Why LLM Token Usage Spikes and How to Troubleshoot It

A sudden increase in LLM token usage does not necessarily mean that users typed longer prompts or that the model returned more visible text. API totals can rise when conversation history, retrieved documents, tool definitions, images, audio, generated tool calls, reasoning tokens, cache writes, repeated calls, or automatic retries are added to a request. A larger context-window limit only raises capacity; it does not consume tokens until your application sends more content.

Find the cause by starting with the usage metadata attached to each response, then compare the fully rendered request and model settings with a known-good request. Verify cache reads and writes, inspect tool traces and retry logs, and compare the same model, endpoint, fields and time window. The workflow below separates input, output, cache, tool, thinking and retry effects so you can reduce usage without relying on a rough character or word count.

What a token spike actually means

A token is a unit used by a model’s tokenizer; it is not the same as a word, character or message. The API’s reported total is an accounting result for a particular request, model and endpoint. It may include categories that are not visible in the text shown to a user.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Visible text is only one part of the request

The input can include system and developer instructions, the complete conversation state, retrieved passages, examples, JSON schemas, tool definitions and metadata. An application may display only the latest user message while silently resending all of that context. OpenAI’s token guidance explains why a plain-text estimate can differ from the tokens counted for the complete API request; use the provider’s current counting method rather than counting characters in a UI textbox (OpenAI’s token explanation and token-counting guide).

Multimodal content can change accounting

Images and other non-text parts of a request can contribute to input usage even when the visible prompt is unchanged. Gemini’s documentation describes token accounting for text, images and other supported content, as well as separate input, output, thinking, cached-content and tool-use categories (Gemini token documentation). A text-only counter therefore cannot audit a multimodal request.

Generated structure, tools and thinking may be separate

Some models generate structural content or tool-call arguments that are not displayed as ordinary prose. Providers may report thinking or tool-use tokens separately, or include them in a total without presenting them as visible answer text. Treat the response’s usage object and call trace as authoritative for that request; do not infer the total from the final paragraph alone.

Context limits are not automatic consumption

Increasing a model’s context allowance does not by itself increase usage. The submitted history, documents, examples and tool schemas determine the input for each call. OpenAI’s conversation-state guidance is useful when checking what your application actually resends (conversation state documentation). A larger allowance can, however, make it easier for an application to retain more history before anyone notices the growth.

A request-level troubleshooting workflow

Run these checks on one normal request and one request from the spike. Preserve request IDs, timestamps and model names so that every comparison refers to the same accounting context.

1. Capture the complete usage breakdown

  1. Store the provider response’s input and output counts, total count, model, endpoint, request ID and timestamp.
  2. Record cached-input or cached-content reads and cache-write counts when they are available.
  3. Record thinking and tool-use counts separately instead of folding them into a single application metric.
  4. Store status codes and error details alongside successful responses; a failed attempt can still be followed by another billed attempt, depending on the provider and your application.

Field names differ between OpenAI Chat Completions and Responses, and between providers. Do not build a parser that assumes one universal prompt_tokens or total_tokens field. Keep the provider’s raw usage object for later audits, then map its documented categories into your own internal schema.

2. Diff the rendered requests, not the source template

Serialize the exact payload sent over the wire for a baseline and a spike, with secrets removed, and compare:

  • system and developer instructions, including dynamically inserted policy text;
  • conversation history and any duplicated turns;
  • retrieved documents, search results, file contents and examples;
  • tool declarations, function descriptions, parameter schemas and allowed tools;
  • images, audio or other attached modalities;
  • maximum output settings, reasoning controls and response-format schemas;
  • model, API endpoint and any conversation or cache identifiers.

Log both item counts and byte sizes for retrieved data, but use provider token counts for the final comparison. A compact source file can expand substantially after templates, history and retrieval results are rendered.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Check whether the cache was read or rewritten

For repeated prompts, inspect cache-read and cache-write fields on every response. A stable-looking prompt is not proof that a cache entry was reused. OpenAI describes prompt caching as reuse of a matching prefix and provides diagnostics for checking cache behavior (prompt caching and cache diagnostics). Anthropic likewise documents cache-usage fields; changing a prefix block can prevent a match (Anthropic prompt caching). A cache write or a sudden fall in cache reads can make otherwise similar requests look much more expensive.

