Which Cancer Symptoms in Younger Adults Should Prompt a Doctor’s Visit?

A younger adult should arrange a medical assessment for a new, unusual, unexplained, persistent, or worsening change—especially a lump or swelling, unusual bleeding or bruising, blood in stool or urine, a changing mole, a sore that does not heal, lasting bowel or urinary changes, a cough or hoarseness that will not go away, or unexplained weight loss, fever, night sweats, or severe fatigue. These signs usually have causes other than cancer, but they deserve an explanation.

Do not wait for pain. The National Cancer Institute (NCI) notes that cancer often does not cause pain and advises seeing a doctor when symptoms have not improved after a few weeks. A severe or rapidly worsening problem warrants local urgent or emergency care rather than waiting for a routine appointment.

Who counts as a younger adult?

The American Cancer Society (ACS) uses “adolescents and young adults” (AYA) for people ages 15–39. Cancer is less common in this age range than in older adults, and many symptoms overlap with infections, injuries, medication effects, hormonal changes, digestive disorders, stress, and other ordinary conditions. That overlap is why a symptom is a reason to seek an evaluation, not proof of cancer.

ACS estimates that about 88,120 adolescents and young adults in the United States will be diagnosed with cancer in 2026. Its 2026 overview says people ages 15–39 account for around 4% of new cancer diagnoses each year. Those are population estimates, not an individual’s odds, and they should not be used to dismiss or diagnose a symptom.

Symptoms and screening are different. Screening looks for certain cancers before symptoms appear, while a symptom-led visit investigates a change you have noticed. There is no screening test for every cancer; recommendations depend on age, anatomy, personal history, family cancer syndromes, geography, and current guidelines.

When a symptom should trigger an appointment

Use the persistence-and-change rule

Contact a clinician when a change is:

  • New or unusual for you.
  • Unexplained by an injury, infection, medication, or known condition.
  • Persistent or repeatedly returning.
  • Worsening, spreading, or accompanied by other changes.

NCI’s general guidance is to seek care if symptoms do not get better after a few weeks. That timeframe is a prompt for an ordinary appointment, not permission to wait when a problem is severe, rapidly worsening, or frightening. You also do not need to wait several weeks if the change is clearly abnormal or you are at increased risk because of a previous cancer, a strong family history, or an inherited cancer syndrome.

Arrange prompt care for these warning signs

  • A new lump or swelling: This may be in the neck, armpit, groin, breast or chest, abdomen, or elsewhere. A lump that enlarges, feels fixed or hard, persists, or is associated with night sweats, fever, weight loss, or ongoing pain should be examined.
  • Unusual bleeding or bruising: Examples include bruises without a clear injury, frequent nosebleeds, bleeding gums, unusually heavy or irregular menstrual bleeding, or bleeding after sex. Report bleeding that is new, recurrent, or difficult to control.
  • Blood in stool or urine: Red, maroon, or black stool and pink, red, or cola-colored urine can have many causes, including hemorrhoids, infections, stones, or medications, but visible blood needs medical advice rather than self-diagnosis.
  • Persistent bowel or bladder changes: A lasting change in bowel frequency or appearance, ongoing diarrhea or constipation, a feeling of incomplete emptying, persistent abdominal fullness, pain with bowel movements, urinary frequency, urgency, difficulty starting, or a continuing change in urine flow should be assessed.
  • A changing skin spot or a sore that will not heal: Have a clinician examine a new mole or spot, or one that changes in size, shape, color, border, or symptoms such as bleeding or itching. A sore, scab, or ulcer that repeatedly returns or does not heal also deserves an examination.
  • A cough or hoarseness that does not go away: Ongoing cough, voice change, coughing up blood, chest discomfort, or shortness of breath should not be attributed indefinitely to a cold, reflux, smoking, or allergies without review.
  • Unexplained weight or appetite change: Losing weight without trying, a sustained loss of appetite, or unexplained abdominal fullness is a reason to call a clinician, particularly when combined with fatigue, fever, bleeding, or bowel changes.
  • Persistent fever, drenching night sweats, or severe fatigue: Recurrent fevers without a clear infection, soaking night sweats, marked weakness, pale skin, or fatigue that interferes with normal activities should be evaluated.
  • Neurologic or vision changes: New or progressively worsening headaches, repeated nausea or vomiting, seizures, weakness, balance or speech changes, confusion, or new vision changes require medical assessment.

The NCI lists many of these symptoms while emphasizing that they can result from non-cancer conditions. Its advice is to have persistent problems diagnosed and treated early, not to assume the worst.

Patterns ACS describes in adolescents and young adults

ACS groups possible signs by cancer type. They are examples of patterns clinicians consider, not a checklist for diagnosing yourself. A symptom that is not listed can still merit care if it does not improve or go away. ACS’s AYA guidance is available at Finding and Diagnosing Adolescent and Young Adult (AYA) Cancer.

Brain tumors

Possible patterns include headaches, vision changes, nausea or vomiting, changes in bowel habits, unexplained weight loss, low energy, or weakness. Headaches are common and usually have non-cancer causes; a new pattern, steadily worsening pain, neurologic deficits, or repeated vomiting needs prompt assessment.

