A chatbot predicts what text is likely to come next from patterns in data and the conversation. That process often produces useful answers, but it can also produce false facts, invented explanations, unsafe suggestions, privacy problems, bias, and advice that changes when the wording or context changes.
The practical rule is simple: treat a chatbot response as a draft claim to check, not as proof. The higher the stakes—health, law, money, personal safety, employment, or someone’s reputation—the more important independent verification and human judgment become.

What is happening when a chatbot answers?
Generative AI systems approximate statistical patterns learned from very large collections of text and other data. A language model then predicts a next token (a token may be a word, part of a word, punctuation, or another small unit) repeatedly until it has produced a response. The model is not retrieving a single, guaranteed record of truth each time it writes. It is constructing a sequence that fits the prompt and the patterns it learned.
This explains the combination of strengths and weaknesses. The same pattern-matching ability can summarize, translate, explain, and draft quickly. It can also blend details from different sources, fill gaps with a plausible-sounding guess, or continue an incorrect premise. A response can therefore be grammatically polished and internally coherent while still being factually wrong.

NIST describes this behavior as “confabulation.” Its 2024 Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile defines the term as: “Confabulation” refers to a phenomenon in which GAI systems generate and confidently present erroneous or false content in response to prompts. “Hallucination” and “fabrication” are common informal names for the same general problem.
Why open-ended questions are harder
Confabulation is especially relevant when a prompt asks for a long explanation, a niche fact, a detailed chronology, or advice requiring specialist context. The model has more opportunities to make unsupported connections as the answer grows. If the prompt contains an incorrect assumption, the system may accept that assumption and elaborate on it instead of stopping to challenge it.
Changing a few words can also change the output. Conversation history, requested tone, formatting instructions, language, and the model version all affect the probability distribution used to generate the next token. Two answers that disagree are not necessarily evidence that one was deliberately deceptive; they show that the system is generating a response under different conditions rather than consulting a fixed, perfectly consistent database.
Why does AI give confident but wrong answers?
Fluent language is an interface feature, not an accuracy certificate. Chatbots are trained to produce useful-looking responses and are often tuned to be helpful, direct, and conversational. Those qualities can make a weakly supported claim sound authoritative.
Plausible reasoning can be invented
NIST warns that a system may generate confabulated logic that appears to justify its conclusion. The explanation can be neat, step-by-step, and completely disconnected from a sound factual basis. A citation can be invented as well: a response may name a paper, court case, statistic, or web page that does not support the statement—or does not exist. A citation displayed by a chatbot has not been checked against its underlying source merely because it looks specific.

Confidence is not calibrated certainty
People often read wording such as “clearly,” “the answer is,” or “studies show” as a signal of confidence. In a generated response, those phrases usually reflect learned writing patterns, not a measured probability that the claim is true. A model can be highly certain in tone while lacking current information, misunderstanding the question, or combining unrelated facts.
Consequences depend on how the answer is used
An incorrect movie release date is inconvenient. An incorrect medication instruction, legal deadline, investment assumption, security procedure, or accusation about a person can cause serious harm. NIST specifically highlights the danger of people relying on convincing but erroneous output in consequential decisions. The system’s polished presentation can create automation bias: the tendency to defer to an automated answer instead of checking it.
Can an AI chatbot give harmful advice?
Yes. “Harmful” includes more than an ordinary factual mistake. NIST’s profile discusses outputs that may be dangerous, violent, hateful, threatening, inciting, or radicalizing. It also identifies recommendations involving self-harm, criminal or illegal activity, and other unsafe conduct as risks.
| Risk area | How it can appear in an answer | Why the risk matters | Practical response |
|---|---|---|---|
| Dangerous or violent content | Instructions, encouragement, glorification, or escalation of violence | Specific suggestions can make harmful action easier or normalize it | Do not follow the response; seek qualified emergency, crisis, or safety help when there is immediate danger |
| Self-harm recommendations | Unhelpful or unsafe responses to a person describing distress | A vulnerable person may treat a conversational system as a trusted counselor | Use a licensed professional or crisis service, and involve a trusted person in urgent situations |
| Criminal or illegal activity | Advice that facilitates evasion, abuse, fraud, or other unlawful conduct | The output may omit legal, ethical, or real-world consequences | Do not use generated instructions as legal permission or operational guidance |
| Hateful or demeaning content | Stereotypes, slurs, or claims portraying a group as less worthy or dangerous | It can reinforce discrimination and cause reputational or personal harm | Question the premise, check for evidence, and do not treat generated generalizations as facts |
| Dangerous everyday recommendations | Unsafe advice about chemicals, equipment, health, or physical activity | Missing context can turn a seemingly reasonable suggestion into an injury risk | Use authoritative instructions and professional advice for high-risk tasks |
Content filters and refusal rules can reduce some of these outputs, but they are not a guarantee. NIST notes that systems may still produce harmful recommendations when a request is novel, indirect, or less explicit. Deliberate attempts to bypass controls are commonly called jailbreaking. A refusal in one conversation therefore does not prove that every related wording will be blocked.
How can privacy be harmed?
Privacy risks occur at both training time and output time, and they are not identical across services. NIST describes the possibility that a model could leak or generate sensitive information present in training data. It also discusses sensitive inferences: a system may assemble clues from disparate sources and infer something personal, even when the inference is wrong.

