Why Enterprise AI Pilots Fail to Reach Production—and How to Fix the Bottlenecks

An AI pilot can prove that a model produces useful outputs; it does not prove that an enterprise can run the whole service safely, reliably, and economically. Moving from demo to production means solving for real data, permissions, governance, integrations, users, operating costs, and accountable ownership—not just model quality. The practical fix is to design for those conditions from the start and agree in advance how success will be measured and whether the work will scale.

Why a successful pilot can still fail in production

A pilot usually operates inside a smaller, more controlled environment than the service it is meant to become. Teams may use curated data, simplified assumptions, and manual review of model outputs. Production has to work across data platforms, SaaS applications, and operational systems, where information may be inconsistent, access is governed, and workflows must continue even when the model is uncertain or unavailable. IBM’s enterprise analysis describes this gap between isolated model performance and the demands of a working service.

The key distinction is scope: a pilot tests whether a model or workflow appears promising under defined conditions; production tests whether the complete service can perform its job within enterprise constraints. A useful answer on a demo screen is not enough if it cannot be securely routed into the system where work happens, or if a person must repeatedly repair the result by hand.

What the reported evidence says about the bottlenecks

Several 2025 surveys point to skills, risk, data, and integration as recurring concerns. Their figures describe the populations and questions in those studies; they are not a universal estimate of the share of AI pilots that fail.

Finding What the study reported How to read it
Skills and expertise 56% of enterprises cited a lack of AI skills and expertise as a barrier. Concentrix and Everest Group’s 2025 study of more than 450 enterprises worldwide.
Cybersecurity and model risk 51% cited cybersecurity and model risk. Same Concentrix and Everest Group study and population.
Data integrity and bias 47% cited data integrity and bias. Same Concentrix and Everest Group study and population.
Legacy integration 41% cited legacy integration challenges. Same Concentrix and Everest Group study and population.
Infrastructure complexity 34% cited infrastructure complexity. Same Concentrix and Everest Group study and population.
Testing-to-implementation transition 27% reported a successful transition from testing to real-world implementation. Concentrix and Everest Group’s 2025 study; this is the study’s reported transition finding, not a cross-industry failure rate.
Enterprise-wide scaling 77% reported that fewer than 40% of their GenAI pilots had been scaled enterprise-wide. Concentrix and Everest Group’s 2025 study. This describes the respondents’ reported scaling experience, not the success rate of all AI pilots.
Data-readiness impact 42% of enterprises said more than half of their AI projects had been delayed, underperformed, or failed because of data-readiness issues. Redpoint Content’s Q1 2025 survey, as reported by Fivetran, covered 401 data leaders and professionals in the US, UK, Europe, Middle East and Africa, and Asia-Pacific, at enterprises with 500 to more than 5,000 employees.
Regulatory compliance 59% named regulatory compliance their top challenge in managing data for AI. Redpoint Content’s Q1 2025 survey, as reported by Fivetran; same sample and scope.
Pipeline maintenance Among centralized enterprises, 67% allocated over 80% of engineering resources to data-pipeline maintenance. Redpoint Content’s Q1 2025 survey, as reported by Fivetran; the finding applies to centralized enterprises in that survey.

The surveys should not be collapsed into one ranking or overall failure percentage: they ask different questions and cover different populations. The useful takeaway is that technical capability is only one part of readiness; organizations also need the people, controls, data foundations, and operating capacity to make a service dependable.

Where pilots get stuck

The data does not resemble the real workload

Clean, selected examples can conceal fragmented sources, incomplete labels, weak lineage, stale information, or permission constraints. A service exposed to a broader mix of cases may behave differently, and teams may spend significant effort maintaining pipelines before they can even evaluate output quality. The Redpoint Content survey findings reported by Fivetran illustrate how often respondents associated data-readiness problems with delays, underperformance, or failure; they do not establish that every affected project had the same cause.

