Why Freezing Damages Red Blood Cells—and How Blood Banks Prevent It

Freezing can injure red blood cells because ice removes water from the liquid around them, concentrating salts and other solutes. The cells shrink from osmotic dehydration, their membranes experience changing mechanical and permeability stresses, and additional damage can occur during thawing or exposure to cryoprotectant. Ice crystals are only part of the explanation.

Blood banks reduce that injury with validated processing: glycerol is added before storage at −65 °C or colder, the unit is thawed under controlled conditions, and glycerol is washed out before transfusion. The resulting deglycerolized component is used mainly when a rare antigen profile or another clinical circumstance makes ordinary red-cell units unsuitable.

What happens to a red blood cell as it freezes?

A red blood cell is mostly water enclosed by a flexible membrane. In a blood component, that water is shared with a surrounding solution containing sodium, chloride, proteins and other dissolved substances. As temperature falls, some of the extracellular water becomes ice. Ice is made largely of water, so the remaining liquid contains a higher concentration of everything that did not freeze.

Ice concentrates the surrounding solution

The concentration effect is the central physical problem. The liquid channels between growing ice crystals become progressively saltier and more chemically concentrated. Water moves out of the cells toward that more concentrated environment, and the cells shrink. D. E. Pegg and M. P. Diaper described slowly frozen erythrocytes this way: “When cells are frozen slowly in aqueous suspensions, the solutes in the suspending solution concentrate as the amount of ice increases; the cells undergo osmotic dehydration and are sequestered in ever-narrowing liquid-filled channels.”

That description also explains why “the cells were kept cold” is not a sufficient safety argument. Cold storage without a validated cryopreservation method does not prevent the solution around a cell from changing as ice forms.

Dehydration and membrane stress occur together

Cell shrinkage changes membrane tension, surface-area-to-volume relationships and the distribution of membrane proteins. At the same time, concentrated solutes can disturb proteins and lipids, while the membrane’s permeability changes with temperature and solution composition. When the cell later takes up water during thawing, it may not return to its original shape or leak-free state. Hemoglobin released from damaged cells is measured as free hemoglobin and is one of the quality concerns for a thawed component.

Ice is not the only cause of injury

Freezing injury is multifactorial. Ice-related effects, osmotic and solute changes, membrane permeability, the rate and temperature of freezing, exposure to the cryoprotective solution, thawing conditions and glycerol removal can each influence post-thaw recovery. The 1988 study by Pegg and Diaper linked injury to solute concentration and changes in the surrounding environment; it did not show that ice crystals alone account for every damaged cell.

Why laboratories add glycerol before freezing

Glycerol is a cryoprotective agent. In a validated red-cell process, it reduces some of the damaging effects of water movement and high solute concentrations during freezing. It does not make cells invulnerable, and the amount, mixing, temperature and timing are controlled by the laboratory’s approved procedure and the equipment manufacturer’s instructions.

Glycerol creates a second processing requirement: it must be removed after thawing. The AABB Circular of Information (June 2024) states that “the glycerol must be removed from the thawed component before it is infused.” Infusing a highly glycerolized component directly could expose circulating blood to a major osmotic shift and increase the risk of red-cell lysis.

How a blood bank prepares a frozen red-cell unit

Exact volumes, centrifuge settings, wash cycles and device parameters belong in a validated blood-bank procedure, not a consumer recipe. The overall sequence is nevertheless useful for understanding where protection and risk occur.

1. Select and qualify the component

The laboratory confirms the donor unit, blood group and antigen typing, checks its suitability for the intended patient and verifies that the unit and its attached records meet local release requirements. Frozen storage is particularly valuable for unusual red-cell antigen phenotypes, because compatible units may be difficult to locate later. A unit is not frozen merely because freezing is available; the expected clinical value has to justify the additional processing and post-thaw handling.

2. Add glycerol under controlled conditions

Glycerol is introduced before freezing using a validated method. AABB’s Circular describes frozen red blood cells prepared by adding glycerol before storage at −65 °C or colder. Mixing must be controlled so that cells are exposed evenly, and the process must avoid excessive mechanical stress or temperature shock. Centrifugation has traditionally been used for glycerol addition and removal; commercial cell washers are common professional equipment in U.S. practice.

3. Freeze and store the unit

The unit is frozen and maintained at the validated temperature. The −65 °C-or-colder figure is a component-description requirement in the AABB Circular, not a recommendation for a household freezer. Blood banks use monitored, alarmed storage and documented handling procedures because a temperature excursion can affect the product even when the bag still appears intact.

4. Thaw the unit when it is needed

Thawing is controlled to limit uneven warming and prevent additional osmotic or thermal stress. The unit’s labeling, the facility’s procedure and the equipment instructions determine how thawing is performed. A thawed unit is not automatically ready for infusion: it still contains glycerol and requires deglycerolization.

