Why AI Agents Make Mistakes With Company Data—and How to Fix It

AI agents make mistakes with company data for two connected reasons: they can only synthesize what their retrieval system can find and they may be given more authority than the evidence justifies. Missing or stale records produce ordinary quality errors; poisoned documents, hidden instructions, incorrect permissions and unrestricted write tools turn the same weakness into a security incident.

The practical remedy is a controlled evidence pipeline. Assign owners to authoritative sources, retrieve only data the requesting identity is allowed to see, preserve provenance, test retrieval independently from generation, and require deterministic checks or human approval before consequential actions. This approach also answers the common context-window concern: retrieve a small, relevant, current and authorized set of passages instead of copying an entire company knowledge base into every prompt.

What an AI agent can—and cannot—know

An agent does not have a complete, self-updating copy of a company. It combines its model with the documents, databases, memories and tool results made available at runtime. If an authoritative policy is absent, inaccessible, stale or ranked below a contradictory page, the model has no reliable basis for choosing the right answer.

Microsoft Learn states the dependency plainly: “Because agents synthesize information rather than create it, their accuracy depends entirely on the quality and accessibility of underlying sources.” That is a statement about architecture, not a measured error rate. It means a fluent answer is not evidence that the underlying records were complete, current or authorized.

Retrieval is therefore a trust boundary. A passage returned by search, a memory entry, a web page or a tool response is data supplied to the agent, not automatically a trusted instruction. The agent must preserve the distinction between governing instructions and untrusted content while also checking whether the caller may use that content.

Six ways company-data agents go wrong

Incomplete, fragmented or stale sources

Company knowledge is often split across ticket systems, shared drives, wikis, spreadsheets, contracts and application databases. Different teams may maintain overlapping copies with different update schedules. A model can summarize only what is indexed and retrievable, so it may omit an exception, use an expired procedure or combine two versions that were never meant to be read together.

Ownership and lifecycle gaps make the problem persistent. If nobody is responsible for a document domain, there may be no defined freshness target, retention rule or process for retiring obsolete content. An agent can then be technically available while its evidence base quietly degrades.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Retrieval that finds the wrong evidence

A search layer can miss the system of record, rank a persuasive but unofficial page above an approved policy, or apply an access rule incorrectly. Relevance alone is not enough. Retrieval needs identity-aware authorization, metadata filters, freshness signals and citations that let a reviewer inspect the exact source used.

A generated answer can sound certain even when the top result was merely similar to the question. The failure occurred before generation: the agent was supplied with the wrong evidence or with no evidence that met the required authority threshold.

Poisoned documents, indexes or embeddings

Grounding content can be manipulated. An attacker might edit an internal wiki, compromise a public page that is indexed, insert misleading text into a document, or tamper with vector-index content and embeddings. The resulting passage may look like normal company evidence and produce a coherent but false answer.

Ingestion is consequently part of the security boundary. Restrict who can add, alter or delete indexed content; scan material before embedding; record document versions and changes; and watch for unusual shifts in what the retriever returns.

Instructions hidden inside data

Retrieved text, memories and tool outputs may contain imperative language such as instructions to ignore policy, disclose secrets or call another tool. Treating those words as higher-priority guidance creates prompt-injection risk. The agent should parse such content as untrusted data, extract only the fields the task requires and apply policy checks before acting on any requested operation.

More authority than the task requires

An incorrect answer is limited when an agent is read-only and a person verifies it. The consequences change when the same agent can edit records, send messages, approve payments, change access or delete data. Microsoft’s risk guidance summarizes the control principle: “Apply least privilege and least action. Allow only the minimum tools, data, and operations required. Deny everything else by default.”

Write capability should be separated from read capability. Sensitive, high-impact or difficult-to-reverse operations need deterministic validation and an approval gate rather than a model’s unreviewed decision.

Unclear ownership of agents and dependencies

Every agent has dependencies: identity providers, indexes, connectors, tools, prompts, models and source systems. If those components are unmanaged, it becomes difficult to determine who approved a permission, who changed a retrieval rule or who is accountable for an action. An inventory with named owners and change records is a reliability control as well as a security control.

How to use current company data without filling the context window

The informal question people often ask is how to ground an agent in current company data without placing the entire knowledge base in one prompt. The answer is selective retrieval, not indiscriminate context stuffing:

  1. Index governed sources. Connect the systems of record and attach metadata such as owner, document type, effective date, sensitivity and retention status.
  2. Filter before ranking. Apply the caller’s identity, tenant or role and mandatory metadata constraints before selecting passages. A highly relevant document that the caller cannot access must never enter the prompt.
  3. Retrieve a task-sized evidence set. Search for the specific question or subtask, then return only the passages needed to support it. Use scope, date and source-type filters to reduce irrelevant material.
  4. Expose provenance. Keep document identifiers, versions and locations with every passage so the answer can cite what it relied on and a reviewer can open the permitted source.
  5. Check freshness and conflicts. Prefer the effective version, flag expired material and route contradictory authoritative sources for resolution instead of asking the model to guess.
  6. Separate evidence from instructions. Place retrieved text in a clearly identified data channel and keep system and developer controls outside that channel. Do not allow a passage to redefine the agent’s permissions.