4. Trace tools and repeated calls

Build a per-request call trace containing the initial model call, every tool invocation, each tool result and every follow-up model call. Look for a tool being called repeatedly with unchanged arguments, a planner that never receives the expected result, or a fallback endpoint that is unavailable. Google’s troubleshooting guide lists repetitive tool calls as a possible failure mode when context is lost or an endpoint cannot be used (Gemini troubleshooting guide). Count tokens across the whole trace, not just the first response.

5. Separate retries from legitimate traffic

Join application logs, SDK logs and provider responses by request ID or a generated attempt ID. For each attempt, capture the HTTP status, error body, retry headers and elapsed time. OpenAI recommends honoring a valid Retry-After; when it is absent, use bounded exponential backoff with jitter, and check whether the SDK already retries before adding another loop (OpenAI rate-limit guidance). Anthropic documents a retry-after header on 429 responses and warns that sharp usage increases can trigger acceleration limits (Anthropic rate limits). Never continuously resend an unsuccessful request without a cap and a stop condition.

6. Compare like with like

Hold the model, endpoint, usage fields, conversation state, cache configuration and measurement window constant. A provider dashboard or quota meter may use a different definition from a field in an individual response. Use the model’s current documentation to interpret each number, and do not compare an input-only field from one provider with a total field from another.

Provider differences that affect diagnosis

Provider documentation What to inspect Important qualification
OpenAI token counting Input and output usage in the endpoint’s response object; complete rendered messages, tools and multimodal parts. Chat Completions and Responses expose differently named fields. Use the field definitions for the endpoint and model you actually call.
OpenAI cache diagnostics Cache reads, cache writes and prefix placement. Caching depends on a matching prefix; a small change early in the prefix can eliminate reuse.
Gemini token documentation Input, output, thinking, cached content, tool-use and total categories. These categories are provider-defined; do not assume their totals map directly to another provider’s fields.
Anthropic prompt caching Cache creation and cache-read usage fields, plus the exact prefix blocks. Changing a prefix block can prevent a cache match, even if the user-facing question is unchanged.
Anthropic rate limits 429 responses, retry-after, and acceleration-limit behavior. A sudden traffic increase can produce rate-limit pressure that causes additional attempts in your application.

The table is a diagnostic map, not a conversion chart. Token fields and definitions are not interchangeable across vendors.

Common causes and how to confirm each one

Conversation or retrieval growth

Plot input tokens per request against conversation-turn count and retrieved-document count. If both rise together, enforce a history policy: retain only turns needed for the task, summarize older turns once, and cap the amount of retrieved material before it enters the prompt. Keep the original documents outside the model context when a later tool can fetch a small, relevant excerpt.

Duplicated instructions or data

Template composition can insert the same policy, schema or document more than once. Hash each rendered section and log its character length and token count. Duplicate hashes in a single payload are a strong signal to fix the assembly code rather than tune the model.

Output or reasoning expansion

Compare output and thinking categories separately. A spike confined to output usually indicates a changed maximum-output setting, a prompt that asks for longer structured content, or a model behavior change. A spike in thinking or tool-use fields requires checking the model’s current controls and the call trace; shortening visible prose will not necessarily reduce those categories.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Multimodal attachments

Compare requests with and without each image, audio clip or file. Record dimensions, duration or size where the provider exposes them, and avoid attaching the same asset on every conversational turn. If an image is needed only once, store its interpretation and send the compact result on later turns.

Cache misses and writes

Compare cache-read and cache-write counts before changing prompts. Put stable instructions, schemas and tool definitions in the cacheable prefix, and keep volatile user data later when the provider’s caching model supports that arrangement. A cache write is not evidence of a successful read on the next request; verify both events.

Tool loops and fan-out

Set a maximum number of tool rounds and log the reason each round started. Detect identical tool names and arguments, empty tool results, and calls to disabled endpoints. Return a clear error to the model or terminate the run instead of allowing an unbounded loop.

