Microsoft 365 Copilot can produce answers that sound certain but are wrong, incomplete, or out of date. Its citations and connections to work or web information can help you trace an answer, but they do not certify that the wording is accurate. Before relying on a consequential response, check its claims against the original sources and look for missing context.
Why Microsoft 365 Copilot may give a wrong answer
Copilot generates responses from information available to the particular experience and prompt. Microsoft says its output may be inaccurate or out of date, and its validation guidance warns that polished writing can still be wrong, incomplete, or risky to use. There is no general error-rate figure in the cited Microsoft guidance that can predict whether a particular answer is reliable.

It may not have the right information
Depending on the experience, settings, and prompt, Copilot may use work content such as files, email, chats, and meetings; public web information; content you attach; or a combination. Work or school grounding depends on license and enabled capabilities, and Copilot can reference only organizational content you are authorized to access. If relevant accessible sources are unavailable, it may give a more general response. Administrator settings can also affect source availability, including web search. Microsoft explains these conditions in What information does Copilot use to answer my prompt? and How grounding works with a work or school account.

For example, a work account may not surface a policy document you cannot access, while an answer without work-data grounding may not reflect your organization’s internal rules. In the described experience, Work IQ being off means Copilot answers without referencing Microsoft 365 work data. Available modes and controls vary, so check the options in your own account rather than assuming all users see the same sources.

Its sources may be stale or poorly selected
An answer can inherit outdated information from an old file or page. If you ask about a policy, product, or decision, specify the current authoritative source and check its date and relevant section. When several similar files exist, name the one you mean. This reduces ambiguity but cannot guarantee that Copilot interprets it correctly.
A summary can distort or add to its sources
Combining material from multiple sources can introduce errors, and a generated summary may include information that is absent from its inputs. A citation may point to a real source without supporting the exact sentence beside it. Check for unsupported additions, altered meaning, and qualifications that disappeared in the summary. Microsoft Learn describes ungrounded generated content as a known large-language-model risk in Data, Privacy, and Security for Microsoft Copilot.

The prompt or answer may omit important context
A prompt that leaves out a date, audience, jurisdiction, exception, or constraint can lead to a response that is plausible but unsuitable. The answer itself may also overstate what its sources establish. Smooth wording is not evidence that assumptions, dependencies, and exceptions have been handled.

Microsoft Support’s validation guidance organizes the review around four checks: Source, Verified, Context, and Resilient. Use them in that order, giving the most attention to claims that would matter most if wrong.
Rank #3
- Source: Open the inline citations and the Sources list or pane. Follow each important claim to the original file, message, data, or web page. Compare the source passage with Copilot’s wording: does it support the claim, and has Copilot preserved its caveats? Microsoft explains how to review sources in Control and review sources of Microsoft Copilot Chat’s responses.
- Verified: Independently confirm high-impact details, especially names, dates, numbers, and recommendations. A plausible value in a cited response is not enough; check it in a trusted source or the original record.
- Context: Look for assumptions, exceptions, dependencies, and risks the answer did not mention. Ask whether including them would change the conclusion. A concise summary can omit context that matters to a decision.
- Resilient: Consider alternate interpretations, different audiences, and edge cases that could make the response misleading or incomplete. You can ask Copilot to identify gaps or challenge its own answer, but treat that critique as a lead to verify—not as independent proof.
Microsoft’s full checklist is in Validate Copilot output before you act on it.
How much verification is enough?
Match the effort to the potential consequences. Check especially carefully when an answer will guide a decision, go to customers or leadership, combine several sources, or inform time-sensitive or difficult-to-reverse work. For a low-stakes draft, a quick source check may be adequate; a recommendation that affects people, money, compliance, or operations calls for independent confirmation of the material facts and their context. The person using the output remains accountable for acting on it.

Microsoft also describes evaluation of groundedness, response quality, and citation relevance, including outcomes such as unsupported claims, missing or irrelevant citations, and inconsistent results. That is evaluation context, not a promise that any individual response is correct; see Application card: Microsoft Copilot (for organizations).

Do not forward or act on a response until you have corrected or replaced the unsupported information with verified material. Feedback can report a problem, but it does not substitute for checking the answer before use.