The proof ends at the model output

A prototype may show a useful answer without handling the next step: enforcing policy, updating a system of record, routing work to the right person, or safely handing off an uncertain case. If integration is deferred until after the demo, teams can discover late that the workflow depends on interfaces, identities, or legacy systems that are difficult to connect.

Risk and governance are treated as a final gate

Security, privacy, legal, regulatory, and model-risk requirements shape what data the service can use and what actions it can take. If those stakeholders are brought in only after the prototype works, the team may have to redesign its data access, review process, or even the use case. HPE has highlighted gaps in involving stakeholders such as legal, HR, and CISO teams in its survey reporting; those findings should be understood as HPE survey results, not as representative of every enterprise.

HPE Fellow and HPE Labs Chief Architect Kirk Bresniker put the deployment risk this way: “No matter how successful an AI prototype is at passing tests in isolation on synthetic data, it can all be undermined if the developers fail to pressure test their models against the real-world security, regulatory, and IT conditions of a particular enterprise,”

No team is accountable for the result after launch

A model can meet a technical target while the business outcome remains undefined. Without a named sponsor, a workflow owner, a plan for user adoption, and funded ongoing operations, a promising experiment can remain an experiment. Unclear ownership also makes it hard to decide who handles incidents, approves changes, or pays for run costs.

How to move an AI pilot into production

Concentrix and Everest Group’s 2025 framework recommends identifying a small set of high-value use cases and appointing executive sponsors. The following sequence turns that principle into production-readiness decisions the team can make before the pilot ends.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Choose a measurable workflow problem. Identify the user and process owner, document the current baseline, and set a target business outcome. Define acceptable failure modes—for example, when the service must abstain or escalate—instead of selecting a use case solely because a model demonstration looks impressive.
  2. Map the production path while building the pilot. Identify source data, identity and permission requirements, target systems, integration points, human review, fallback behavior, latency needs, and likely operating costs. Treat each as part of the design, not as a later handoff to an implementation team.
  3. Test with representative data and enterprise controls. Evaluate realistic cases and edge cases, and document data lineage, access, approvals, privacy, security, and applicable policy requirements. Bring legal, risk, security, data, product, and affected business owners into the work early enough to shape it.
  4. Define how the service will be operated. Create an evaluation plan and telemetry for the use case. Specify who monitors quality and reliability, how data or model changes are assessed, how incidents are handled, when a person takes over, and how the service can be rolled back. Use repeatable deployment practices and the MLOps capability needed for the chosen system.
  5. Agree on scale-or-stop criteria before the pilot closes. Set the funding decision, ROI threshold, risk tolerance, and criteria for scaling, revising, or stopping. Track conversion into production and realized business value rather than counting demos or pilots as outcomes.
  6. Reuse what has proved dependable. Turn successful components—such as integrations, governance patterns, prompts, models, and playbooks—into reusable assets. Cross-skill product, data, and domain teams, then use operational reviews and post-mortems to improve the next deployment.

How to judge whether a pilot is production-ready

Use the same decision criteria when comparing use cases or reviewing a proposed launch. A pilot is not ready just because its model outputs look good; the team should be able to explain how the full workflow performs and who owns it.

  • Business value: Is there a named outcome, baseline, measurable return, and plausible time to value?
  • Data readiness: Are quality, lineage, access, representativeness, integration, and update frequency sufficient for the real workload?
  • Governance and risk: Are security, privacy, regulatory fit, auditability, and human oversight addressed for the intended use?
  • Operational fit: Can the service work with systems of record, remain reliable, be monitored, fall back safely, and be maintained by an accountable owner?
  • Capacity and economics: Are the required skills and infrastructure available, and are run costs and funding predictable enough to support the service?
  • Reuse and change: Can the approach be repeated across teams, and are adoption, training, and partner responsibilities clear?

A “no” does not always mean a use case should be abandoned. It identifies a specific dependency to resolve, a risk to accept explicitly, or a reason to stop before committing to a larger rollout. The decision should be made against the business outcome and risk tolerance agreed for that use case.
