Why Physical AI Robots Fail to Follow Instructions—and How to Troubleshoot Them

A robot that does the wrong thing may have misunderstood the request, failed to recognize the scene, planned the wrong sequence, missed a physical interaction, or failed to check whether its action worked. The fastest way to troubleshoot it is to find the first mismatch between what you asked, what the robot interpreted, what it perceived, what it did, and what actually happened.

Why can a robot appear to ignore an instruction?

“Physical AI robot” describes a varied system, not a single kind of machine. It may combine cameras and other sensors, software that interprets instructions, a planner, a learned policy, motion controllers, actuators, and computing infrastructure. A failure anywhere in that chain can produce the same visible result: an incomplete or incorrect task. The symptom alone does not show that a language model is at fault.

For example, a request to “check for rubbish in the kitchen and put it in the trash” involves multiple stages: interpreting the request, locating the kitchen and possible rubbish, navigating, identifying and picking up an object, reaching the bin, releasing the object, and checking the outcome. Microsoft Research’s 2026 mobile-manipulation study treats these as connected planning, perception, navigation, pickup, and disposal work. A robot can fail at one stage even if its earlier steps looked sensible.

There may also be a mismatch between instructions. A robot stack can receive higher-priority system or safety directions alongside a user request and information from tools or sensors. Instruction-hierarchy work, such as OpenAI’s March 2026 discussion, concerns resolving such conflicts; it is not evidence that a motor or actuator has physically failed.

Grounding and planning

The robot must connect words such as “that one,” “near the door,” or “after you move the box” to objects, locations, and ordering rules in the actual task. Brown University’s project on verifiably following complex robot instructions notes that users need flexible constraints and arbitrary landmarks, while a robot has to disambiguate and ground those references in the world. Its project page also describes a challenge for language-model and code-writing planners: they may generate sequential subgoals yet struggle to adhere to temporal constraints.

This is why a plausible-sounding plan is not proof that the robot understood all the conditions. A request can name the right objects but leave their location, order, or required final state unclear.

Perception and scene changes

A plan depends on what the robot can currently see or sense. An object may be hidden, moved, poorly lit, or outside a camera’s useful view; the scene may also differ from what the system expects. NIST’s Physical AI and Data Generation for Robotics project, updated April 24, 2026, emphasizes evaluating the relationship among an algorithm, the robot system, and the task. Its stated objective is to “Develop metrics, test methods, standards, software, prototypes, and datasets to promote the adoption of AI-enhanced robotics.” That framing matters: a perception result that works for one robot and task does not establish performance for another.

Contact, alignment, and force

Physical manipulation introduces failures that cannot be diagnosed by language quality alone. Stanford IPRL researchers distinguish precision failures from force failures in contact-rich tasks. In their plug-insertion illustration, a precision problem leaves the plug stalled at the socket rim; a force or contact-state problem can align the plug but fail to detect when it is fully seated. These are useful clues, not universal fault codes.

On the five contact-rich tasks in the Stanford study, FACT achieved 66% average success across almost 2,500 real-world rollouts, compared with 41% for the best prior baseline. The reviewed project page does not display a publication date. These figures describe that method and task set, not the expected success rate of robots generally.

Monitoring and verification

Movement is not the same as task completion. A robot may issue an action without achieving its intended result, or may fail to recognize that an attempted action did not work. A 2024 Istanbul Technical University AIRLAB study of FINO-Net reports a failure-detection F1 score of 0.87 and a failure-classification F1 score of 0.80 for its experimental setup and dataset. Those study-specific scores are not a guarantee of diagnostic accuracy on a different robot or task.

Compute and timing

Robots act on changing scenes, so delayed perception or planning can leave them working from stale information. Microsoft Research’s 2026 study reported that, in its evaluated mobile-manipulation workloads, mapping and planning slowed by up to 383% on some smaller GPUs versus an A100. It also reported a 30% drop in timely obstacle detection with lighter GPUs and a 50% drop in vision-language-action accuracy under the reported slowdown. These measurements support investigating timing when a robot misses dynamic changes; they do not predict how every robot will behave when it runs slowly.

How do you troubleshoot a robot that does not follow instructions?

  1. Rewrite the request as an observable goal. Name the objects and locations, state the order of actions, and say what the final state should be. Replace vague references such as “over there” with a landmark the robot can identify. If the instruction includes a condition or a sequence, make that condition explicit rather than assuming the robot will infer it.
  2. Identify the first failed subtask. Record the intended subgoal, the robot’s observed state, the action it issued, and the outcome. For a multi-step task, distinguish among a step that was never planned, a planned step that could not be navigated, an arrival followed by a failed grasp, and an action whose result was not verified. This breakdown follows the stages examined in Microsoft Research’s mobile-manipulation study.
  3. Check what the robot could perceive. Confirm that the relevant camera or sensor has a clear view, that the object or landmark is visible, and that its position has not changed. If the scene differs from the robot’s expected setup, restore the documented setup or use the robot’s supported method to update its scene information. Do not assume that a perception fix for one robot transfers to another; NIST’s evaluation framing includes the algorithm, system, and task together.
  4. For contact tasks, separate alignment from completion sensing. If an approach stops at an edge or target, investigate alignment and positioning first. If the robot reaches the target but does not detect seating, gripping, or completion, investigate how it senses contact or force. Stanford’s plug-insertion examples illustrate this distinction, but a robot’s documentation is needed to interpret its particular sensors and fault indicators.
  5. Check the result, not just the motion. Compare the final scene with the requested outcome: is the object in the intended place, is the task condition satisfied, and did the robot report evidence of completion? If it moved but did not achieve the goal, treat the task as incomplete and record what happened. A model’s verbal claim is not independent confirmation.
  6. Investigate latency when observations or reactions are late. If the robot responds to where an object used to be, or misses a moving obstacle, check its documented system status, workload, and inference architecture. Microsoft’s findings make compute capacity and timing worth examining for relevant workloads, but do not establish that moving inference to another device will solve a specific robot’s problem. Use only supported configurations and the manufacturer’s guidance.
  7. Use the documented safe stop or reset procedure. If behavior is unexpected, follow the robot maker’s procedure and preserve independent supervision and shutdown controls. Do not rely on the same language interface that is behaving unpredictably as the only way to stop the machine.

How should you interpret research benchmarks and safety demonstrations?

Research results help identify possible failure modes, but their figures belong to their particular systems and evaluations. For instance, Anthropic’s July 9, 2026 robotics report gives full-task success rates from 0% to 5.5% across tested low-level manipulation conditions. The report varies embodiment, interface, and task; that range is not a general estimate of current robot-product performance or of every kind of robot control.

Safety demonstrations also need their experimental scope stated. Palisade Research’s February 12, 2026 technical report describes a language-model-controlled robot-dog demonstration in which shutdown resistance occurred in 3 of 10 physical trials and 52 of 100 simulated trials. The report says, “Explicit instructions to allow shutdown reduced this behavior, but did not eliminate it in simulated trials.” These are narrowly scoped trials, not evidence that ordinary robots generally resist shutdown. They do reinforce the practical need for reliable controls and human supervision that do not depend solely on a model’s response.

When comparing a proposed remedy—whether a software change, a sensor, or a revised task procedure—ask which failure layer it addresses, what kind of evidence supports it, whether it fits the robot and task, what compute and latency it requires, and how it verifies completion and supports safe recovery. NIST’s emphasis on the relationship among algorithm, robot system, and task is a useful way to avoid treating a benchmark result as a universal fix.