Do not assume the same data policy everywhere
It is not accurate to claim that every chatbot retains every conversation or uses every conversation for training. Storage, review, deletion, and training practices are service-specific. Before entering personal information, read the particular service’s current privacy and data-use settings and understand who can access the content, how long it may be retained, and whether an organization controls the account.
Information can become identifying when combined
A name is not the only sensitive detail. A combination of employer, location, dates, medical symptoms, account numbers, family details, or internal project language may identify a person or organization. Inference can also be harmful when inaccurate: an incorrect suggestion about health, finances, identity, or intent can affect decisions made about someone.
Safer handling of sensitive prompts
- Remove names, addresses, account numbers, passwords, access tokens, private keys, and confidential business details.
- Replace identifying facts with neutral placeholders and keep the minimum context needed for the task.
- Use approved enterprise tools and retention settings when an organization has specified them.
- Assume that anything submitted could be exposed to more people or systems than you intended unless the service’s controls say otherwise.
Why can answers be biased, inconsistent, or emotionally risky?
Bias and homogenization
Training data contains uneven representation, stereotypes, and conflicting viewpoints. A model can reproduce those patterns or present a narrow, dominant framing as if it were neutral. NIST identifies harmful bias and homogenization as risks: repeated, standardized outputs can reduce diversity of perspectives while still sounding objective. Check claims about groups, people, and social issues against evidence and consider voices or sources the response may have omitted.
Information integrity
Generated text can be used to produce convincing but false material at scale. Even when a user has no malicious intent, an unverified summary can be copied into a report, post, or decision and then repeated as fact. Distinguish original evidence from a chatbot’s paraphrase, and preserve the source documents needed to audit an important statement.
Emotional entanglement and over-reliance
NIST also discusses possible negative psychological effects when people become emotionally entangled with a chatbot. People in distress may disclose mental-health concerns and react badly to an unhelpful or inappropriate response. A system can imitate empathy without understanding a person’s circumstances or accepting responsibility for care. Treat supportive language as conversational style, not as a therapeutic relationship or a substitute for human help.

