Why Your Enterprise RAG Pipeline Is Failing Before the First Query Runs

If an enterprise retrieval-augmented generation (RAG) system gives weak answers, the cause may be upstream of the question: the source document could be missing, misread, split badly, stale, or indexed with the wrong permissions. Trace one representative document from its source through extraction, chunking, indexing, and retrieval before changing prompts or models. A model cannot answer from evidence that the preparation path never made available.

What can fail before a user asks anything?

A RAG system has two connected paths. The preparation path ingests source material, extracts text and structure, divides it into retrievable units, attaches metadata, and builds or updates an index. The request path searches that index for relevant passages and supplies them to a model, which generates an answer. A defect in preparation can therefore be present long before the first query—and remain hidden until someone asks about the affected material.

Microsoft Learn’s RAG guidance, last updated August 21, 2026, notes that data preparation and indexing strategy affect answer quality: incomplete or irrelevant retrieved passages can lead to inaccurate responses. That does not mean every poor answer is an indexing problem. It means retrieval should be inspected before generation is blamed.

Trace one document through the pipeline

Choose a document that should support a known internal question. Follow its source version and identity through each stage, preserving the same document identifier where possible. This narrows the investigation: if the content disappears at extraction, there is no reason to tune ranking; if the right passage is retrieved, then prompt handling and generation deserve attention.

  1. Confirm source eligibility and lineage. Identify the repository and connector, who supplied or changed the item, whether the source is approved, when it was last updated, and whether the indexed copy corresponds to the intended version. Keep provenance and integrity records so an unexpected or modified source can be investigated.
  2. Compare the source with extracted content. Inspect parser output against the original, especially scanned pages, tables, figures, headings, lists, and multi-column layouts. Machine-readable text extraction may not preserve structure. Google Cloud’s Gemini Enterprise documentation describes OCR for non-searchable scanned PDFs and layout parsing for structural elements in supported formats.
  3. Inspect chunks in context. Read the actual text units that will be retrieved. Check whether a passage retains the heading or surrounding context needed to make it intelligible, whether tables remain understandable, and whether its source identifier and metadata are useful. Google documents layout-aware chunking that keeps content associated with a detected layout entity and optional ancestor headings to reduce context loss. These are design choices to validate against your corpus, not universal defaults.
  4. Verify processing and index contents. Check ingestion status and error records, confirm the expected item and its chunks are present, and inspect the index fields used by the application. A successful source connection does not by itself prove that the intended content was parsed, embedded, and indexed successfully.
  5. Check permissions on the retrieved unit. Compare source-system permissions with the metadata carried by each chunk. Test with identities that should have access and identities that should not. When source permissions change, verify that the indexed representation is refreshed. The source system should remain authoritative; the model should not decide whether a user is allowed to see a passage.
  6. Run retrieval without generation. For a small set of representative questions, inspect returned passages, source IDs, ranking, filters, and citation metadata before calling the model. This reveals whether relevant evidence is available and whether access controls are working.
  7. Inspect generation only when evidence is right. Compare the supplied context with the answer. If relevant passages are present but ignored or misstated, review prompt construction, passage limits, token budget, grounding instructions, and citation rendering. Grounding can reduce unsupported guessing, but does not guarantee a correct answer.
  8. Make the path observable and safe. Record stage status, source and chunk lineage, authorization decisions, retrieval results, and output attribution. OWASP’s RAG Security Cheat Sheet recommends observability across the pipeline and fail-closed behavior when retrieval or access control fails.

