A fever after vaccination is usually a temporary sign that the immune system is responding to the vaccine, not that the vaccine caused the disease it prevents. Most vaccine reactions are mild and settle within a few days, but some symptoms require urgent or emergency care. The vaccine received, the person’s age, the timing, and accompanying symptoms all matter.
Why a vaccine can raise body temperature
Vaccines expose the immune system to antigens or other signals that train it to recognize and fight a germ. While that protection is developing, immune activity can produce temporary symptoms such as fever, tiredness, chills, body aches, or soreness where the injection was given. The Centers for Disease Control and Prevention (CDC) describes these as normal signs that the body is building immunity in its COVID-19 vaccine explanation.

A post-vaccination fever does not mean the vaccine gave you the infection it is designed to prevent. It also does not prove that immunity has formed, because the presence, height, or duration of fever does not measure protection.
Adjuvants can make reactions more noticeable
Some vaccines contain adjuvants, ingredients intended to produce a stronger immune response. CDC says adjuvanted vaccines can cause more local reactions and more systemic reactions—including fever, chills, and body aches—than non-adjuvanted vaccines. That general comparison cannot identify which ingredient caused one person’s fever, and it does not predict the reaction to every vaccine.

What timing and duration are typical?
There is no single schedule for fever after every vaccine. CDC says most vaccine side effects are minor and go away within a few days, but reactions differ by vaccine and by person. Check the Vaccine Information Statement (VIS) for the product you received; it lists expected reactions and warnings specific to that vaccine.
| Situation | What the guidance supports | What not to assume |
|---|---|---|
| Common post-vaccination reaction | Low-grade fever, a sore arm, tiredness, or other mild symptoms that generally fade within a few days. | That every fever is harmless or that every vaccine causes fever. |
| Live attenuated vaccine | Fever or rash can occur later because the vaccine virus needs several days to replicate. CDC general immunization guidance describes usually mild systemic reactions occurring about 3–21 days after administration for this category. | That the 3–21-day interval applies to all vaccines. |
| COVID-19 vaccination | The immune system may take a few weeks to produce T- and B-lymphocytes. A fever, when it occurs, is a short-term symptom and does not need to last for the immune-building process. | That a fever must continue for weeks or that its duration proves immunity. |
If symptoms begin outside the expected window in the relevant VIS, are unusually intense, or continue instead of improving, contact a healthcare professional. A fever after vaccination can also coincide with an unrelated infection or another condition; timing alone cannot establish the cause.
Use the symptom pattern to decide how urgently to act
Temperature is only one part of the assessment. The combination of symptoms, the person’s age, and the vaccine involved determines whether to monitor, call a clinician, or seek emergency help.

| Pattern | Action |
|---|---|
| Mild fever or soreness with otherwise stable behavior | Monitor, encourage fluids and rest, and follow the product’s VIS. Contact a clinician if the reaction is severe, persistent, worsening, or concerning. |
| Hives, swelling of the face or throat, trouble breathing, fast heartbeat, dizziness, or weakness | Call 911 and go to the nearest hospital. These can signal a severe allergic reaction; do not wait for them to pass. |
| Temperature of 100.4°F (38°C) or higher in a child younger than 12 weeks | Treat this as an emergency warning sign and obtain urgent medical evaluation. The CDC threshold is age-based and applies even when the fever follows vaccination. |
| Chest pain, shortness of breath, or a fast, fluttering, or pounding heartbeat after a COVID-19 vaccine | Seek medical attention right away, as directed in the CDC COVID-19 Vaccine Information Statement. |
| Severe, persistent, worsening, or otherwise unusual symptoms | Call a healthcare professional for individualized advice, even if no single emergency sign is present. |
Emergency allergic reaction: call 911
Severe allergic reactions are uncommon, but they can become life-threatening quickly. After any vaccine, call 911 and get the person to a hospital if hives appear with swelling of the face or throat, difficulty breathing, a fast heartbeat, dizziness, or weakness. Do not drive past breathing or throat symptoms hoping they will resolve, and do not delay while trying home treatments.
Stay with the person, follow the emergency dispatcher’s instructions, and tell responders which vaccine was given and when. A short observation period without symptoms does not guarantee that a later illness is related to the vaccine, so new severe symptoms still warrant assessment.
Fever in a baby younger than 12 weeks
For an infant younger than 12 weeks, any temperature of 100.4°F (38°C) or higher is an emergency warning sign in CDC guidance. That warning is from general respiratory-illness guidance rather than a vaccine-specific rule, but the age threshold still matters when the fever follows immunization.