5. Remove glycerol by progressive washing

Deglycerolization uses a series of washes with progressively lower sodium-chloride concentrations. The final red-cell suspension is placed in 0.9% sodium chloride, with or without small amounts of dextrose, as described by the AABB Circular. The gradual change is important: removing glycerol abruptly would expose cells to a rapid osmotic transition and could cause hemolysis.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Washing also removes much of the plasma and other soluble material. That can be clinically useful for a patient who has previously experienced a severe allergic transfusion reaction, although the indication and product choice remain clinical decisions.

6. Inspect, test and release

The laboratory checks the finished component using its validated quality-control program. Relevant checks can include appearance, volume, cell recovery, free hemoglobin and residual glycerol, depending on the applicable procedure and regulatory requirements. The AABB Circular describes deglycerolized units as containing 80% or more of the red cells from the original unit. That is a product recovery description, not a promise that every individual unit will have identical recovery.

Open and closed post-thaw systems have different time limits

The system used during processing changes how long the component may be stored after thawing. These limits must be read together with the unit label, the current controlled Circular, local procedures and the manufacturer’s instructions.

Processing path Post-thaw handling described by AABB Why the distinction matters
Open system Transfuse within 24 hours after thawing. Opening the system introduces a contamination-control constraint, so the allowable interval is short.
Closed system Store at 1–6 °C and transfuse within a two-week interval, subject to manufacturer instructions. The closed pathway supports a longer refrigerated interval, but the validated product and device instructions still govern.

These are not interchangeable options. A facility cannot gain the closed-system interval simply by placing an openly handled unit back in a refrigerator, and it cannot extend an interval beyond the labeling or validated procedure.

How quality standards address post-thaw damage

AABB standards and the Circular describe preparation methods intended to minimize post-thaw hemolysis and preserve usable red cells. The commonly cited recovery benchmark is at least 80% of the pre-deglycerolization red-cell content in the Circular’s description of deglycerolized units. A proposed 35th-edition draft also used mean recovery of at least 80%, but draft wording is not operative standards text.

The current AABB Standards for Blood Banks and Transfusion Services are the 35th edition, effective April 1, 2026. Compliance decisions should use the paid current standards text and the facility’s approved procedures, not a draft circulated for comment. AABB identifies the June 2024 Circular as a controlled document formally recognized by the U.S. Food and Drug Administration on September 25, 2024 as an extension of labeling; it must not be casually edited or replaced with an informal summary.

What laboratories look for when a unit may have failed

  • Unexpected hemolysis: free hemoglobin or a visibly abnormal supernatant can indicate membrane damage.
  • Low cell recovery: excessive cell loss during freezing, thawing or washing can make the unit unsuitable.
  • Residual glycerol: inadequate washing leaves more cryoprotectant than the validated process allows.
  • Temperature or timing deviation: a storage excursion, delayed washing or missed transfusion window requires review against the product’s instructions.
  • Container or seal problem: a compromised bag changes the contamination and storage assessment.

When a result falls outside the approved acceptance criteria, staff quarantine the component and follow their deviation, investigation and disposition procedures. They do not “fix” it by improvising extra washes or changing the storage temperature.

Why frozen red cells are most useful for rare phenotypes

For common blood-group requirements, a standard refrigerated red-cell component is usually simpler to issue. Frozen inventory becomes especially valuable when a patient needs red cells lacking an uncommon antigen or has developed antibodies that make ordinary units inappropriate. A blood bank can preserve a rare-compatible unit for future use rather than rely on finding another donor at the moment of an urgent transfusion.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

After thawing and washing, deglycerolized cells are generally reserved for situations in which standard components are unavailable or unsuitable. The extra steps consume staff time, equipment capacity and a portion of the original red-cell mass, so the benefit is greatest when compatibility or another clinical requirement outweighs that burden.

What this process is—and is not

It is a regulated blood-component workflow

Frozen red-cell preparation takes place in licensed or otherwise authorized blood-bank settings using monitored equipment, validated methods, trained staff and traceable records. Product labeling, transfusion-service procedures and applicable regulatory requirements determine the exact details.

It is not ordinary household freezing

A domestic freezer does not provide the validated freezing profile, continuous monitoring, controlled glycerol addition, sterile washing or post-thaw testing required for a transfusable component. Placing whole blood or a red-cell bag in a household appliance could damage the cells and creates serious contamination and safety risks. This subject should not be treated as a home preservation technique.

How special regulatory exceptions should be interpreted

U.S. Food and Drug Administration exceptions documents list limited approvals for deviations involving described rare antigen-negative frozen units. The cited cases include inspection and testing such as recovery, free hemoglobin and residual glycerol. These are narrowly defined exceptions for the situations and conditions stated in the approvals. They are not general permission to alter routine storage temperatures, skip deglycerolization or extend a component’s dating period.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The practical decision for a transfusion service

The decision is a balance between preserving a hard-to-replace red-cell phenotype and accepting the handling complexity of a cryopreserved component. A service evaluates the patient’s compatibility needs, inventory availability, urgency, validated equipment, post-thaw testing capacity and the allowable transfusion interval. If a standard component meets the clinical need, it usually avoids the extra thawing and washing pathway. If compatible blood is rare or standard components are inappropriate, frozen inventory can provide a dependable reserve—provided every processing and release step is followed.