This design reduces context size while improving traceability. It does not guarantee correctness: retrieval quality, source governance and authorization still determine what the model sees.

A remediation sequence that works in practice

1. Define authoritative sources and owners

Start with the business questions the agent must answer. For each data domain, record the system of record, accountable owner, permitted audiences, expected update interval, retention period and conditions under which a record becomes invalid.

  • Document acceptable staleness, such as real-time, daily or version-based freshness.
  • Mark duplicates and unofficial guidance so they cannot outrank approved material by accident.
  • Assign an owner who can approve changes and respond when sources conflict.

Microsoft’s architecture guidance recommends documenting retrieval decisions and data domains. Without this inventory, later tests cannot establish whether a missing answer reflects a retrieval bug or a missing source.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Enforce authorization during retrieval

Authorization must happen before content is placed in the agent’s context. Use document-level permissions or security trimming tied to the requesting identity. Managed identities can reduce the risk of embedding long-lived credentials in prompts or connectors.

  • Pass the caller’s identity and relevant claims to every retrieval request.
  • Apply tenant, department, matter, classification and effective-date filters before ranking results.
  • Re-check authorization when a memory is written or reused; a once-authorized result may become restricted later.
  • Return source identifiers and citations only for content the caller is allowed to inspect.

Do not infer permission from the model’s response. A polished answer can accidentally combine records from users with different access rights if the retrieval layer failed to trim them.

3. Keep instructions and data separate

Define a strict trust model for each input channel. System and developer policies control behavior; retrieved documents, memories, user text and tool responses are untrusted inputs unless an independent control promotes a specific field.

  • Sanitize markup and unexpected control sequences before indexing or displaying content to the model.
  • Inspect retrieved passages for instructions that conflict with policy or attempt to redirect the task.
  • Extract structured fields into schemas and reject values that fail type, range or allow-list validation.
  • Require a policy decision outside the model before a requested tool call can change data or disclose restricted information.

Prompt wording alone is not a complete defense. A malicious instruction can arrive through a document or a connector even when the user’s request is harmless.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

4. Protect ingestion and the index path

Security controls must cover the route from source system to retriever. Limit write, modify and delete permissions on indexes; authenticate connectors; and keep an audit trail of who changed a document, chunk, embedding or metadata filter.

  • Scan documents for malware, suspicious markup and unexpected instructions before embedding.
  • Record source version, ingestion time, effective date and hash or equivalent change identifier.
  • Refresh indexes on a schedule appropriate to the source’s freshness requirement, and remove superseded material.
  • Monitor retrieval distributions, sudden ranking changes and unusual spikes in content from one source.
  • Rebuild or quarantine affected index partitions when a source or embedding pipeline is suspected of compromise.

These measures address both accidental drift and deliberate poisoning. A vector index should be treated as an operational system, not an immutable cache.

5. Test retrieval separately from generated answers

Evaluate the evidence pipeline before judging prose quality. A representative test set should include routine questions, ambiguous requests, permission-boundary cases, stale-versus-current conflicts and adversarial documents containing hidden instructions.

  1. Verify that the expected authoritative source is retrieved.
  2. Verify that unauthorized sources are excluded for each test identity.
  3. Check that returned passages meet the required freshness and version rules.
  4. Confirm that citations identify the actual passages used.
  5. Ask the model to answer only from the supplied evidence and mark unsupported questions as unresolved.
  6. Repeat the tests after material changes to prompts, models, connectors, retrieval settings or tools.

Microsoft recommends adversarial test harnesses before material changes. Retrieval recall and authorization failures should be fixed in the retrieval layer rather than hidden by asking the model to sound less confident.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Constrain tools and gate consequential actions

Give an agent only the data, tools and operations required for its defined task. A support triage agent may need read access to tickets and product documentation but not permission to close accounts. A reporting agent may prepare a change set but not apply it.

  • Default to read-only connectors.
  • Use separate identities and credentials for reading and writing.
  • Validate parameters deterministically, including targets, amounts, recipients, dates and allowed state transitions.
  • Require a person to approve sensitive, high-impact or irreversible operations.
  • Provide a pause, revoke or stop mechanism that works independently of the model.

NIST’s workshop summary illustrates a useful spectrum: read-only, constrained-write and write patterns suit different environments and risks. The correct level depends on the operation, access pattern, reliability requirement, modality and monitoring available; it is not a universal autonomy ranking.

7. Make behavior auditable

Record the request identity, retrieved source identifiers and versions, model and prompt configuration, tools called, parameters, approvals, outputs and final outcomes. Logs should be protected from unauthorized alteration and retained according to the applicable policy.

Where appropriate, show the agent’s plan, evidence and pending action to the reviewer. Monitor source changes, connector health, permission failures, citation coverage and unusual tool activity. An audit trail turns a vague complaint that an agent was wrong into a reproducible question: which identity retrieved which version, under which policy, and what action followed?

Choosing a retrieval architecture

Built-in retrieval, a custom retrieval-augmented generation (RAG) service and agentic retrieval can all be valid. Compare them against the controls your use case needs rather than assuming one pattern is universally superior.