The CDC COVID-19 VIS tells people to seek medical attention right away for chest pain, shortness of breath, or feelings of a fast-beating, fluttering, or pounding heart after COVID-19 vaccination. These symptoms are distinct from ordinary arm soreness or a low-grade fever and should not be managed by simply waiting for the fever to end.
When calling, state the vaccine date and product if known, when the symptoms began, whether they are worsening, and whether fainting or breathing difficulty occurred. Emergency services are appropriate if symptoms are severe or rapidly progressing.

Children, fever, and febrile seizures
A febrile seizure is a seizure associated with a fever. CDC says most febrile seizures occur in children 6 to 59 months old, and up to 5% of young children will have at least one, according to its 2026 guidance. They can occur with many fever-causing illnesses, including a cold, influenza, or an ear infection, not only after vaccination.
CDC describes a small increased risk of febrile seizures during the 24 hours after a child receives an inactivated influenza vaccine at the same visit as PCV13 or a DTaP-containing vaccine in the studied circumstances. That finding should not be generalized to every vaccine or every combination. Influenza illness itself can produce high fever and febrile seizures.
What to do during a seizure
- Place the child on a safe surface on their side, away from hard or sharp objects.
- Do not restrain the child and do not put food, liquid, or anything else in the mouth.
- Note when the seizure starts and stops.
- Call emergency services if it lasts longer than five minutes, breathing is impaired, another seizure follows, the child is injured, or the child does not recover normally. Seek medical advice after any first seizure.
What you can do for a mild post-vaccine fever
Measure and record the temperature
A digital thermometer can help you compare a reading with age-specific guidance. Record the number, time, measurement method, vaccine received, and associated symptoms. A temperature reading does not diagnose why the fever occurred, so use it as one piece of information rather than a substitute for medical evaluation.

Support comfort
- Offer fluids as appropriate, allow rest, and avoid overheating with excessive clothing or blankets.
- Use the vaccine’s VIS to check the expected reaction and its usual duration.
- Ask a clinician or pharmacist about an age- and condition-appropriate fever medicine and dose, particularly for infants, children, pregnancy, older adults, or people with chronic conditions.
CDC says evidence does not support taking fever-reducing medicine before or at the time of vaccination to prevent reactions. Such medicines may be used afterward to treat fever or local discomfort that actually occurs, using professional or label guidance. Do not give a child another person’s prescription or guess a dose from age alone.
When a nonemergency call is still the right choice
Contact a healthcare professional when the fever or other symptoms are severe, persist beyond the expected course in the relevant VIS, worsen, or simply seem unlike the person’s usual post-vaccine reactions. Also call if the person has a condition or takes medication that changes infection or bleeding risk, or if you are unsure how to measure the temperature or treat discomfort.
No universal temperature cutoff for every age and vaccine is established by the guidance summarized here. Outside the specific infant threshold, clinicians use the whole picture: age, medical history, vaccine type, timing, measured temperature, hydration, breathing, alertness, and symptom progression.

Information to have ready when seeking care
- Name the vaccine and the date and time it was administered; bring the vaccination record or VIS if available.
- Report the highest measured temperature, how it was measured, and when it began.
- List other symptoms, including rash, swelling, breathing trouble, chest pain, palpitations, vomiting, poor feeding, unusual sleepiness, or a seizure.
- Give the person’s age, relevant medical conditions, allergies, current medicines, and any recent illness or exposure.
- Describe any fever medicine already given, including the product, strength, dose, and time.
Common misconceptions to avoid
- “Fever means the vaccine caused the disease.” A vaccine-related immune response can cause fever without causing the disease being prevented.
- “A higher fever means stronger immunity.” Fever intensity is not a measure of protection.
- “Every vaccine fever starts the next day and ends quickly.” Timing is vaccine-specific; later reactions are possible after live attenuated vaccines.
- “Any fever after vaccination is automatically safe.” Emergency allergy signs, the infant threshold, COVID-19 cardiac or breathing symptoms, and worsening illness need prompt care.
The safest approach is to match the reaction to the vaccine-specific information, the person’s age, and the accompanying symptoms rather than relying on a single number or timetable.
