Why AI Chatbots Make Mistakes—and How to Spot Them

AI chatbots can produce fluent, detailed answers that are false. They generate likely text from learned patterns rather than proving each sentence against reality, so confidence, precision and a polished explanation are not evidence of correctness. The safest approach is to treat an answer as a draft: identify its important claims, check those claims against suitable original sources, and escalate consequential decisions to an authoritative source or qualified professional.

What an AI chatbot is actually doing

A language model generates text one token (a word or word fragment) at a time. It has learned statistical relationships from training data and uses those patterns to predict what text should come next. That process is extremely good at producing coherent language, but it does not attach a guaranteed true-or-false label to every sentence.

OpenAI describes pretraining as supplying fluent text rather than an explicit truth database for every statement. NIST uses the term confabulation for cases in which a generative-AI system “generate[s] and confidently present[s] erroneous or false content in response to prompts.” “Hallucination” and “fabrication” are also common names for the same broad failure mode. The behavior is not deliberate lying: the system is generating a plausible continuation, not forming a human intention to deceive.

This distinction explains why an answer can be grammatically perfect, well organized and completely wrong. The model’s objective is to produce a likely sequence of words that fits the prompt and conversation. Truth may correlate with likely wording, but the two are not identical.

Why chatbots make mistakes

They do not automatically know whether a sentence is true

Training exposes a model to many examples of language, but not to a reliable, complete label for every factual proposition. Common facts often have strong, repeated patterns. An obscure date, an unusual technical parameter or a newly announced policy may have weak or conflicting patterns. The model can still produce a smooth answer by combining nearby associations, even when the specific combination is unsupported.

The information may be missing, stale or inaccessible

A model’s built-in knowledge has a cutoff or other coverage limits. Even when a service offers web search, you cannot assume that every response used it. OpenAI distinguishes answers generated from training from answers produced with search or deep-research tools. A question about today’s price, a current office-holder, a live software release or a newly changed rule requires a current source; a fluent response based only on older training data may be obsolete.

Ambiguous prompts force the model to guess

Questions that omit a country, date, edition, audience, unit or definition can have several valid interpretations. “What is the legal limit?” depends on jurisdiction and circumstance. “How much storage does it need?” depends on the application version, operating system and workload. If the chatbot does not ask a clarifying question, it may silently select an assumption and present the resulting answer as if the question had been precise.

Rare details are harder than familiar patterns

Names, identifiers, archival facts, small companies, obscure medical conditions and niche standards may appear infrequently or inconsistently in training material. A model can blend a real name with the wrong organization, substitute a similar event or fill a gap with an invented detail. Specificity is therefore not a reliability signal: an exact-looking number can be less trustworthy than a cautious range supported by a source.

Long and specialized tasks accumulate errors

Every additional step in a calculation, legal interpretation, code explanation or historical narrative creates another opportunity for a wrong assumption. A response can begin with a correct premise and drift while combining many facts. Contradictions within one answer or between turns are possible because the model is generating each passage from context, not maintaining a guaranteed, internally verified database.

It may invent supporting evidence

Chatbots can generate plausible-looking book titles, URLs, article names, quotations and citations that do not exist. They can also cite a real page that does not support the nearby claim. NIST warns that fabricated supporting citations can increase misplaced trust, because the appearance of evidence makes an answer seem independently verified.

Evaluation can reward guessing

In a 2025 explanation, OpenAI argues that evaluations focused heavily on exact-answer accuracy can make guessing look better than admitting uncertainty. A model that says “I don’t know” receives no exact-answer credit, while a confident guess may occasionally score as correct. This is an argument about training and evaluation incentives, not proof that every model or response is wrong. In many situations, an explicit uncertainty statement or a clarifying question is more useful than an unsupported answer.

