Falcon 9 flights were temporarily paused after the rocket’s upper stage failed in space during SpaceX’s Starlink Group 9-3 mission on July 11, 2024. The first-stage booster did not explode or crash. The Federal Aviation Administration (FAA) required an investigation, then authorized Falcon 9 to return to flight on July 25 after determining that the anomaly posed no public-safety issue. The technical investigation could continue after launches resumed.
What happened on July 11, 2024?
SpaceX launched the Starlink Group 9-3 mission on a Falcon 9. The FAA’s incident description identifies a failure of the rocket’s upper stage in space. A contemporary Futurism report said the Starlink satellites were released but did not reach their intended orbit and were expected to re-enter Earth’s atmosphere and fully demise.

That distinction matters. The headline’s word “explodes” is a shorthand description of an in-space vehicle failure, not evidence that the reusable first-stage booster blew up or that a rocket crashed near a populated area. The FAA reported no public injuries or public property damage from the incident.

Why did the FAA ground Falcon 9?
The FAA required SpaceX to investigate the mishap before normal operations could continue. The agency said it would participate in the investigation and had to approve SpaceX’s final report and proposed corrective actions. The stated goals were to establish the root cause, improve public safety and prevent a recurrence.

At the time of the July 15 report, that requirement meant Falcon 9 missions were on hold. “Grounded” therefore described a temporary regulatory pause across the fleet, not a permanent cancellation of Falcon 9 flights.
Was ice buildup the cause?
Futurism’s contemporaneous account described visible ice buildup before the upper-stage failure. The material publicly available for this account does not establish icing as the confirmed root cause. It is more accurate to describe ice as an observed condition in the report, while treating the final technical cause as unresolved in the sources available here.

When did Falcon 9 flights resume?
- July 11: The Starlink Group 9-3 upper stage failed in space.
- July 11 onward: The FAA required a mishap investigation and SpaceX paused Falcon 9 launches.
- July 15: SpaceX requested an FAA public-safety determination for a return to flight while the investigation was still underway.
- July 25: The FAA authorized Falcon 9 to resume flying after determining that no public-safety issues were involved in the anomaly. Other launch-license requirements still applied.
Resumption did not mean the investigation had been closed or that a final cause had been publicly established. It meant the FAA had made a separate safety determination allowing launches to proceed under the applicable licensing conditions.
How the FAA can authorize a return before an investigation ends
FAA process guidance describes two routes after a launch mishap:
Rank #3
- HOBBY MODEL KIT – Unassembled model packed in an envelope with easy to follow instructions. Ideal for ages 14 and up.
- NO GLUE OR SOLDER NEEDED – Parts can be easily clipped from the metal sheets. Tweezers are the recommended tool for bending and twisting the connection tabs.
- SPACEX FALCON 9 - 3 Sheet Model with a Moderate difficulty level. Assembled Size: 3.00 L x 1.50 W x 7.00 H inches. 84 Pieces.
- FROM STEEL SHEETS TO 3D – Pop out the pieces and connect using tabs and holes. Includes illustrated instructions.
- HIGHLY DETAILED ETCHED MODEL – Display your 3D model once completed - collect and build them all.
| Route | What it means |
|---|---|
| Final-report route | The FAA accepts the final mishap report and corrective actions before return to flight. |
| Separate return-to-flight determination | The FAA permits launches while the investigation continues, after evaluating whether the anomaly presents a public-safety concern. |
In making that determination, the FAA considers the nature and consequences of the mishap, any debris, the performance of safety-critical systems and whether existing safety analyses remain valid. The agency’s July 2024 wording was: “A return to flight is based on the FAA determining that any system, process or procedure related to the mishap does not affect public safety.”
What happened to the Starlink satellites?
Because the upper stage did not place the payload into its intended orbit, the satellites were not expected to operate normally. The contemporary account said they would re-enter and fully demise in Earth’s atmosphere. That outcome concerns the mission’s payload; it does not indicate that the first-stage booster was lost.

The main body of the first-stage rocket is precision aluminum alloy tube, and the rest of the rocket materials are made of high-quality imported resin-with excellent laser forming ability and long-lasting preservation
What is known—and not known—about the final cause?
- Established: The July 11 Starlink Group 9-3 mission experienced an upper-stage failure in space, prompting an FAA investigation.
- Established: The FAA found no public-safety issue in its July 25 return-to-flight decision.
- Not established by the available sources: The final root cause and the complete set of corrective actions.
- Not established: That the reported ice buildup caused the failure.
Accordingly, claims that identify icing as the definitive explanation go beyond what the cited FAA statements and contemporary reporting establish.

No. This was a July 2024 temporary grounding. Falcon 9 return to flight was authorized on July 25, 2024, after the FAA’s public-safety review. The headline should be read as a description of the pause immediately after the July 11 anomaly, not as an ongoing fleet-wide ban.
