AI can get a remembered fact wrong for three different reasons: it may not have stored the fact, it may retrieve the wrong information, or it may have the right context and still generate a false answer. The fix depends on which failure occurred. For a personal assistant, inspect and correct the memory and its source; for an AI application, test retrieval separately from how the model uses retrieved context.
What “AI memory” means—and what can fail
AI memory is not one universal store. In a consumer assistant, relevant information may come from saved memories, summaries of chats, conversation history, files, or connected apps. In an AI application, it may come from retrieved external records, structured context, or behavior learned by the model. Those sources are not interchangeable, and they may have different controls.
A memory summary is not necessarily a complete account of everything the assistant can use. Some systems selectively retrieve relevant details, while a summary may omit information or its source. OpenAI’s Memory FAQ distinguishes saved memories from information derived from chat history and notes that summaries can be incomplete. Its controls and availability may vary by plan, region, platform, and workspace, so check the current settings in your own account.
- Missing or incomplete information: the fact was never saved, was omitted from a summary, or was not available in the current context.
- Stale information: a once-accurate detail, such as a job, location, or preference, no longer applies. OpenAI notes that saved memories can become outdated, incorrect, or irrelevant in its memory update.
- Retrieval error: an application selected the wrong record or included too much irrelevant context.
- Generation error: the model had suitable context but answered incorrectly anyway. OpenAI’s API documentation puts it plainly: “The model can also get the right context and do the wrong thing with it.” See Optimizing LLM Accuracy.
A confident tone does not establish that the remembered fact is correct. OpenAI defines hallucinations as “plausible but false statements generated by language models” in its September 5, 2025 article, Why language models hallucinate.

How to correct a wrong memory in a consumer assistant
- Inspect what the assistant remembers. Ask it what it remembers about the relevant subject, or open the product’s memory summary and saved-memory controls. Look for the specific wrong detail and, if shown, the source. A summary may not expose every detail or source the system can use.
- Correct the fact directly. When the product offers editing or correction, state the accurate information clearly and add a date or context if it can change. ChatGPT’s documented controls include entering a correction, highlighting text and providing a correction, or choosing “Don’t mention this again” where available. These controls can affect future personalization without deleting the original material.
- Decide whether you want correction or deletion. Correcting a fact is different from removing it. If you want information removed from ChatGPT, check each place it may exist: OpenAI says this can require deleting both the saved memory and the chat where it was first shared, and removing it from other relevant sources such as a summary, file, or connected app. Deleting a chat alone does not necessarily delete a separate saved memory.
- Check the next answer. Ask the assistant to state the relevant fact and its source, where supported. If it still conflicts with what you told it, correct it again and check whether the information also exists in another source. Do not assume a correction updated every connected source or every copy.
OpenAI’s Memory FAQ says updates and deletions can take time to propagate. It also says logs of deleted saved memories may be retained for up to 30 days for safety and debugging. Those statements apply to ChatGPT’s documented controls, not to every AI service.
Why an AI may still get a fact wrong after correction
Changing one saved item does not necessarily change the original chat, a file, a connected app, or a separate summary. The assistant may encounter a conflicting copy, fail to retrieve the correction, or use the right correction incorrectly. Time-sensitive details can also become stale again, so include context such as “as of October 2026” when that precision matters.

Memory questions can depend on dates, locations, or combining several records. The Memory-QA paper describes these as challenges in multimodal recall. In a concrete case, check whether “last Tuesday” was resolved to the intended date, whether the system chose the most recent relevant entry, and whether it combined the entries correctly. The paper’s authors report that PENSIEVE improved end-to-end QA accuracy by up to 14% over compared state-of-the-art multimodal retrieval-augmented systems on their benchmark; that result is specific to the paper’s evaluation, not a general measure of consumer assistant memory.
How developers should debug memory errors
For an AI application, separate failures in context selection from failures in answer generation. OpenAI’s developer guide recommends evaluating which layer failed before tuning the system. Inspect the retrieved records and the context passed to the model; then assess whether the model followed that context.

- If retrieval is wrong or noisy: improve relevance and reduce irrelevant context. Changing the prompt alone will not reliably repair a retrieval system that supplies the wrong records.
- If retrieval is suitable but the answer is wrong: improve the prompt and the method used to get the model to apply the context. Better storage or retrieval alone cannot guarantee correct use.
- If the task requires learned behavior: consider fine-tuning where appropriate. Fine-tuning, prompt work, and retrieval address different problems; they are not universal substitutes for one another.
A 2025 survey proposes one useful vocabulary for discussing system design: parametric, contextual structured, and contextual unstructured memory representations, alongside operations it calls consolidation, updating, indexing, forgetting, retrieval, and compression. Evaluate answer quality as well as whether the system abstains when it lacks support. In its 2025 SimpleQA comparison, OpenAI reported these results for two named models:

These figures apply to the named models on that evaluation; they are not general AI-memory error rates. They illustrate why accuracy alone can hide an important distinction: a system may answer more often by guessing, while a system that abstains more may avoid some errors. Before changing a system, identify what kind of memory it uses and what the correction is meant to change. These distinctions apply whether you are using a personal assistant or maintaining an AI application.

How AI memory is changing
In an April 2026 announcement, OpenAI described an evolution from saved memories introduced in April 2024, to broader use of chat context in April 2025, to a more capable memory architecture it calls “dreaming.” The announcement reports improved relevant-fact recall in an evaluation, but that does not show that memory mistakes have been eliminated or that the evaluation covers every user task.
