There is no single checklist that applies to everyone. In the United States, an oncology team must match your diagnosis and treatment history to a product’s current FDA indication or to a clinical-trial protocol, then decide whether collection, manufacturing, chemotherapy and infusion are safe and practical for you.
The National Cancer Institute (NCI) pages cited here are U.S.-focused. Product indications and trial criteria can change, so confirm the current FDA labeling and your treatment center’s assessment.

What doctors mean by “eligible”
CAR T-cell therapy is a personalized immunotherapy. T cells are collected from your blood, sent to a laboratory, genetically modified so they can recognize a target on cancer cells, multiplied, and then infused back into you. The treatment is therefore both a medical decision and a manufacturing process; a person must fit the disease requirements and be able to undergo the associated steps.
An oncologist generally considers four linked questions:

- Does the cancer type, target and disease status match an approved CAR T product?
- Does the prior-treatment history meet that product’s requirements?
- Are current health, laboratory findings and cancer circumstances compatible with collection and treatment?
- If an approved use does not fit, is there a suitable clinical trial with its own protocol criteria?
These are clinician assessments, not a self-screening test. The NCI says that side-effect likelihood and severity depend on baseline health, cancer type and extent, treatment type and dose. Your team should explain which factors matter for the exact product being considered.
NCI describes CAR T treatment as established for several blood cancers, while use in solid tumors remains experimental. Read the NCI overview of T-cell transfer therapy for the treatment concept and safety discussion: National Cancer Institute: T-cell transfer therapy.
Cancer type and target are the first filter
Approved CAR T products are directed at particular cancers and cellular targets. A diagnosis that sounds similar to an approved indication may still differ in the required disease subtype, age group, treatment line, response status or biomarker. Your oncologist will review pathology, imaging, blood or marrow findings and any target testing required by the product label.

“Broad category” does not mean every person with that cancer qualifies. The NCI specifically advises checking the current product information and the treating center’s eligibility assessment. See the NCI research feature for the product list and its stated indications: NCI CAR T-cell research overview.
Solid tumors are a different situation
The NCI’s general patient page says CAR T has been studied in cancers such as breast and brain cancer but remains experimental for solid tumors. Someone with a solid tumor may be considered for a clinical trial if a protocol matches the diagnosis and other requirements; that is not the same as an FDA-approved use. Ask specifically whether a trial is available and what its protocol requires.
Previous treatment and response can determine eligibility
CAR T is often considered after other therapies, but the number and type of previous treatments differ by product. “I have had several treatments” is not enough information for an eligibility decision: the team needs drug names or classes, dates, responses, relapses and reasons a treatment stopped.
Example: Aucatzyl for adult B-cell ALL
The NCI states that obecabtagene autoleucel (Aucatzyl) is approved for adults whose B-cell ALL has returned or did not get better during or after other treatment. The drug page was updated March 5, 2025. This example illustrates that both the diagnosis and the disease’s response or relapse status matter; it is not a universal rule for other CAR T products. Example: Abecma for relapsed or refractory multiple myeloma
The NCI states that idecabtagene vicleucel (Abecma) is approved for adults with relapsed or refractory multiple myeloma after at least two prior lines that included an immunomodulating agent, a proteasome inhibitor and an anti-CD38 monoclonal antibody. The page was updated April 24, 2025. “At least two lines” and the required drug classes apply to that stated indication; they should not be generalized to every myeloma product or to another cancer. NCI: idecabtagene vicleucel
What to bring to the treatment-history review
- A complete list of systemic therapies, radiation, surgery and transplants, with dates when possible.
- Whether each treatment produced a response, stable disease, progression or relapse.
- Pathology and molecular reports, including any target or biomarker testing.
- Recent scans, blood counts, marrow results and summaries from other cancer centers.
- Medication, allergy and infection histories.
The center will translate that record into the exact wording of the product label or trial protocol. Do not assume that a treatment class qualifies unless the team confirms it.
Health and current disease circumstances also matter
Even when the diagnosis and prior therapy fit, the team must decide whether treatment can be delivered safely. The sources reviewed do not establish one universal age limit, organ-function cutoff, disease-burden threshold or performance-status rule for all CAR T products. Requirements can differ by product, center and trial.