Common forms of chatbot error

Error type What it looks like Why it matters
Invented fact A person, product feature, date or statistic is stated confidently but cannot be corroborated. You may repeat false information or make a decision on a nonexistent premise.
False citation A precise-looking reference leads to a missing page, wrong title or unrelated passage. The citation creates an illusion of verification.
Misquotation Words are attributed to a person or document without matching the original wording. Small wording changes can reverse legal, scientific or historical meaning.
Out-of-date answer The response describes an older law, price, interface, model or policy as current. Time-sensitive information can become unsafe or unusable.
Wrong scope A regional rule, one software edition or one study is presented as universal. A true statement in one context becomes false in another.
Calculation or unit error The arithmetic appears neat but uses the wrong unit, baseline or conversion. Financial, engineering, health and performance decisions can be materially affected.
Internal contradiction Different paragraphs or turns give incompatible dates, totals or recommendations. It signals that the answer was not consistently checked.

How to fact-check an AI answer

Use this process when the answer matters. It is a verification method, not a guarantee that every error will be found.

  1. Define the decision and its stakes. Mark claims that could affect health, money, safety, legal rights, employment, education, privacy or reputation. Those claims deserve independent confirmation even when the rest of the answer is merely informational.
  2. Split the response into individual claims. Separate dates, names, numbers, causal explanations, quotations and recommendations. A paragraph that sounds persuasive may contain several claims, only some of which are supported.
  3. Write down hidden assumptions. Record the country, jurisdiction, date, product edition, operating system, units, age group, sample size or definition the answer appears to use. Ask the chatbot to state its assumptions if they are unclear, but do not treat its statement of assumptions as proof.
  4. Rank claims by risk and specificity. Check unusual names, exact figures, recent events, technical limits and strong causal statements before spending time on general background. A precise claim with a large consequence is a high-priority check.
  5. Find an appropriate independent source. Prefer the issuing government, court, regulator, standards body, original research paper, manufacturer documentation or other source with direct responsibility for the information. For professional decisions, consult the relevant qualified practitioner.
  6. Open every citation. Confirm that the page exists, that its date and scope match the claim, and that the passage actually supports the wording beside it. A real source can be cited out of context, and a nonexistent source can look entirely plausible in generated text.
  7. Verify quotations at the original document. Compare the exact words, speaker or author, date, edition, translation and surrounding context. Do not rely on quotation marks supplied by the chatbot.
  8. Recalculate figures yourself. Check units, rounding, denominators, currency, time period and whether the number is a one-time value, a recurring amount or a range. For code or technical instructions, test in a safe environment and compare the behavior with current documentation.
  9. Check time and regional scope. Ask whether the answer needed information after the model’s knowledge cutoff or a particular jurisdiction. If so, verify a dated, local source. Do not infer that a web search occurred just because the response contains links or recent-sounding language.
  10. Ask for uncertainty and missing inputs. A useful follow-up is: “List each claim that you cannot verify, state the assumptions, and identify what information would change the answer.” Treat the result as a checklist for your own checking, not as an independent review.
  11. Stop when the evidence conflicts. Preserve the disagreement, identify which source has authority and date, and seek expert help when the consequence is significant. Do not average contradictory claims into a new number.

Signals that an answer needs extra scrutiny

  • It gives an exact date, percentage, dosage, legal threshold or performance figure without naming a source.
  • It answers a current-events or product-version question without stating the date or whether browsing was used.
  • It supplies several citations that you have not opened, especially when the links contain odd titles or do not load.
  • It quotes a person or document but omits the original publication, page, section or context.
  • It confidently resolves an ambiguous question without asking about location, timeframe, edition or intended use.
  • It uses absolute language such as “always,” “never,” “proven” or “guaranteed” for a complicated subject.
  • Its numbers change between paragraphs, or its conclusion does not follow from the evidence it presents.
  • It claims to have performed an action—such as checking a live database, running code or reading a private file—that you did not actually authorize or observe.

How current information and browsing change the check

Browsing can improve an answer by giving the model access to current pages, but it does not eliminate the need to inspect those pages. Search results can be incomplete, a retrieved page can be misread, and a source can be outdated or inappropriate for the question. Check the publication date, the authority of the publisher, the exact passage and whether the page covers your region and situation.

For a time-sensitive question, include a cutoff in your own verification: “What was true on 17 October 2026?” is materially different from “What is true now?” Preserve the source’s date when you record the answer. If a service reports that it searched, regard that as a description of a tool action, not as evidence that the resulting claim is correct.