Where to look when the document disappears or degrades

Stage Typical defect What to inspect Corrective direction
Source onboarding Unapproved, altered, poisoned, or untraceable content enters ingestion. Source, uploader, approval, integrity record, and update history. Use approved-source workflows, integrity checks, and provenance records, as recommended by OWASP.
Extraction Scans, figures, tables, headings, or multi-column layouts are omitted or flattened. Compare original pages with parser output; inspect OCR text and detected structure. Match parsing to the file type; enable OCR for scanned PDFs and use layout-aware parsing where supported, as documented by Google Cloud.
Chunking Passages lose hierarchy, context, or attribution. Chunk text, boundaries, headings, source IDs, and table readability. Use coherent, structure-aware chunks and test heading inclusion and chunk size against representative questions.
Metadata and permissions A chunk lacks current tenant, owner, classification, or access information. Compare stored metadata with source-of-truth permissions; test both authorized and restricted identities. Carry relevant controls with each chunk and enforce them at retrieval time, following OWASP guidance.
Indexing Processing failures, missing documents, stale vectors, or unsuitable fields prevent discovery. Ingestion status, item lookup, index schema, and update history. Reprocess affected content and verify the fields and search modes used by the application.
Retrieval Query and index do not align, ranking is weak, filters exclude relevant passages, or results are incomplete. Retrieval-only results across representative questions, including rankings and filters. Review chunking, embeddings, search configuration, filtering, and reranking; Microsoft documents keyword, semantic, vector, and hybrid approaches.
Generation Relevant evidence is present, but the answer ignores or misstates it. Compare prompt context and generated claims with the retrieved passages. Bound the context, instruct the model to stay grounded, render citations, and evaluate the complete answer path.

Choose parsing, chunks, and retrieval for the corpus

Parser choice

Use digital text extraction when a file’s text is machine-readable and preserves what the application needs. Scanned or image-based PDFs may require OCR. Files whose headings, tables, or layout carry meaning may need layout-aware parsing, where supported. Check the formats and ingestion path your implementation actually supports; a parser’s availability in one product workflow does not establish support in every connector or federated-search setup.

Chunk design

Evaluate whether each chunk is coherent on its own, whether it retains enough context to answer likely questions, and whether the resulting passages are precise enough to retrieve without excessive token use. Layout-aware chunks and ancestor headings can help preserve structure, but the right boundaries depend on the documents and questions. Test alternatives with representative internal queries rather than assuming a vendor default fits the corpus.

Retrieval mode

Keyword search can matter when exact terms are important; semantic and vector approaches can help find conceptually related material; hybrid retrieval combines approaches. Microsoft Learn lists these options and warns that search configuration affects relevance. Compare exact-term handling, semantic relevance, filters, ranking, latency, and operating cost in your own workload. No single mode is established as best for every corpus.

Keep access control attached to evidence

Permission checks must apply to the chunks returned for a particular identity, not just to the original document at ingestion time. OWASP recommends storing classification, owner, roles, and tenant metadata with every chunk and enforcing access checks during retrieval. Log the identity and relevant metadata associated with returned chunks, and keep permissions synchronized with their source of truth.

This is both a reliability and security concern. Microsoft notes that uncontrolled access to source content can expose sensitive indexed material. If an authorization check or required retrieval step fails, do not silently pass uncertain evidence to the model; OWASP recommends failing closed.

Evaluate retrieval before evaluating answers

Build a compact diagnostic set of questions for which you know which documents and passages should be relevant. For each one, inspect whether the expected source is present, whether the returned passage actually supports the question, how it is ranked, and whether filters or permissions remove it. Then evaluate generated claims and citations against those retrieved passages. This separates three different problems: unavailable evidence, available but poorly selected evidence, and evidence that reaches the model but is used incorrectly.

Why this is also a security and operations problem

OWASP’s Cheat Sheet Series describes the risk succinctly: “RAG does not reduce risk — it redistributes it across the data pipeline, creating new attack surfaces at every stage from ingestion to generation to output.” A pipeline that cannot establish where content came from, what changed, who may retrieve it, and which passages supported an answer is difficult to debug and unsafe to trust.

The OWASP guidance emphasizes source allowlisting, integrity and provenance checks, chunk-level controls, retrieval-time authorization, observability, and fail-closed behavior. Treat those as operating requirements alongside relevance testing—not as safeguards that can be delegated to the model.