- Current cancer status: disease burden, speed of progression and whether the cancer needs control while cells are being manufactured.
- Organ and marrow function: blood counts and the condition of organs needed to tolerate the planned treatment, as determined by the center’s protocol.
- Infections and immune status: active or recent infections and other conditions that could alter timing or risk.
- Other medicines and treatments: drugs or radiation that may need to be held, changed or coordinated.
- Ability to complete monitoring: travel, caregiver support, communication and access to urgent medical evaluation as required by the center.
Only the treating team can weigh these factors together. A person can be medically fit in general and still not meet a particular indication, or meet the indication but need another plan because of immediate safety concerns.
How the treatment process affects the decision
Eligibility includes whether the treatment can be organized in time and whether you can safely pass through each stage.
- Referral and confirmation: The center verifies the diagnosis, target and prior-treatment history against the product label or trial protocol.
- Cell collection: T cells are collected from your blood by a process called leukapheresis. The center explains preparation, vascular access and any medication instructions.
- Laboratory engineering and growth: The cells are modified to recognize the cancer target and expanded. Manufacturing is individualized; the center must explain its expected schedule.
- Bridging or disease-control treatment: Because manufacturing takes time, your oncologist may discuss treatment to control the cancer while the cells are prepared. Whether this is needed depends on your disease and center.
- Lymphodepleting chemotherapy: Many CAR T protocols use chemotherapy before infusion to prepare the immune system. The exact drugs, timing and suitability are product-specific.
- Infusion and monitoring: The modified cells are infused, followed by close observation and scheduled follow-up. The center should give you written instructions and an urgent contact route.
How long manufacturing may take
NCI pages give different general estimates: one overview describes approximately 3 to 5 weeks from collection to infusion, while the broader T-cell transfer page says cell growth can take 2 to 8 weeks. These are estimates from different NCI pages, not a guaranteed schedule. Ask the actual center:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- What is the expected collection-to-infusion interval for this product?
- What could delay manufacturing or release?
- What treatment, if any, will control the cancer during the wait?
- Where will you stay, and how quickly must you be able to reach the center after infusion?
Serious effects require a monitoring and contact plan
CAR T cells can activate a powerful immune response. The NCI highlights cytokine release syndrome (CRS), which can include fever, low blood pressure and breathing problems. It also highlights immune effector cell-associated neurotoxicity syndrome (ICANS), a neurologic toxicity that can involve confusion, excessive sleepiness or impaired speech. These effects can be serious and may require urgent treatment.

Before treatment, ask the center to state plainly:
- Which symptoms require an immediate call and which require emergency services.
- Whether monitoring occurs in the hospital, an outpatient unit or both, and for how long.
- Who answers after hours and which facility should evaluate you.
- Whether a caregiver must remain with you and for what period.
- How follow-up visits, blood tests and neurologic checks will be arranged.
Do not wait for a scheduled appointment if the team tells you to report fever, breathing difficulty, faintness, new confusion, unusual sleepiness, trouble speaking or another urgent symptom.
How to compare CAR T, standard treatment and a clinical trial
If you do not fit an approved indication, a trial may still be possible; if you do fit, alternatives may remain appropriate. Compare each option using the same questions rather than assuming that CAR T is automatically the best or fastest choice.
| Decision area | What to ask |
|---|---|
| Fit | Does the option match my exact diagnosis, target, disease status and treatment history? |
| Expected benefit | What outcomes have been observed in people most like me, and how uncertain is that estimate? |
| Risks | Which short- and long-term toxicities are possible, and how are they treated? |
| Timing | How soon can treatment start, and what happens if the cancer changes while I wait? |
| Logistics | Where are collection, infusion and monitoring performed; what travel and caregiver commitments apply? |
| Alternatives | What approved treatments, supportive care options or other trials should be considered now? |
The cited NCI material does not support a patient-specific prediction of benefit. Your oncologist must interpret trial results in light of your cancer, prior treatments, health and goals.

Eligibility and testing
- Does my precise diagnosis and its target match an FDA-approved CAR T product?
- If not, is there a relevant clinical trial, and what are that protocol’s inclusion and exclusion criteria?
- Which prior treatments and response history does the indication require, and does my record meet them?
- What pathology, target or biomarker tests must be confirmed or repeated?
- Which aspects of my current health, cancer status or other medicines could affect eligibility or safety?
Benefits, uncertainty and alternatives
- What benefit is realistic for someone with my disease and treatment history?
- What evidence applies to my exact product and situation, and what remains uncertain?
- What standard treatments, supportive approaches or trials should we compare?
- If we wait for manufacturing, what is the plan if my cancer progresses?
Process and practical requirements
- What is this center’s actual collection-to-infusion timeline?
- Where will collection, lymphodepleting treatment, infusion and follow-up occur?
- Will I need a caregiver, nearby lodging or restrictions on driving and travel?
- What current FDA label and patient information should I read for the exact product?
Safety and urgent contact
- What CRS and ICANS symptoms should I watch for, and when should I call?
- Who do I contact urgently during the day, overnight and on weekends?
- Where will I be monitored, and how long will observation continue?
- What follow-up tests and longer-term monitoring are required?
How to interpret reported outcomes
Results from trials describe groups, not guaranteed individual outcomes. In its 2025 research feature, the NCI reports that a trial involving people with advanced follicular lymphoma found axi-cel eliminated the cancer in nearly 80% of participants; a later update noted that many remained without recurrence at three years. The same feature reports that, in clinical trials involving people with large-cell lymphoma, more than 30% of participants were alive without evidence of cancer five years after treatment. Those figures are product- and study-specific and should not be applied to every CAR T product, cancer or patient.
Discuss how the study population compares with you, what endpoint was measured, the follow-up duration and how treatment-related risks affect the choice. The NCI article quotes immunotherapy pioneer Steven Rosenberg, M.D., Ph.D.: “ [CAR T cells] have become a part of modern medicine.” The brackets and wording are reproduced from the NCI article, updated February 26, 2025.
Key takeaway
You may be eligible when your exact cancer and target fit a current product indication or trial, your previous treatments satisfy its requirements, and your health and circumstances allow safe collection, manufacturing, chemotherapy, infusion and monitoring. Because no universal checklist exists, bring your complete treatment record and use the questions above to obtain a product-specific decision from an oncology team.
