Track both what is done by the next retrospective and what is eventually completed; the first reveals whether the team’s cadence supports follow-through, while the second shows whether work was delivered after a delay. Neither percentage, on its own, explains why actions stall.
What an action-item completion rate tells you—and what it does not
Use a completion rate as a prompt for inspection, not a verdict on the team. A low rate can point to vague commitments, excessive scope, missing ownership, work that never entered the team’s normal planning, or dependencies outside the team’s authority. An unfinished item may also be active work rather than abandoned work.

There is no established representative, independent industry-wide completion rate in the available evidence. Two vendor sources report different figures using different populations and measures, so their percentages should not be combined or treated as a benchmark:

- TeamRetro platform data: On an undated first-party page accessed in 2026, TeamRetro reports approximately 73% eventual completion across hundreds of thousands of retrospective actions, approximately 90% completion for actions with a named owner and due date, and a median completion time of approximately six weeks. TeamRetro cautions that its users are a selected sample of people using its tool, not a cross-industry average. These are vendor-reported platform figures, not an independently established industry result. TeamRetro’s action-item data.
- Easy Agile survey: Its 2026 vendor report says 69% of surveyed teams implement less than 60% of retrospective actions before the next retrospective, while 61% rate retrospectives effective at identifying meaningful improvements. Full report details are gated. This survey result is not directly comparable with TeamRetro’s platform-event data. Easy Agile’s report.
Those figures can provide context, but they do not tell your team whether its own actions are appropriately scoped, controllable, or changing the underlying problem.

Define the rate before interpreting it
Record the cohort, denominator, observation window, completion rule, and treatment of cancelled, blocked, or escalated work. Without those choices, two rates with the same label may describe different things.
Choose a cohort and denominator
A practical cohort is all actions created in a specified retrospective, or across a clearly defined set of retrospectives. State whether the denominator includes every recorded action or excludes items formally cancelled or merged. Keep the original list and preserve status changes rather than quietly removing inconvenient items.
Set a fixed checkpoint and a completion rule
At the next retrospective, count how many cohort actions meet your definition of complete. A next-retrospective snapshot is useful for seeing whether work moves within the team’s cadence, but items still in progress will count as incomplete at that point. Separately record eventual completion so delayed delivery is not mistaken for abandonment. An eventual rate alone has the opposite weakness: it can hide how long the team waited.

- Minimalist Monochrome Design: Clean, modern design that complements any workspace while keeping you organized and focused.
Define “complete” in terms of the action’s intended deliverable or change, not merely that someone discussed it or moved a card. If an action is an experiment, decide in advance whether completion means running it, reviewing its result, or both.
Keep status categories distinct
- In progress: Work is actively moving, with a next step and owner.
- Blocked: Progress depends on a named person, team, decision, or resource; record the dependency and follow-up date.
- Escalated: The team lacks authority to make the change; record who received the escalation and when it will be reviewed.
- Cancelled or no longer relevant: Priorities or circumstances changed; record the reason rather than treating it as delivered.
- Complete: The agreed deliverable or change is done under the team’s stated rule.
Alongside each fixed-checkpoint rate, consider time-to-completion and the ratio of open to delivered actions. These show whether the team is finishing late, accumulating unfinished commitments, or closing work promptly. Interpret them against the same cohort and rules.
Diagnose why actions are not getting done
Review each unfinished action before creating new ones. Scrum.org recommends keeping improvement work visible, assigning a directly responsible individual, and reviewing the team’s state at the next retrospective. Use the review to find the failure point, not to assign blame. Scrum.org’s guidance on effective retrospectives.