Retries and duplicate submissions

Use an idempotency or correlation key where the API supports one, and make the application distinguish an intentional retry from a new user request. Cap attempts, apply exponential backoff with jitter when instructed, and stop on non-retryable errors. A queue that redelivers a job after a timeout can create duplicates even when the user clicked only once.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reducing usage without breaking answer quality

  1. Measure first. Establish a per-request baseline for input, output, cache, thinking and tool categories. Set alerts on changes in each category rather than on total tokens alone.
  2. Control context. Cap history and retrieval results, remove duplicate instructions, and summarize stale turns. Keep a separate audit store if you must retain the full conversation.
  3. Stabilize cache prefixes. Keep durable instructions and schemas byte-for-byte stable where the provider’s cache requires a matching prefix. Put changing user text after that prefix and verify cache reads.
  4. Constrain generation. Set an output ceiling appropriate to the task, request a concise schema when prose is unnecessary, and avoid asking for hidden intermediate work that your application does not use. Confirm how the selected model reports reasoning or structural tokens before assuming a ceiling covers them.
  5. Make tools deliberate. Offer only tools needed for the current task, narrow their schemas, validate arguments, and enforce a round limit. Cache or compress large tool results before sending them back to the model.
  6. Handle media economically. Attach an asset only when it changes the decision, and pass a stored summary on later turns when possible.
  7. Harden retries. Honor provider retry headers, account for SDK defaults, use bounded backoff and jitter, and record every attempt. Never hide retries inside a generic success metric.
  8. Compare alternatives carefully. When changing a model or endpoint, run the same representative requests and compare the documented usage categories, latency, quality and failure behavior separately.

Instrumentation that makes the next spike explainable

A useful usage record has one row per model attempt, not one row per user conversation. Include:

  • timestamp, environment, service and deployment version;
  • provider, model, endpoint and region when applicable;
  • conversation, job, request and attempt IDs;
  • input, output, total, cached-read, cache-write, thinking and tool-use counts as reported;
  • number of messages, retrieved items, tool definitions, tool rounds and multimodal parts;
  • output-limit and reasoning-related settings;
  • HTTP status, error type, retry-after value, retry number and final outcome;
  • a redacted hash of each major prompt section so changes can be detected without storing secrets.

Build dashboards with at least four views: tokens per successful request, tokens per user task including all tool rounds, cache-read ratio, and retry rate. Alert on a deviation from a rolling baseline and investigate the category that moved first. Keep raw provider responses for a bounded retention period so a later field-definition change does not erase the evidence needed to reconcile totals.

Symptoms, tests and fixes

Symptom Fastest confirming test Likely fix
Prompt counter is low but API input is high Diff the wire payload against the text counted locally. Include history, tools, schemas, retrieved data and media in the audit; use provider counting for the final figure.
Total rises while visible answers stay short Separate output, thinking, tool-use and structural categories. Adjust model controls, output limits or tool flow based on the category that increased.
Repeated requests have unpredictable totals Inspect cache-read and cache-write fields and compare prefix hashes. Stabilize the cacheable prefix and remove accidental early-prefix changes.
One user action produces many attempts Join application, SDK and provider logs by attempt ID. Cap retries, honor retry headers, and eliminate duplicate queue delivery or nested retry loops.
Usage climbs with no prompt-code change Compare retrieved-data size, conversation length, attachments, model version and traffic rate. Apply context limits, investigate media and retrieval growth, and check for acceleration-limit retries.
Tool-call count keeps increasing Review the call trace for identical arguments, empty results or unavailable endpoints. Validate tool results, remove unusable tools and enforce a maximum round count.

When a dashboard and a response disagree

First check whether they cover the same time window, project, model family and endpoint. Then check whether one includes retries, cache writes, tool calls or failed attempts while the other counts only completed responses. Finally, compare the dashboard’s documented unit with the response field’s definition. If the discrepancy remains, preserve the raw response, request ID and dashboard export for the provider’s support process instead of applying an arbitrary conversion.

Bottom line

Treat a token spike as an observability problem before treating it as a prompt-writing problem. The reliable sequence is: read the provider’s per-response breakdown, diff the fully rendered request, verify cache behavior, trace tools and retries, and compare equivalent models and windows. Once the first category that changed is known, reduce context, stabilize cache prefixes, constrain tool and output behavior, or fix retry logic specifically—without guessing from visible text length.
