steps, failed calls, and any statement that goes beyond what the trace records.
- Read back important outcomes: Verify consequential changes in the affected system or an authoritative record.
- Document limits: Note unavailable traces, disabled or incomplete capture, and outcomes that could not be independently confirmed.
