Why AI Agents Repeat Mistakes Across Turns and How to Prevent It

An AI agent repeats a mistake when the correction is not available, not retrieved, or not trusted at the moment it decides. A later model call does not automatically learn from an earlier turn. The application must preserve the right state, carry it into the next run, retrieve relevant memories, and give the agent a way to revise a bad assumption. Even with a complete transcript, an agent can remain anchored to an early error.

The reliable fix is an explicit memory pipeline: keep short-lived session state separate from durable memory, save and resume the correct session, extract reusable facts instead of replaying every thought, retrieve progressively, check recalled claims against current instructions and live tools, and test recovery as a first-class capability.

What is actually causing the repetition?

“Memory” is not one feature. Several independent failures can produce the same symptom—your agent does the same wrong thing after you already corrected it.

There is no durable state

Many agent applications create a new session for each request. The new run receives the current prompt and tool definitions but none of the previous failure, repair, preference, or decision. IBM Research’s preliminary report Memory as Middleware for Self-Improving AI Agents describes agents as “stateless across sessions by default and therefore operationally amnesic.” That is a description of a default architecture, not a claim that every framework behaves identically.

The previous state exists but was not resumed

A framework may support persisted transcripts, checkpoints, or memory stores while the application still starts an empty conversation. Common causes include generating a new conversation identifier, writing to one store and reading from another, expiring the record too soon, or launching a fresh sandbox whose memory files are empty. Persistence only helps when a later run points to the same persisted state and the framework’s resume contract is followed.

The correction was lost to context limits

Models have a bounded context window. Long histories can be truncated, summarized, or selectively loaded. The OpenAI Agents SDK documentation notes that overlong conversations may be truncated in a way that preserves the beginning and end; a correction in the middle can disappear. A transcript that looks complete in a database is not necessarily complete in the model input.

An early wrong assumption became an anchor

Microsoft Research’s LLMs Get Lost In Multi-Turn Conversation reports that models often make an early assumption, attempt a solution prematurely, and then over-rely on that path. The publication page summarizes the problem this way: “when LLMs take a wrong turn in a conversation, they get lost and do not recover.” This is a recovery failure, not merely a storage failure. Supplying the old transcript can even strengthen the anchor if the mistaken reasoning is presented as authoritative.

Raw replay carried noise or stale reasoning

Every prior thought, tool trace, and temporary hypothesis is not a useful memory. Apple Machine Learning Research’s 2026 study of selective persistent memory found that retaining reusable task information while discarding session-specific reasoning outperformed both full-history persistence and a no-memory baseline in its evaluated deployments. The result supports selective retention for those tasks; it does not prove that a summary is always better for every agent.

The memory is irrelevant, stale, or wrongly scoped

A preference for one project may be injected into another. A tool schema may have changed. A remembered workaround may have been valid only for an old software version. The OpenAI Agents SDK documentation treats memory as guidance and describes updating stale memories. Microsoft’s multi-agent architecture guidance emphasizes scope, governance, security, and forgetting. Without those controls, an apparently helpful memory becomes a durable source of error.

Session history and durable memory solve different problems

Keep these concepts separate in both your design and your debugging.

Type Purpose Typical contents Lifetime Failure when missing
Session history Continue one interaction or workflow Recent user messages, tool calls and outputs, current plan, pending approvals Until the task ends or the session expires The agent loses immediate context, including an in-progress correction
Durable memory Carry selected knowledge into later conversations Stable preferences, decisions, constraints, schemas, tool configuration, reusable procedures Until revised, deleted, or expired by policy The agent starts future work without known preferences or prior repairs
Authoritative knowledge retrieval Supply facts that change independently of a conversation Current policies, product records, permissions, documentation, operational data Maintained by the source system The agent relies on an outdated personal memory or an unauthorized copy

A production agent may need all three. Session state answers “what is happening in this task?” Durable memory answers “what should remain useful later?” Authoritative retrieval answers “what is true now, and may I access it?” Do not use an ungoverned personal memory store as a substitute for permission-checked enterprise knowledge.

How a reliable memory pipeline works

Think of each model call as a pipeline rather than a magical conversation.

  1. Identify the continuity boundary. Decide whether the next call belongs to the same task, a related task, or a new conversation. Assign a stable session or conversation identifier where the framework requires one.
  2. Load state deliberately. Retrieve the persisted session checkpoint, completed tool outputs, pending actions, and relevant durable memories. Verify that the loaded record belongs to the intended user, project, and workflow.
  3. Build a compact working context. Include current instructions first, then a concise task state, relevant memories, and only the tool records needed to continue. Keep the assembled input within the model’s context budget.
  4. Expose provenance and uncertainty. Store when a memory was created, what interaction or document supports it, its scope, and whether it is confirmed, inferred, or unresolved. This lets the agent treat a recollection as a candidate claim rather than an instruction.
  5. Check contradictions. Compare recalled constraints with the latest user message, live tool results, current schemas, and access permissions. If they conflict, ask for clarification or follow the higher-authority current source.
  6. Run the next action. Require a fresh plan when the previous attempt failed, rather than allowing the old plan to flow through unchanged.
  7. Write back selectively. Save confirmed decisions, durable preferences, repaired procedures, and useful outcomes. Do not automatically promote every intermediate thought or failed hypothesis.

