Karlach’s Act One romance scene is tied to the tiefling celebration; her Act Two relationship scene depends on continuing the romance and completing both infernal-engine upgrades. Astarion’s romance can begin through an Act One proposal or a celebration scene. In Act Two, meeting Araj Oblodra at Moonrise Towers and respecting Astarion’s refusal can lead to a follow-up scene; tell him you care about him to move into a partnership.
Karlach: unlock her romance scenes
Act One: start the romance at the tiefling celebration
Flirting with Karlach builds approval but does not, by itself, start the romance. After her first engine upgrade, answering “Of course I do” when she asks if you want to touch her grants approval; after the Paladins of Tyr encounter, telling her she looks “Hot” is another approval-raising response. These are examples rather than mandatory dialogue choices. The approval reference lists a 20-approval threshold for the celebration route.

At the tiefling celebration, tell Karlach you like her and accept her invitation to spend time together. If you want her scene to be your first celebration romance scene, do not initiate another romance scene that night. A second-choice route may be available if you have no more than one other active romance. An existing partnership, too little approval, or too many active romances can make her decline.

- Define the future of the Forgotten Realms - through your choices, and the roll of the dice. No matter who you play, or what you roll, the world and its inhabitants will react to your story
- Player-initiated turn-based mode - allows you to pause the world around you at any time even outside of combat. Whether you see an opportunity for a tactical advantage before combat begins, want to pull off a heist with pin-point precision, or need to escape a fiendish trap. Split your party, prepare ambushes, sneak in the darkness -- create your own luck!
Act Two: complete both engine upgrades
Continue the romance and complete both of Karlach’s infernal-engine upgrades. The evening cutscene after the second upgrade progresses the relationship. The approval reference also says that touching or hugging her to show the upgrade worked can start the romance at this point if it has not already begun. The romance reference describes the scene and its progression.
There is a qualification around an earlier night-visit scene: the romance reference says it is bugged and does not trigger without mods. Because that is a live community reference and triggers can change with patches, treat the celebration as the practical initiation route rather than assuming the earlier scene will occur.

Astarion: start and advance his romance
Act One: accept his proposal or choose him at the celebration
One route is to reach at least 40 approval before the Save the Refugees quest resolves. A subsequent conversation can prompt Astarion to propose spending the night together. Agree, then take a long rest for the scene. The bite-night scene is not required for the later romance. The romance reference describes the proposal route.

Agreeing to his proposal triggers the scene. If you already slept together before the party, agreeing again does not produce another romance scene.

Act Two: follow up at Moonrise Towers
Bring Astarion when you speak with Araj Oblodra at Moonrise Towers. Your responses can progress or end the romance: respect his refusal and respond supportively to reach the subsequent long-rest conversation. Trying to manipulate him into sex or making him feel used can cause him to end the relationship.
There is an alternate route to a similar follow-up: complete “Kill Raphael’s Old Enemy,” visit Moonrise Towers, do not speak with Araj, and have at least 70 approval with Astarion. The approval reference bases its listed approval changes on official dialogue files as of Patch 7. See Astarion’s approval reference.

Choose partnership in the follow-up scene
In the second romantic cutscene, tell Astarion that you care about him to advance to the partnered stage. Choosing the friend response ends the romance without approval loss, according to the approval reference. Astarion’s approval page lists the dialogue outcomes.

How the extra scenes fit into the game’s patches
Larian Studios’ Patch 2 announcement says the update added a new Karlach epilogue scene and additional Karlach moments in Acts One and Two. Larian’s Patch 4 notes show that romance triggers have been adjusted in patches, but the cited romance-trigger change concerns Lae’zel, not Karlach or Astarion. The specific thresholds and branches above come from live BG3 Wiki romance and approval references; check those pages against your current game build if a trigger does not occur.