Leukemia or lymphoma

ACS notes low appetite or weight loss, easy bleeding or bruising, frequent infections, fevers, heavy night sweats, low energy, pale skin, shortness of breath, abdominal swelling, bone pain, or enlarged lymph nodes—especially in the neck, chest, armpits, or groin. These findings can also occur with infections, anemia, immune disorders, or other conditions, so testing is needed to identify the cause.

Melanoma

A new skin spot or a spot changing in size, shape, or color should be checked. Take note of when you first noticed it and how it has changed; do not wait for pain, because concerning skin changes may not hurt.

Ovarian cancer

Persistent abdominal pain or fullness, or continuing changes in bowel or bladder habits, can be among the symptoms considered. Digestive, urinary, menstrual, and gynecologic conditions are much more common, but symptoms that do not settle should be discussed with a clinician.

How long should you wait?

For a mild symptom that is improving and has an obvious short-term explanation, monitor it briefly while following appropriate self-care. If it remains, returns repeatedly, or worsens over a few weeks, book an appointment. NCI states: “If you have symptoms that do not get better after a few weeks, see your doctor so that problems can be diagnosed and treated as early as possible.” Read the guidance at NCI’s Symptoms of Cancer.

Do not use the few-weeks advice to postpone care for severe pain, heavy bleeding, fainting, confusion, new weakness, trouble breathing, coughing up blood, inability to keep fluids down, a rapidly enlarging swelling, or any symptom that feels immediately dangerous. Contact local urgent or emergency services for those situations. The available guidance does not establish one cancer-specific emergency threshold for every symptom.

What happens at the doctor’s visit?

Start with a primary-care clinician, gynecologist, dermatologist, or other appropriate professional; urgent care can help when a routine appointment is unavailable and the problem cannot wait. Tell the office the main symptom and how quickly you need to be seen.

ACS says a healthcare team may:

  1. Ask when the symptom began, whether it is changing, what makes it better or worse, and whether you have other symptoms.
  2. Review your medical, medication, menstrual, vaccination, and family histories, including previous cancer or inherited cancer syndromes when relevant.
  3. Perform a physical examination, such as checking a lump, lymph nodes, skin, abdomen, neurologic function, or vital signs.
  4. Order tests based on the findings. These may include blood or urine tests, imaging, or a procedure to examine tissue or another body area.
  5. Refer you to a specialist if a finding needs further assessment. The specialist depends on your age, symptoms, and suspected cancer type.

Not everyone needs every test, and an initial examination often identifies a non-cancer explanation. A referral or scan is an investigation, not a diagnosis.

Prepare useful information

  • Write the start date, frequency, progression, and severity of each symptom.
  • List medicines, supplements, allergies, recent infections, injuries, travel, and relevant exposures.
  • Record unintentional weight change, fevers, night sweats, menstrual or reproductive changes, bowel or urinary changes, and family cancer history.
  • For a skin change, keep dated photographs in consistent lighting, but do not delay an examination while tracking it.
  • Ask what result would require follow-up and when you should contact the office again.

Why symptoms do not identify cancer by themselves

Fatigue can follow poor sleep or anemia; swollen nodes often follow infection; blood in stool may come from hemorrhoids; cough can follow a virus or reflux; and changing bowel habits can reflect diet, medication, or inflammatory disease. The same symptom can have different meanings at different ages and with different examination findings. ACS’s general information at Signs and Symptoms of Cancer emphasizes that symptoms are nonspecific.

Conversely, feeling well does not rule out cancer. Some cancers cause few or no early symptoms, which is why age- and risk-appropriate screening remains separate from symptom-based care. Do not substitute a home test, device, supplement, or symptom-tracking product for a clinician’s evaluation; the reviewed sources do not establish any consumer product as a diagnostic aid.

What is known about colorectal warning signs in younger adults?

An NCI report dated June 15, 2023, summarized a study of insurance claims involving more than 5,000 people diagnosed with colorectal cancer before age 50 and more than 22,000 controls. The report discusses potential warning signs but does not make any single sign diagnostic or provide a safe self-triage rule. Read it at NCI’s “Study Identifies Potential Warning Signs of Colorectal Cancer in Younger Adults.” The practical response remains the same: persistent bowel changes or blood in stool should be discussed with a healthcare professional.

Screening is a separate conversation

People without symptoms may still qualify for screening, while people with symptoms need diagnostic evaluation even if they are younger than a routine screening age. ACS notes that screening can find some cancers before symptoms but that no screening test covers every cancer. Cervical screening applies to people with a cervix according to current local guidance. Earlier or additional screening may be recommended for someone with a prior cancer history or a known family cancer syndrome. Ask a clinician which recommendations fit your age, anatomy, personal history, and family history.

Questions to ask yourself before calling

  • What exactly changed, and what is my baseline?
  • Has it persisted, worsened, or returned?
  • Is there bleeding, a lump, weight loss, fever, night sweats, severe fatigue, or a neurologic change alongside it?
  • Could an injury, infection, medication, pregnancy, menstrual change, or chronic condition explain it—and has that explanation actually resolved the symptom?
  • Do I have a previous cancer, strong family history, or inherited syndrome that changes my risk?

If the change is new, unexplained, persistent, or worsening, the safest next step is to arrange an assessment rather than trying to determine from a symptom list whether it is cancer. As the ACS puts it, “if there’s a sign or symptom that does not get better or go away, even one that is not on these lists, see a doctor to get checked.”
