An AI answer can fail in two different ways: it can state a fact that is false, or it can give an explanation that sounds coherent but does not describe what actually produced the answer. Those failures require different checks. Verify factual claims against reliable evidence, and treat a model’s account of its reasoning as a claim that needs independent testing—not as a transparent transcript of its internal computation.
Two different problems are often called “unreliability”
When an assistant produces a wrong answer, the obvious question is whether the answer is true. When it provides a rationale, a second question appears: did that rationale actually cause the answer, or is it a persuasive story assembled afterward?

| Problem | What fails | What can test it |
|---|---|---|
| Hallucination | The answer contains a false, unsupported or invented factual claim. | Check the claim against a primary document, reliable dataset, calculation or other independent evidence. |
| Unfaithful explanation | The explanation does not accurately represent the factors or steps that generated the answer, even if the answer happens to be correct. | Use controlled interventions, reproducible procedures, or an independently executable chain of steps. |
These tests are not interchangeable. A source can establish that a date, number or quotation is correct without proving that the model used that source or that its stated rationale caused the output. Conversely, a faithful account of a computation would not make a wrong input or an incorrect conclusion true.

Prediction is not a built-in truth check
Language models are trained to predict likely continuations of text. The training objective rewards a continuation that resembles patterns in the data; it does not automatically attach a true-or-false label to every sentence. Common names, familiar wording and highly repeated facts can therefore be easier to reproduce than obscure, recent or low-frequency details.

OpenAI’s September 5, 2025 explainer, Why language models hallucinate, defines the phenomenon this way: “Hallucinations are plausible but false statements generated by language models.” The wording matters: plausibility is about how naturally a sentence fits its context, not whether the sentence is supported by evidence.
Accuracy-only tests can reward guessing
If an evaluation counts every unanswered item as a failure but gives credit only for an exact answer, a system can improve its score by guessing instead of expressing uncertainty. OpenAI’s 2025 analysis argues that this incentive helps explain why models may fabricate a specific answer rather than abstain. That is an argument about training and evaluation incentives, not proof that every model or deployment uses identical objectives.
Uncertainty-aware evaluation changes the choice. A benchmark can reward a calibrated “I don’t have enough information” when the evidence is insufficient, and penalize confident errors more heavily when they would cause harm. Users should apply the same principle manually: ask what evidence would justify an answer before treating fluency as confidence.
Why a convincing explanation may not be faithful
A rationale can be plausible without recording the computation
A free-form explanation is another sequence of text generated by the model. It may summarize a genuine pattern, but it can also rationalize an answer after the fact. The explanation’s logical order and polished language are not evidence that the model followed those steps internally.
This distinction is sometimes called faithfulness: whether an explanation corresponds to the causes of an output. It is separate from plausibility, meaning whether a reader finds the explanation coherent and persuasive.

Biasing cues can disappear from the stated reasoning
Turpin, Michael, Perez and Bowman tested this issue in 2023 with GPT-3.5 and Claude 1.0 on 13 BIG-Bench Hard tasks. They manipulated features such as the order of answer options. Under those conditions, the models’ accuracy fell by as much as 36% in the reported experiments, while their chain-of-thought explanations often failed to mention the influential cue and instead justified the selected answer.
“Our findings indicate that CoT explanations can be plausible yet misleading, which risks increasing our trust in LLMs without guaranteeing their safety.” — Turpin, Michael, Perez and Bowman, 2023

The 36% figure is a maximum drop in that particular experiment, not a general hallucination rate or a prediction for every current model. The broader lesson is methodological: if changing an input feature changes the answer but the explanation does not acknowledge that feature, the explanation is not a dependable causal record.
What the published results do—and do not—show
| Result | Scope | Correct interpretation |
|---|---|---|
| Up to 36% accuracy drop | Turpin et al., 2023; GPT-3.5 and Claude 1.0; biasing conditions across 13 BIG-Bench Hard tasks. | Evidence that explanations can omit influential cues under tested conditions, not a universal reliability percentage. |
| 6.3% relative gain on Math Word Problems; 3.4% on Planning; 5.5% on Multi-hop QA; 21.4% on Relational Inference | Lyu and co-authors, 2023; their Faithful CoT approach compared with standard CoT on specified benchmark domains. | Reported relative gains for that method and those tasks, not guaranteed improvements for arbitrary prompts. |
| 40% higher score | OpenAI’s GPT-4 technical report, 2023; GPT-4 versus the then-current GPT-3.5 on OpenAI’s internal adversarial factuality evaluations. | A release-time, publisher-reported comparison for one evaluation, not a current cross-model ranking. |
No independently comparable, current statistic establishes how often all contemporary models produce unfaithful explanations. A single percentage should therefore never be used as a blanket trust score.
How to check an AI answer and its reasoning
1. Separate claims from explanations
Copy the answer into two lists. The first contains checkable assertions: names, dates, measurements, legal requirements, causal statements and calculations. The second contains the model’s explanation of how it reached the answer. Verify the first list for truth; test the second for procedural or causal correspondence.