What benchmark numbers do—and do not—tell you

Published evaluations are useful only when you keep their scope attached to the number. They are not universal everyday error rates for all chatbots.

GPT-5-main: 26% lower claim-level hallucination rate than GPT-4o; GPT-5-thinking: 65% lower than o3. OpenAI Deployment Safety Hub, GPT-5 System Card (2025). The metric is the percentage of factual claims with minor or major errors under the card’s evaluation setup. It is a relative, model-specific comparison, not a general reliability score.
75% agreement between the system-card grader and human factuality judgments. The grader used in that OpenAI evaluation. This measures grader agreement, not model accuracy and not the percentage of answers that are true.

No source establishes one reliable percentage for how often all AI chatbots make mistakes in ordinary use. When comparing evaluations, match the model and version, benchmark or prompt set, browsing conditions, claim-level versus response-level metric, abstention policy and evaluation date.

Using chatbots safely for common tasks

Research and study

Use the chatbot to generate search terms, explain a difficult passage, outline competing interpretations or suggest questions. Retrieve the underlying papers, books or official documents yourself. Verify quotations, page references, dates and numerical results before citing them in an assignment or publication.

Writing and editing

It can help reorganize prose, propose examples or identify unclear sentences. Keep responsibility for factual claims, attribution and originality. A confident rewrite can silently change a qualification or introduce a new assertion, so compare the edited version with the source text.

Programming and technical work

Ask for assumptions, supported versions and a small reproducible example. Read the documentation for the exact library or platform, inspect security-sensitive code and run tests before deployment. Never paste secrets or private data merely to obtain debugging help.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Health, legal and financial decisions

A chatbot can help you prepare questions or understand terminology, but its output is not a diagnosis, legal opinion or personalized financial advice. Confirm the relevant facts with a licensed professional or authoritative service, especially when timing, dosage, eligibility, deadlines or money are involved.

Prompts that improve transparency

Better prompts can expose uncertainty, although they cannot turn generated text into verified fact. Ask the system to:

  • “Separate your answer into checkable claims and label assumptions.”
  • “State what information may be out of date and the date you are using.”
  • “Give the original source for each quotation, number and technical requirement; if you cannot verify one, say so.”
  • “Present alternative interpretations of this ambiguous question and tell me what detail would distinguish them.”
  • “Do not guess. Ask a clarifying question or say that you cannot establish the answer.”

These prompts make gaps easier to see. Asking the same chatbot to “check itself” is not independent verification: it can repeat the original error or invent a second justification.

Why confidence is a poor reliability signal

People naturally use fluency, detail and a decisive tone as shortcuts for expertise. Chatbots are optimized to produce those qualities, so the shortcut fails. A short answer with a careful qualification may be more reliable than a long answer filled with dates and citations. Conversely, uncertainty language alone is not proof of correctness; it is simply an appropriate signal that you should investigate further.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OpenAI’s user guidance summarizes the practical rule as: “Use ChatGPT as a first draft, not a final source.” The same principle applies to other generative chatbots: use them to accelerate understanding and source-finding, then verify the claims that matter.

What chatbots cannot reliably determine about authorship

Do not use a chatbot as an authorship detector. OpenAI says ChatGPT cannot reliably tell whether it wrote a particular passage or whether text was generated by AI. A response claiming to identify its own writing is therefore not evidence of provenance. Establish authorship through document history, source records, drafting evidence or an institution’s approved process instead.

A compact verification checklist

  • What exact claims am I relying on?
  • Which claim would cause the most harm if false?
  • What date, location, edition, unit or definition applies?
  • Did the answer actually use a current source, or am I assuming it did?
  • Does each citation exist and support the precise nearby statement?
  • Did I check quotations, figures, calculations and technical details at the original source?
  • Are there conflicting sources or unstated assumptions?
  • Should a qualified professional make or confirm this decision?

The central lesson is simple: a chatbot’s confidence is a feature of presentation, not a measurement of truth. Generation can produce useful drafts, explanations and leads, while independent checking supplies the reliability that the model itself cannot guarantee.