Prevention plan: implement it step by step

1. Define what must survive

Write a continuity specification before choosing a memory database. For each item, mark whether it belongs to:

  • the current task only;
  • future conversations with the same user;
  • a project or team scope;
  • an authoritative source that should be retrieved rather than copied into memory.

Include privacy and deletion requirements in the same specification. A user preference may be durable, while a temporary password, one-time token, or sensitive document excerpt should not be.

  • [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by

2. Persist completed state and resume the same identity

At the end of every meaningful turn, checkpoint the state needed to continue: user and task identifiers, the current plan, completed tool calls and outputs, pending approvals, and the latest correction. On resume, log the identifier being requested and the identifier actually loaded. A successful database write is not proof that the next run read it.

Framework implementations differ. The OpenAI Agents SDK and Microsoft’s session guidance both make persistence contingent on explicitly retaining or resuming state. Follow the selected framework’s ownership rules for sessions, checkpoints, and storage; do not assume that a new process or sandbox inherits prior files.

3. Extract reusable information instead of treating the transcript as memory

After a task, run a memory-extraction step with a strict schema. Apple’s reported approach retains reusable task specifications, data schemas, tool configurations, and output constraints while discarding session-specific reasoning traces. A practical record can contain:

  • Claim: the preference, decision, constraint, or procedure.
  • Scope: user, project, team, environment, or task.
  • Evidence: the message, tool result, or document that supports it.
  • Status: confirmed, inferred, disputed, or superseded.
  • Freshness: creation time, last verification time, and an expiry rule where appropriate.

Store a failed attempt as a lesson only when the failure and its remedy are clear. “The API failed” is not reusable guidance; “Endpoint X rejects payload Y; use schema Z, verified on date D” is.

4. Retrieve progressively

Do not inject every memory into every prompt. Start with a compact index or summary containing candidate memories and their scopes. Open the underlying episodic record only when the current task needs detail. This reduces context pressure and lowers the chance that an old, irrelevant instruction competes with a current one.

Use explicit retrieval filters for user, project, environment, data classification, and time range. Rank by relevance and authority, not only by semantic similarity. A recent, user-confirmed constraint should outrank an older inferred preference.

5. Make contradiction and staleness checks mandatory

Before using a recalled item, ask three questions:

  1. Does the latest user instruction override it?
  2. Does live system state or a tool result contradict it?
  3. Is it still inside its scope and freshness window?

If the answer to any check is negative, mark the memory stale or disputed, exclude it from the action plan, and record the update. The SDK documentation specifically describes updating stale memories; implement that behavior as a write operation, not merely a prompt suggestion.

6. Separate memory from authoritative knowledge

Microsoft’s multi-agent reference architecture warns: “A frequent design mistake is to treat enterprise content as memory.” Policies, inventory, permissions, prices, and other changing facts should be retrieved from current, access-controlled systems at request time. Persist only the user- or task-specific context that makes future work more efficient. This preserves authorization checks and avoids silently retaining a document that has since changed or been revoked.

7. Add a recovery instruction after failure

When a tool call fails, save the error, input, environment, and attempted remedy as a structured event. On the next turn, tell the planner that the previous action failed and require a different plan or an explicit justification for retrying. A generic “try again” instruction is insufficient: it often reproduces the identical call.

Should you save the whole conversation or summarize it?

Neither option is universally correct. Use the representation that matches the task and the cost of being wrong.

Approach Strength Risk Best fit
Full transcript Preserves exact wording, chronology, and tool evidence Consumes context, carries stale reasoning, and can preserve an early anchor Auditing, dispute resolution, or short sessions where every detail matters
Compact summary Low context cost and fast resumption May omit a qualification or misstate a correction Long-running workflows with stable state and a verification path
Structured semantic memory Precise retrieval of preferences, constraints, and schemas Extraction errors or incorrect scope can become durable Recurring tasks with identifiable reusable facts
Episodic records on demand Retains detailed evidence without injecting it every time Requires effective retrieval and indexing Investigations, tool-heavy work, and workflows that need occasional deep history
Hybrid Combines a small working summary, structured facts, and expandable evidence More components to govern and test Most production systems with both continuity and audit needs

Apple Machine Learning Research reported, across three enterprise deployment scenarios, 96% task completion with selective memory, versus 79% without memory and 71% with full history. The same page reported a 14× task-time reduction for zero-token refresh, 97× lower per-invocation token cost for summary-driven generation compared with raw data injection, and 12/12 success in a replication on four public datasets. These are the authors’ results for their evaluated systems and tasks, not guarantees for another architecture.

