AI agents often guess because generating a plausible continuation is the language model’s default operation, while searching requires a separate decision that can be miscalibrated, costly or poorly integrated. If an evaluation awards a point for a lucky answer but nothing for saying “I don’t know,” the system has a measurable incentive to answer even when evidence is missing. Reliable agents therefore need explicit search triggers, calibrated uncertainty and verification—not merely access to a web tool.
Why guessing is the model’s default
A language model learns statistical patterns in text. At answer time it predicts what wording is likely to come next; pretraining does not attach a verified truth label to every proposition it can generate. A fluent sentence can therefore be plausible without being supported.

This becomes unavoidable for some obscure or arbitrary facts. Microsoft Research’s Adam Tauman Kalai and Santosh S. Vempala write that, for facts whose veracity cannot be determined from training data, “hallucination is necessary for language models that satisfy a statistical calibration condition appropriate for generative language models.” Their result is tied to facts that may appear only once in training data, so better prompting cannot make every unknown recoverable from memory.
The practical implication is that confidence in wording is not evidence of confidence in the underlying fact. The agent needs a separate mechanism to recognize when its internal knowledge is insufficient.

Accuracy-only scoring rewards a risky guess
Suppose a benchmark gives one point for a correct answer and zero points for an abstention. If the model assigns any non-zero probability to being right, guessing has positive expected score; leaving the field blank guarantees zero. OpenAI describes this as the incentive structure of a multiple-choice test: strategic guessing can raise accuracy while increasing errors and hallucinations.
OpenAI’s SimpleQA example makes the trade-off visible. The percentages below are the reported outcomes for those systems on that example, not a universal ranking of model quality.
| System | Abstention | Accuracy | Error | What the figures show |
|---|---|---|---|---|
| gpt-5-thinking-mini | 52% | 22% | 26% | It declined frequently, limiting wrong answers but also reducing attempted answers. |
| o4-mini | 1% | 24% | 75% | It scored slightly higher on accuracy while producing far more incorrect answers. |
A leaderboard that reports only the accuracy column can therefore select the second behavior. A safer evaluation reports abstention, calibration and the severity of confident mistakes, rather than treating every unanswered question as a failure.

Searching is a separate policy decision
Giving an agent a search or retrieval API does not make it use that API. The system must first recognize that outside evidence is needed, select a tool, formulate a query, interpret the returned material and incorporate it without inventing unsupported details. A failure at any stage can produce an answer that sounds researched but is not grounded.
Why the trigger can be wrong
Prompt wording, examples and the selected execution trace teach the model when tool use is expected. The ACL Findings 2024 paper ProbeCal studies uncertainty calibration for tool-using agents and identifies prompt design and trace selection as sources of miscalibration. An agent may be too willing to search for easy questions, or too reluctant to search when its parametric memory is stale.
Cost and latency change the incentives
Search adds network delay, token use and sometimes a paid call. A model optimized for fast completion can learn that answering immediately is cheaper than checking. A retrieved page can also be noisy, contradictory or difficult to parse, so a poorly designed policy may decide that a quick guess is more reliable than a bad result.

Google Research’s 2024 evaluation covered more than 340 experiments. It found that few-shot tool assistance generally did not outperform no-tool baselines, with substantial variation by strategy and setup. For knowledge-seeking tasks, retrieving evidence and then using it to edit or verify an answer performed measurably better than relying only on tool calls during generation. The lesson is not that tools are useless; it is that tool exposure alone is not a workflow.
What retrieval-augmented generation fixes—and what it cannot
Retrieval-augmented generation (RAG) gives the model current or domain-specific documents, reducing dependence on stale information stored in model parameters. It can supply a policy document, a current product specification or a primary source that was unavailable during training.
Retrieval is not proof, however. Documents can be irrelevant, incomplete, outdated, low-authority or mutually inconsistent. The model can misread a relevant passage, cite a source that does not support the claim or answer confidently when the retrieved set is empty.

An AAAI 2026 study found that irrelevant retrieved documents could make retrieval-augmented models refuse questions they could have answered. It also found that improved refusal behavior did not necessarily produce better calibration or higher accuracy. A system may therefore become more cautious without becoming more correct.
The key distinction is knowledge access versus epistemic control. Search supplies evidence. A separate policy must decide whether evidence is needed, whether the retrieved material is sufficient and authoritative, how conflicts are resolved and when the only honest response is a clarification or abstention.
How to make an agent check its work
A dependable agent treats looking and verification as observable steps rather than an optional suggestion in a system prompt.
Best Value
- Classify the request. Mark questions as stable, time-sensitive, domain-specific, high-impact or underdetermined. Require external evidence for categories where model memory is likely to be stale or the cost of error is high.
- Make “look first” measurable. Log whether the agent searched, the query it issued, the tools selected and the reason for skipping them. A policy cannot be improved if tool decisions are invisible.
- Retrieve with coverage and authority checks. Prefer primary or official material, check publication dates and test whether the result actually addresses the question. Record empty, conflicting and irrelevant-result cases instead of silently proceeding.
- Build a claim–evidence map. Before finalizing, list each material factual claim and the passage that supports it. Remove claims with no support or label them as uncertain.
- Verify after drafting. Use retrieval to edit or fact-check the generated answer, a pattern that outperformed tool calls used only during generation in Google Research’s comparison. Check numbers, dates, units, qualifiers and whether a citation entails the exact statement.
- Handle uncertainty explicitly. Ask a clarifying question when the request is ambiguous, explain conflicts when sources disagree and abstain when the evidence is insufficient. The wording should change the user’s next action, not merely add a vague disclaimer.
- Evaluate the whole policy. Penalize confident errors more heavily than cautious uncertainty. Include tests with stale, empty, irrelevant and conflicting retrieval results, because refusal, calibration and accuracy can move in opposite directions.
How to compare agent designs
Whether an architecture uses a single search call, iterative retrieval or a separate verifier, compare the behavior below rather than counting tool calls.
| Axis | Question to measure |
|---|---|
| Trigger quality | Does the agent search when the question needs outside evidence, and avoid unnecessary searches? |
| Retrieval relevance and coverage | Do results contain the information needed, or only superficially related text? |
| Source authority and freshness | Are claims based on trustworthy, current material appropriate to the subject and region? |
| Tool-use calibration | Do stated or internal probabilities predict when a search will help? |
| Conflict and irrelevance handling | Does the system detect contradictory or unusable documents instead of forcing an answer? |
| Abstention behavior | Does it decline underdetermined questions without refusing answerable ones? |
| Latency and cost | What delay and resource use does verification add, and is that trade-off appropriate? |
| Auditability | Can an operator reconstruct the query, sources and evidence behind each material claim? |
The practical bottom line for builders
Hallucination is not simply a missing web plug-in. It results from a generator optimized to continue text, an evaluation scheme that may reward guessing and a tool-use policy that can misjudge when evidence is needed. Retrieval reduces reliance on stale memory, but only a measured workflow—search trigger, source assessment, claim-level verification and calibrated abstention—turns access to information into dependable answers.