Evaluation axis Built-in retrieval Custom RAG Agentic retrieval
Authoritative source coverage Strong when supported connectors map cleanly to systems of record; gaps require documented exceptions. Can cover specialized or proprietary systems, but the team owns connector quality. Can select among sources dynamically, increasing the need for source allow-lists and oversight.
Freshness Depends on the platform’s synchronization and refresh controls. Defined by the ingestion and re-indexing pipeline you operate. May query live tools, but each tool’s latency, failure and freshness behavior must be controlled.
Permission enforcement Use the platform’s identity, document-level controls and security trimming where available. Requires you to implement and test authorization at every connector and retrieval step. Must enforce permissions across every dynamically chosen source and tool.
Provenance and citations Often available through platform metadata; verify that citations are visible to the intended user. Fully configurable, with corresponding logging and maintenance responsibility. Must preserve provenance across multiple searches and intermediate plans.
Relevance and answer quality Convenient defaults; tune ranking, metadata filters and source scopes when needed. Maximum control over chunking, ranking and evaluation, with greater engineering effort. Can decompose complex questions, but extra steps create more opportunities for drift or injection.
Auditability and change monitoring Use the provider’s logs and controls, then verify retention and export requirements. Design your own complete event and change records. Log every subtask, retrieval decision and tool call to reconstruct the plan.
Integration and maintenance burden Lower when requirements fit the platform; document any compliance or identity exceptions. Higher operational responsibility for connectors, indexes, tests and upgrades. Highest orchestration complexity because the agent chooses actions and sources at runtime.
Read-only versus write access Keep retrieval and action permissions separate; use built-in approval controls if they meet requirements. Implement separate service identities, validators and approval workflows. Use strict tool allow-lists and independent gates because dynamic planning expands the attack surface.

Microsoft recommends built-in retrieval as a default when its accuracy, compliance, identity and auditing controls meet the requirements. That is vendor guidance for its platform, not an independent benchmark. If a built-in option cannot satisfy a documented requirement, record the exception and specify which custom control replaces it. Microsoft-managed search services, including Azure AI Search, may be relevant for permission-aware retrieval, but suitability still depends on the implementation and governance around them.

Controls for ambiguous and conflicting answers

Not every disagreement should be resolved by ranking. Establish deterministic rules for common conflicts:

  • Prefer the current effective version over an expired version.
  • Prefer the designated system of record over a copy in a general wiki.
  • Require the same customer, case, region or product scope before combining records.
  • Return an explicit conflict or escalation when two authoritative sources disagree.
  • Decline to answer when no source meets the task’s authority, freshness or permission threshold.

Confidence language is useful only when tied to evidence. A low-confidence label cannot compensate for a citation to an unauthorized or poisoned document.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Operational monitoring after launch

Reliability work continues after deployment. Track signals that reveal both quality drift and attack activity:

  • Percentage of answers with citations to permitted, current sources.
  • Retrieval misses on a maintained test set, including adversarial cases.
  • Permission-denied events and attempted access outside the agent’s scope.
  • Changes in source freshness, document ownership and index composition.
  • Unexpected shifts in ranking, embedding distributions or source usage.
  • Tool-call volume, rejected parameters, approval overrides and emergency stops.
  • Incidents in which a human corrected an answer or reversed an action.

Use these measures for investigation and regression testing, not as a claim that a single score proves reliability. A high citation rate can still hide a wrong source, and a low intervention rate can reflect insufficient review rather than accurate automation.

What the available evidence does—and does not—show

The guidance behind these controls describes threats, architecture patterns and governance practices; it does not establish a representative enterprise error rate or a percentage of agents that fail because of company data. No such prevalence figure should be inferred from the recommendations.

NIST’s 2025 article reports that approximately 140 experts attended the January workshop it summarizes. That attendance number describes participation in a taxonomy discussion, not agent reliability or incident frequency. The taxonomy work is presented as ongoing rather than a final, comprehensive classification.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Microsoft’s recommendations should likewise be read as Microsoft guidance, including its advice about built-in retrieval and permission-aware architecture, rather than as neutral comparative testing.

Deployment checklist

  • Every data domain has a named owner, system of record, freshness target and retention rule.
  • Retrieval applies document-level authorization or security trimming before content enters context.
  • Managed identities, metadata filters and source citations are configured and tested.
  • Prompts, retrieved text, memories and tool results have separate trust levels.
  • Documents are scanned before embedding, and index changes are versioned and auditable.
  • Representative and adversarial retrieval tests run before and after material changes.
  • Read and write identities are separate, with least-privilege tool allow-lists.
  • Parameters are validated outside the model, and high-impact actions require approval.
  • Logs capture sources, versions, tools, parameters, approvals and outcomes.
  • A reliable pause, revoke or stop mechanism has been exercised in an operational drill.

An agent becomes dependable when its evidence, identity, permissions and actions are controlled as one system. Better prompting may improve phrasing, but it cannot repair an inaccessible source, an incorrect permission decision or an unrestricted write path.
