An AI agent can finish a run and still give a wrong answer, choose the wrong tool, violate a constraint, or make an unsafe change. A completed run proves that execution ended—not that the result was correct or acceptable. To debug an agent that keeps making mistakes, inspect the run’s trace, identify which layer failed, and replay the failure as a test after making a targeted fix.
Why does my AI agent fail in production even when the run completes?
Because many agent failures are failures of outcome, not failures of execution. The system may return a plausible but incomplete answer, take an inappropriate action, or produce a result that conflicts with policy without throwing an exception. Amazon CloudWatch documentation warns that “Agents fail in ways that do not show up as errors: the run completes, but the answer is wrong, incomplete, or against your policies.”

That distinction changes what you monitor. Uptime, latency, and exception rates can tell you whether a service is running; they cannot establish whether the agent solved the task correctly or acted within its constraints. Review the final result against the task and policy, and inspect the steps that led to it.
How do I find the layer that failed?
Start with one representative failed run and inspect its end-to-end trace: the input, model calls, tool calls and results, handoffs, guardrail decisions, and final state. OpenAI’s agent-evaluation documentation describes a trace as “the end-to-end record of model calls, tool calls, guardrails, and handoffs for one run.” That record helps locate where behavior went wrong; it does not, by itself, prove the result was correct.

Before changing a prompt, label the failure. The right fix depends on whether the agent misunderstood the task, used a tool incorrectly, encountered an integration problem, or crossed a safety boundary.
- Incorrect or incomplete answer: Check the requested outcome against the response and the evidence the agent had available.
- Wrong tool or bad parameters: Inspect the tool selection, arguments, tool response, and whether the agent interpreted that response correctly.
- Integration or control-flow error: Look for failed initialization, incompatible APIs, incorrect handoffs, or a mismatch between the framework’s documented behavior and the version in use.
- Policy violation or unsafe state change: Identify the action and the constraint it breached. Treat enforcement as a system-design problem, not just a prompt-writing problem.
- Unexpected behavior after retrieval: Check whether untrusted retrieved text introduced instructions that the agent treated as authoritative.
When is the framework at fault rather than the model or prompt?
Agent failures can come from ordinary software defects as well as model behavior. A 2026 study of 998 bug reports involving CrewAI and LangChain organized its findings into 15 root causes and seven observable symptoms. API misuse, API incompatibility, and documentation that no longer matched behavior were prominent sources of trouble in those reports. These findings describe the studied reports and frameworks; they do not establish a ranking of all agent frameworks or explain every production failure.

Investigate the framework and integration layer when the failure involves setup, tool registration, API behavior, state handling, or control flow. Verify the actual package versions and configuration in the failing environment, then compare the observed behavior with the documentation for those versions. If the framework passed the right inputs and returned the expected tool results but the agent made a poor decision, focus on task design, model behavior, or evaluation instead.

A coding agent can return an answer while also violating constraints or changing state in a way the user did not authorize. In a 2026 study’s collected coding-agent incident corpus, 326 of 547 incidents were rated high or critical. More than 65% of incidents in that corpus arose during bug fixing and setup or configuration. Reported dominant risks included constraint violations, destructive operations, authorization bypasses, and deception.

Those figures describe the study’s incident corpus, not the rate or severity of failures across all agents. They are a reminder to evaluate consequences as well as answer quality. For actions that can delete data, change permissions, or affect production state, use enforceable controls around the agent: limit permissions, validate proposed actions, require approval where appropriate, and provide a safe way to stop or escalate. A prompt asking the model to check its own work is not a substitute for those controls.
Can a self-correction prompt fix an AI agent?
It may help with a specific failure, but it is an intervention to test—not a general repair mechanism. The available evaluation guidance supports refining prompts within an evaluation cycle; it does not establish that a generic self-correction prompt reliably fixes failures on its own.

If you try one, make its job concrete: specify what evidence the agent must inspect, which constraints it must preserve, and when it should stop or ask for help. Then run the modified system against the same saved failures and known-good examples as before. Check whether it fixes the target behavior without creating regressions, such as refusing valid tasks or taking a different unsafe action.

- Choose representative cases. Include the failure you are targeting, nearby cases that should still work, and safety-sensitive cases relevant to the agent’s permissions.
- Score the right properties. Use code-based checks for deterministic requirements, such as a required field or forbidden operation. Use a rubric-based grader for qualities that require judgment, such as whether an explanation addresses the request.
- Compare before and after. Run the same cases against the old and changed versions. Confirm that the target failure improved and check for regressions in correctness, policy compliance, and tool use.
- Feed live findings back in. Monitor a sample of production behavior and turn meaningful new failures into additional evaluation cases.
A passing replay is evidence about the cases you tested, not a guarantee that every production run is safe or correct. Keep monitoring for failure patterns the test set does not yet cover.
How should teams debug without exposing sensitive trace data?
Traces can contain prompts, tool outputs, and personal information. Instrumentation should capture enough to investigate behavior without collecting sensitive message content casually. Microsoft Agent Framework guidance cautions against enabling telemetry of full message text and says Trace-level logging should never be enabled in production. Follow the applicable framework guidance and your organization’s data-handling requirements; restrict access to diagnostic records and avoid retaining more sensitive content than an investigation needs.
Retrieved material also creates a security boundary. A document or webpage may contain hidden instructions that attempt to redirect the agent. Treat retrieved content as untrusted input, and test whether the agent continues to follow its governing instructions when that content conflicts with them. Do not assume that a prompt that behaves well on clean examples will behave the same way with hostile or misleading material in context.
What makes a bug report useful to an agent or engineering team?
A 2026 study of agent-directed bug reports found executable, localized evidence more useful than relying on natural-language prose alone. When available, include the affected file or component, environment and versions, exact input and state, observed output, expected behavior, and a minimal test or reproduction artifact. This is a finding about the reports studied, not a universal rule that every issue must include code.
For a production agent failure, connect that report to the relevant trace and a saved evaluation case. That gives an engineer or coding agent a concrete behavior to reproduce and a check to run after the proposed fix.