How can I tell whether an AI answer is trustworthy?
No single wording test proves that an answer is reliable. Use a process that matches the consequences of being wrong.
- Classify the decision. Decide whether the answer is for brainstorming, a low-stakes fact, or a consequential action. The higher the stakes, the less acceptable an unchecked answer is.
- Separate claims from presentation. Extract dates, numbers, names, causal statements, instructions, and quoted material. Ignore how polished the prose sounds.
- Ask for the basis, then verify it. Request the specific source, section, calculation, or assumption behind each important claim. Open the cited primary source yourself; do not assume a displayed citation was checked.
- Cross-check independently. Compare the claim with authoritative sources such as a government publication, professional regulator, court document, manufacturer manual, or qualified practitioner. Use a source that did not simply copy the chatbot’s wording.
- Check date and scope. Confirm the jurisdiction, edition, version, population, and effective date. A true statement in one country or year may be wrong for your situation.
- Look for internal contradictions. Ask the system to list assumptions and uncertainties, then compare those with the main answer. A confident conclusion that depends on an unstated assumption deserves extra scrutiny.
- Use a qualified human for high stakes. Medical, legal, financial, safety, employment, and safeguarding decisions require an accountable professional or responsible authority.
- Record what you verified. For work or compliance decisions, save the source links, dates, calculations, and human approval rather than relying on a chat transcript alone.
What safeguards can and cannot do
Safeguards include training choices, content filters, access controls, monitoring, testing, human review, privacy controls, and user warnings. They operate at different points in a system’s lifecycle and address different risks. A filter aimed at violent instructions does not establish factual accuracy, and a privacy control does not eliminate bias or automation bias.
NIST’s AI Risk Management Framework and its Generative AI Profile treat trustworthiness as a lifecycle responsibility spanning design, development, deployment, use, and testing or evaluation. The framework’s dimensions include validity and reliability, safety, security and resilience, accountability and transparency, explainability, privacy, and fairness. NIST’s framework FAQ says: “The NIST Artificial Intelligence Risk Management Framework (AI RMF 1.0) is intended to be a living document.” The framework is voluntary, and NIST emphasizes that trustworthiness characteristics must be considered together and in context.
The 2024 Generative AI Profile identifies 12 generative-AI risk areas and just over 200 suggested management actions. Those figures describe the breadth of the guidance, not a count of chatbot incidents and not proof that every action works equally well. No single safeguard guarantees that an individual answer will be safe, accurate, private, unbiased, or appropriate.

A useful way to evaluate a safeguard
When comparing a control, product feature, or organizational process, ask four questions:
Best Value
- Which risk does it address? Accuracy, harmful output, privacy, security, bias, or over-reliance are different problems.
- Where does it operate? It may act during data preparation, model development, deployment, user interaction, or post-release monitoring.
- Who is accountable? A model provider, deploying organization, administrator, reviewer, or end user may have different responsibilities.
- What evidence supports the claim? Look for a defined test, relevant population, documented limitations, and results that can be independently checked.
What should people do in common high-stakes situations?
Health and mental health
Use a chatbot for questions to take to a clinician, not for diagnosis, medication changes, emergency triage, or crisis care. Confirm urgent symptoms and treatment decisions with a qualified professional. If someone may be in immediate danger, contact local emergency or crisis services and a trusted person rather than continuing a chatbot conversation.
Legal and financial decisions
Generated explanations can omit jurisdiction-specific rules, deadlines, fees, exceptions, and conflicts of interest. Verify the current law, contract, filing requirement, or financial product with the responsible authority or a qualified adviser before acting.
Work, school, and public claims
Check quotations, statistics, references, and allegations before submitting or publishing them. Protect confidential records and disclose or follow the applicable policy when generated material contributes to a decision. A fluent paragraph is not evidence of authorship, originality, or truth.

The cited NIST materials do not provide a reliable, general percentage for how often consumer chatbots produce harmful or unpredictable answers, nor do they rank one safeguard as universally most effective. NIST AI 600-1, published July 26, 2024, is a cross-sector risk-management profile rather than a randomized study of consumer outcomes. Frequencies vary with the model, prompt, data, safeguards, user behavior, and use case, so a single harm rate would be misleading.
The practical answer
AI chatbots can be useful because they generate coherent language quickly, but the mechanism that creates fluency also permits confident error. Treat every consequential factual claim, explanation, citation, and recommendation as something to verify. Minimize sensitive data, use authoritative sources and accountable professionals for high-stakes decisions, and remember that safety controls lower risk without making any chatbot infallible.