2. Identify the cost of being wrong
Classify the decision before choosing a review level.
- Low stakes: a wrong trivia detail or draft phrasing may be corrected later.
- Moderate stakes: a work report, financial estimate or technical configuration needs source checking and a second pass.
- High stakes: medical, legal, safety, employment, security or major financial decisions require qualified human review, and may require avoiding model reliance altogether.
3. Check primary evidence, not just citations
Ask the model for the exact source, passage, dataset row or formula supporting each important claim. Open the source yourself and confirm that it supports the specific wording, date, jurisdiction and conditions. A citation that exists but does not entail the claim is not verification.
4. Recompute structured work independently
For arithmetic, statistics, unit conversions, code behavior or formal logic, reproduce the result with a calculator, spreadsheet, interpreter or deterministic solver. Compare inputs as well as outputs; a flawless calculation using a fabricated premise is still wrong.
5. Probe assumptions and uncertainty
Ask: “What information is missing?”, “Which assumption would change the conclusion?”, and “What would make you abstain?” An answer that changes materially when an unstated assumption is supplied should be presented conditionally rather than as a fact.
6. Test whether the explanation tracks causes
Change one relevant input at a time: reorder options, remove a cited fact, alter a numeric value or substitute a counterexample. If the answer changes, see whether the explanation identifies the changed feature. This is stronger evidence than asking the model to repeat its rationale in different words.

When structured reasoning is safer than free-form prose
For some tasks, a model can translate a problem into symbolic steps that a deterministic program executes. This “Faithful CoT” pattern makes the executable portion auditable: a reviewer can inspect the symbols, run the solver and compare intermediate results.
It still has a weak link. The translation from natural language to the symbolic representation can be wrong, omit a constraint or encode the wrong variable. Execution proves that the solver followed the supplied representation; it does not prove that the model’s translation was faithful or that the original answer was appropriate.
Lyu and co-authors reported relative accuracy gains of 6.3% on Math Word Problems, 3.4% on Planning, 5.5% on Multi-hop Question Answering and 21.4% on Relational Inference for their approach versus standard chain-of-thought on the specified benchmarks. Those values describe their experiments, not an expected gain for every implementation.
Process supervision is another approach: evaluate intermediate steps rather than only the final answer. OpenAI reported a process-supervision result on the MATH dataset, but the authors said its generalization beyond mathematics was unknown. A method that works for verifiable algebra may not transfer to open-ended history, policy or diagnosis.

Choosing an explanation-checking method
| Method | Factual verification | Process faithfulness | Auditability | Best fit | Main limitation |
|---|---|---|---|---|---|
| Source and citation checking | Strong when the source directly supports the claim. | Does not establish why the model produced the answer. | High for documented facts. | Research, dates, policies and quotations. | Sources can be misread, outdated or irrelevant to the precise claim. |
| Independent recomputation | Strong for numerical results with verified inputs. | Tests the procedure more than the model’s hidden process. | High when steps are reproducible. | Math, statistics, conversions and code. | Cannot validate fabricated assumptions. |
| Input intervention | May reveal sensitivity to misleading cues. | Useful for testing whether stated reasons track influential features. | Medium; requires controlled variants. | Classification, ranking and decision prompts. | One intervention cannot map the entire computation. |
| Symbolic translation plus solver | Checks the executable result against encoded rules. | Stronger procedural evidence than free-form prose. | High after translation is validated. | Formal calculations and constrained planning. | The translation step can still be wrong. |
| Human expert review | Can integrate sources, context and consequences. | Can challenge both answer and rationale. | Depends on documentation and expertise. | High-stakes or ambiguous decisions. | Costs time and does not eliminate human error. |
A practical prompt and review workflow
- Define the deliverable. State the jurisdiction, date range, units, audience and acceptable uncertainty. Ambiguity creates room for an apparently precise but unsuitable answer.
- Request claims separately from support. Ask for a numbered list of assertions, assumptions and sources rather than one uninterrupted narrative.
- Require abstention conditions. Tell the model to say what it cannot establish and what evidence would be needed to proceed.
- Use tools outside the model. Open cited documents, run calculations independently and inspect code in an isolated environment before accepting results.
- Perform a counterfactual check. Change a potentially biasing detail and compare both outputs and explanations.
- Escalate review with consequences. Have a qualified person approve decisions where an error could injure someone, violate a rule or cause substantial loss.
- Record versions and inputs. Save the prompt, model version, retrieved documents and tool outputs so another reviewer can reproduce the decision.
Why polished language is a poor confidence signal
Fluency reflects learned conventions for sounding helpful, organized and certain. It does not measure source quality, calibration or causal faithfulness. A hesitant-looking answer can be correct, while a detailed explanation can contain an invented premise and a post-hoc justification.
Best Value
Use confidence language only when it is tied to evidence: identify which parts are directly documented, which are calculated, which depend on assumptions and which remain unknown. Ask for competing interpretations when the question is ambiguous instead of rewarding the first coherent story.
Safeguards for consequential use
OpenAI’s GPT-4 technical report stated: “Great care should be taken when using language model outputs, particularly in high-stakes contexts, with the exact protocol (such as human review, grounding with additional context, or avoiding high-stakes uses altogether) matching the needs of a specific use-case.” The report describes GPT-4 at its 2023 release; it is guidance for risk management, not a current ranking of models.
- Keep a human decision-maker accountable for the final action.
- Use authoritative, current documents and restrict the model to the relevant evidence where possible.
- Require a second person or independent system for safety-critical checks.
- Prefer deterministic or formally validated software for calculations and rule enforcement.
- Do not let a model’s explanation substitute for professional judgment, consent or legally required review.
What a trustworthy answer should look like
A responsible response distinguishes observed facts from inferences, states assumptions, gives sources that actually support the claims and identifies uncertainty that could change the result. For a computation, it exposes inputs and reproducible steps. For an open-ended question, it offers ways to verify the important parts and says when it should abstain.
The central discipline is to ask two separate questions every time: Is the answer supported? and Does the explanation faithfully describe how it was produced? The first is answered with evidence; the second requires procedural tests. Neither confidence, detail nor elegant prose can answer both on its own.
