Premium adds Google Health Coach features such as personalized health answers, adaptive fitness plans, and sleep summaries with coaching insights. Google has not said that the Watch 5’s new Health Guardian monthly trend summaries require Premium.
What you can track without Google Health Premium
Google’s US comparison page places core tracking in the included Base tier when Google Health is paired with a watch or tracker. That covers collecting and viewing measurements—not the tailored coaching layered on top.

- Activity: steps, calories, distance, cardio load, and readiness.
- Sleep: sleep score, schedule, duration, and sleep stages.
- Health metrics: heart rate, heart-rate variability (HRV), breathing rate, and blood oxygen (SpO2), among other metrics.
- Health and wellness logs: weight, nutrition, water intake, moods, and cycles.
Google’s Pixel Watch help page also describes tracking steps, calories, and GPS data during activities such as running, weightlifting, and tennis; automatically detecting and saving some activities; setting sleep goals; tracking sleep stages; and monitoring heart rate. It does not identify Premium as a requirement for these basic tracking functions. Google’s activity compatibility table includes Pixel Watch 5 for the listed activity metrics. What requires Google Health Premium
Google describes Premium as the Base benefits plus Google Health Coach. The paid layer is personalized interpretation and guidance, rather than access to the basic measurements above.

Google Pixel Watch Help characterizes Premium as providing “advanced insights and recommendations from the Google Health Coach” to help build habits around fitness, sleep, nutrition, and mindfulness. How to interpret Pixel Watch 5’s new features
Strength workouts and sleep tools
Google’s Pixel Watch 5 announcement describes customized strength workouts, adaptive recommendations that use recovery metrics such as sleep and HRV, and sleep-related features. Its footnotes say some features need Google Health or Google Health Premium, but a footnote should not be treated as proof that a particular Watch 5 function is paywalled unless the note or a current feature page names that function. Google also claims the Watch 5 has 15% more accurate sleep-stage detection than Pixel Watch 4; that is Google’s 2026 comparison, not an independent test result. Health Guardian monthly summaries
Google announced monthly Pixel Watch 5 Health Guardian trend summaries for blood pressure, sleep breathing quality, and insulin resistance. The announcement described the feature as “Rolling out soon” and said the first detailed summary would arrive after one month of wear. It does not state that these summaries require Premium. Availability and requirements may change as the feature rolls out, so check the current Google feature information for your location rather than assuming it is either included or paywalled. Google says the Fitbit app is transitioning to Google Health, so many people may still refer to the paid plan as Fitbit Premium. The current name used in Google’s Watch help and membership materials is Google Health Premium. Google Health transition announcement · Google Pixel Watch help

Google’s help content describes a three-month Premium trial for eligible new users, not an offer guaranteed to every Pixel Watch 5 buyer. Check the offer shown in your account. Google also says Health Coach and Premium coaching features are limited to users over 18 who have a Fitbit or Pixel Watch device, and availability is limited by country. Language, rollout, and plan details can vary by location; the Google Store comparison linked above is US-facing.

Decide based on whether you want tracking or coaching
- If you mainly want activity records, sleep data, and health metrics, Google’s listed Base tier covers those core tracking functions.
- If you want personalized answers, adaptive workout plans, or coaching-oriented sleep summaries, those are Premium additions.
- If a particular new Watch 5 feature is decisive, verify its current availability and subscription requirement in your region; the announcement does not settle every feature’s access terms.