- Fight Ink-bleeding & Great Size: The high-end 100gsm paper could prevent ink bleeding through or feathering, handle double-sided writing and most daily use pens pretty well. The office/business work notebook measures 7.5"x 10"(similar to B5 size), Generous size provides ample space to jot down your meeting notes.
- Each 160 Pages Per Book: Provide ample space for note taking & planning and with the date section at the top for tracking them. With 160 pages for meeting minutes, the manager notebook will cover more than half a year, even in daily use. Also provides index pages for organizing this office planner.
- Better Tool Drives Better Meetings: The hassle of organizing the chaotic meeting notes VS this professional meeting notebook. Definitely a step up! Everything is neatly zoned on each page makes it a breeze to fill them out and ensure all you need are accounted for.
- No clear accountability: Was one willing person responsible for making progress visible, even if several people would do the work? Shared execution still benefits from an explicit coordinator.
- No date or next step: “Next sprint” or “look into this” can conceal a lack of a concrete commitment. Add a due date or a specific review date and a first action.
- Too much scope: Could the work reasonably move within the team’s cadence? If not, reduce it to a first slice or a time-bounded experiment that tests an assumption.
- Invisible work: Did the action live only on a retrospective board, or was it made visible in the team’s normal sprint or work-planning system?
- Outside team control: Does another group, manager, or decision-maker need to act? Escalate with a receiving owner and review date instead of leaving the team with an unowned promise.
- Repeated topic: Has the same problem returned despite earlier actions? Check whether previous actions addressed a symptom rather than the cause, were completed without changing the condition, or were never reviewed.
Repeated topics are useful diagnostic evidence, but not proof that every team has the same problems. A peer-reviewed three-year longitudinal case study of retrospective records from one distributed Scrum organization found recurring discussion of estimation accuracy and task completion, bug fixing, and clarifying instructions. Its authors caution that exact quantities from this single organization should not be generalized. The longitudinal case study.

Make fewer, more actionable commitments
Choose one or two meaningful improvements the team can influence rather than producing a long list of promises. For each action, name a willing person to move it forward, set a date, define a result that can be inspected, and put it where the team already plans or tracks work. At the next retrospective, review it before generating new actions.
The Scrum Guide (2020), in the Sprint Retrospective section, says: “The Scrum Team identifies the most helpful changes to improve its effectiveness. If an action depends on another group, turn it into a controllable next step—such as making a request, identifying a receiving owner, and agreeing when to follow up—while recording the larger dependency as escalated. This makes the team’s own progress visible without pretending it controls the outcome.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Choose a tracking approach the team will maintain
A shared document, an existing work board, or dedicated retrospective software can all support the basic follow-through loop. A dedicated product is not established as necessary; choose based on whether the team can maintain a clear record and revisit it consistently.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →| Approach | What to check | Trade-off |
|---|---|---|
| Shared document | Can the team record owners, dates, status changes, and completion dates in one visible list? | Simple to adopt, but updates can be easy to overlook if the document is not part of regular work. |
| Existing work board | Can retrospective actions move into the team’s normal planning flow while retaining their origin and status history? | Keeps improvement work near delivery work, but may need a label or view to keep actions easy to find. |
| Dedicated retrospective software | Does it make ownership, dates, visibility between meetings, and review practical for this team? | May provide a focused workflow; the evidence does not establish that buying or adopting a dedicated tool raises completion. |
For any approach, keep the same cohort, denominator, window, and completion rule when comparing rates. TeamRetro’s platform sample and Easy Agile’s survey are not equivalent populations or measures, so comparing their figures as if they were a controlled head-to-head result would be misleading.

Use meeting format to improve discussion, not as a completion-rate shortcut
If participation is uneven or the discussion has become stale, changing facilitation may help the meeting surface useful improvements. A peer-reviewed action-research project implemented six retrospective games across six Scrum teams; participant feedback suggested games helped mitigate some issues, including lack of structure, dullness, complaints, and unequal participation. Responses differed, and the study does not show that a particular game increases action-item completion rates. The action-research study of retrospective games.
Scrum.org also identifies psychological safety, participation, a small number of impactful improvements, visible resolution status, recurring unresolved impediments, and stale, mechanically repeated formats as relevant to effective retrospectives. These practices support better inspection; they do not make a completion percentage a substitute for judging whether the team’s working conditions improved.