How to stop an agent repeating a failed tool call

  1. Capture the complete failure event. Record the tool name, normalized arguments, timestamp, environment, response, error class, and whether the action changed external state.
  2. Classify retry safety. Mark the operation idempotent, conditionally safe, or unsafe. Require confirmation before repeating an unsafe action.
  3. Check for changed conditions. Re-read current permissions, resource status, schema versions, and prerequisite outputs. A retry is justified only if the relevant condition has changed or the first call was not actually delivered.
  4. Generate a new plan. Present the failure as a constraint: “the previous payload was rejected for reason R.” Ask the planner to choose a corrected payload, an alternative tool, or a clarifying question.
  5. Deduplicate at the tool boundary. Attach an idempotency key or operation identifier where the tool supports one. Reject an identical pending call unless the agent supplies a changed argument set or an explicit override.
  6. Persist the repair. Save the corrected schema or procedure only after the tool confirms success. Include the evidence and scope so the repair does not become a universal rule.

Evaluation: test recurrence, not just single-turn accuracy

A single-turn benchmark cannot show whether an agent remembers a correction, resumes after a restart, or recovers from an early wrong assumption. Microsoft Research’s 2025 study compared single- and multi-turn settings across six generation tasks, analyzing more than 200,000 simulated conversations. Its page reports an average 39% performance drop in multi-turn use, including increased unreliability and persistence of early wrong assumptions. Treat that figure as the result of the reported setup, not as a universal production failure rate.

Build an evaluation set that deliberately includes:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • a user correction followed by several unrelated turns;
  • a process restart and a resumed session;
  • an interruption while a tool call is pending;
  • a stale preference contradicted by a current instruction;
  • a changed schema or permission;
  • an identical failed tool call that must not be repeated;
  • two projects with similar terminology but different constraints.

Measure more than final success:

Metric Question
Correction recall Did the resumed agent retrieve the relevant repair?
Correction adherence Did it follow the repair instead of merely mentioning it?
Resume fidelity Were completed turns, tool outputs, and pending actions restored?
Stale-memory rate How often did an outdated item influence an action?
Duplicate-action rate How often did it repeat an identical failed or unsafe call?
Scope accuracy Did a memory stay within the right user, project, and environment?
Recovery quality After an early wrong turn, did the agent re-plan or defend the old path?
Context and token cost How much input and repeated generation did the memory strategy consume?

Run the same scenarios with different memory policies—none, full history, selective facts, and a hybrid—and report the task, model, tools, context limits, and deployment setup alongside the result. Evaluation numbers without that context are easy to misapply.

Governance, privacy, and deletion are part of correctness

Memory can cause a security failure even when the model’s answer is linguistically correct. Scope every record to the minimum audience and purpose. Enforce access checks at retrieval time for shared documents. Encrypt sensitive stores, audit reads and writes, define retention periods, and provide a deletion path that removes both the structured memory and any searchable episodic evidence derived from it.

Do not let a model silently convert a private conversation into team knowledge. Require confirmation for memories that affect other users, external actions, compliance decisions, or durable system configuration. Keep provenance so an administrator can answer why a memory exists and which source authorized it.

Troubleshooting checklist

  • The agent forgets immediately: verify that the current call includes the expected session history and that the session identifier is stable.
  • It remembers old facts but ignores a new correction: inspect retrieval ranking, scope filters, freshness checks, and instruction precedence.
  • It loses a correction in long conversations: inspect the final model input for truncation or summarization and promote the correction into a compact, structured state record.
  • It repeats a failed call after restart: persist the failure event and idempotency key, then load them before planning.
  • It applies one project’s rule to another: add project and environment namespaces and reject memories without a matching scope.
  • It cites a remembered policy that changed: replace copied policy text with permission-aware retrieval from the authoritative source.
  • It becomes less reliable after adding memory: compare selective retrieval with full-history injection; remove stale reasoning traces and measure context pressure.

What the evidence supports—and what it does not

The available studies and documentation support a practical conclusion: continuity is an application responsibility, selective memory can outperform indiscriminate replay in evaluated settings, and multi-turn reliability must be measured separately from single-turn quality. They do not establish one universally best framework, vector store, summary format, or memory database. Choose based on continuity scope, data sensitivity, state ownership, retrieval quality, governance requirements, and results on tasks that resemble your deployment.

The most dependable agent is not the one with the longest transcript. It is the one that resumes the right state, retrieves only relevant evidence, treats memory as fallible, notices contradictions, and records a verified repair instead of preserving an attractive explanation for the original mistake.
