Which Apple Watch Health and Safety Settings Should You Turn On?

By Freedom251 Team · · 8 min read

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start with the settings that can help another person reach you or emergency services: complete Medical ID and emergency contacts, learn Emergency SOS, and enable Fall Detection on a compatible watch. Leave Crash Detection enabled on supported models; it is on by default. Add Check In if you want a trusted contact to know when you arrive, finish a workout, or miss a scheduled check-in. Then select health alerts and tracking features that match your needs rather than turning on every notification.

These tools are safeguards, not guarantees. Apple says Apple Watch cannot detect every fall or car crash. Emergency calling normally requires cellular service or Wi-Fi calling, while satellite communication is restricted to supported devices and regions. Your watch model, location, and watchOS version determine which options appear.

Set up the emergency essentials first

These settings have the greatest value when you are unable to explain your condition or contact someone yourself. Configure them on the iPhone paired with your watch, then test that you know how to reach them from the watch.

Medical ID and emergency contacts

  1. On your paired iPhone, open the Health app and tap your profile picture, then Medical ID.
  2. Enter allergies, medical conditions, medications, blood type, organ-donor status, and other information you want responders to see. Add one or more emergency contacts.
  3. Turn on Show When Locked so someone can view the Medical ID from the iPhone lock screen. Consider enabling Share During Emergency Call where the option is offered.
  4. Save the record and review it whenever a medication, diagnosis, phone number, or relationship changes.

On Apple Watch, hold the side button until the sliders appear and slide Medical ID. You can also open the watch’s Settings app and choose SOS > Medical ID. Apple says that, in the United States and Canada, shared Medical ID information may be sent to emergency services when you call or text 911 or use Emergency SOS. That sharing is not a worldwide guarantee, so keep the on-watch record accurate even if you travel.

Emergency SOS

Press and hold the watch’s side button to display the emergency sliders and start an emergency call. Siri or Messages can also be used to contact emergency services, depending on the configuration and connectivity available. After you add emergency contacts, the watch can alert those people after an emergency call; they are separate from the emergency-services operator, so include contacts who can respond to your location and situation.

Practice the side-button gesture while you are calm. A call generally needs cellular service or Wi-Fi calling. If the watch cannot reach a network, it may not be able to place the call. Satellite emergency communication is a limited feature on supported models and in supported regions, not a universal substitute for a connection.

Turn on Fall Detection if a missed fall would be dangerous

Fall Detection looks for a hard-impact fall and can ask whether you are all right. If it detects a hard fall and you remain unresponsive and immobile, Apple documents an automatic-contact sequence: an alert appears first; after about a minute without movement, a 30-second countdown runs before the watch attempts to call emergency services.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How to enable it

  1. On Apple Watch, open Settings > SOS > Fall Detection.
  2. Choose Always On or Only During Workouts.
  3. On the paired iPhone, use Watch app > My Watch > Emergency SOS > Fall Detection to reach the same control.

Always On is the stronger choice for people at risk of falls at home, outdoors, or during ordinary activities. Only During Workouts limits detection to recorded workouts and may suit someone who does not want alerts during day-to-day activity. High-impact movement can trigger a false alert, so respond promptly if the watch asks whether you are safe.

Automatic calling after a hard-impact fall depends on Wrist Detection being enabled. Turning Wrist Detection off prevents that automatic emergency call. Apple also cautions that Fall Detection cannot detect every fall, particularly when the impact or movement pattern does not resemble the events its sensors recognize. Wear the watch snugly enough for its sensors to maintain contact, and keep it charged if you rely on overnight or all-day coverage.

Keep Crash Detection enabled on supported models

Crash Detection is designed to identify a severe car crash and offer a route to emergency services. Apple documents it as on by default on supported Apple Watch models. The setting is still worth checking after a reset, a device transfer, or a change in family settings.

Where to check the control

  1. On the watch, open Settings > SOS > Crash Detection.
  2. Verify Call After Severe Crash is enabled.

After a detected severe crash, the watch sounds an alert and asks for a response. If you do not cancel the process, Apple documents a 10-second interval followed by a 30-second countdown before an automatic call. Cancel the call if you are safe and the detection was incorrect; answer the prompts if you need help.

Crash Detection is not a guarantee that every collision will be recognized. It normally needs cellular service or Wi-Fi calling to contact emergency services. If you drive or ride in areas with weak coverage, understand that the watch may be unable to complete a call even when detection works. Satellite communication, where offered, depends on a supported watch and region.

Add Check In for planned arrivals and activities

Check In is useful when the main risk is that someone will not know you arrived or finished safely. You can configure a check-in to notify a friend when your Apple Watch reaches a destination, when a workout ends, or when a chosen period expires. If the check-in is not completed, you choose what information is shared with the contact.

Good uses for Check In

  • Send a family member an arrival confirmation after a late commute.
  • Let a friend know when a solo run or hike is finished.
  • Set a time-based check-in before a situation where you may not be able to answer messages.

Tell the contact in advance what the check-in means and which details you are comfortable sharing. Check In supplements, rather than replaces, Emergency SOS: it is a communication plan for a trusted person, not an automatic emergency-service call.

Other watch safety features

Apple lists Medical ID, emergency calling, Fall Detection, Crash Detection, Check In, Siren, and satellite communication among Apple Watch safety features. Apple Watch Ultra models include a Siren for attracting attention nearby. Satellite communication is limited by model and region; Apple’s current guide states that Apple Watch Ultra 2 and earlier do not support it. Check the options shown on your specific watch instead of assuming that a feature listed for one model is available on another.

Choose health notifications that answer a real question

Health features are optional monitoring and notification tools. The available documentation does not establish them as diagnostic instruments, so use alerts as prompts to pay attention or seek professional advice, not as a medical verdict.

Heart notifications

  • High- and low-heart-rate alerts: notify you when the watch observes a rate above or below thresholds you set or accept. They can be useful if you have been advised to watch your resting rate or if unusual symptoms occur.
  • Irregular-rhythm notifications: may identify patterns suggestive of atrial fibrillation (AFib). Turn them on if the feature is available in your region and model, and discuss an unexpected notification with a clinician.
  • AFib History: is intended for people already diagnosed with AFib and requires the relevant Health setup. It is not a general screening switch for everyone.
  • Low Cardio Fitness: can flag a lower estimated cardio-fitness level. Treat the trend as a reason to review activity and health with a professional when appropriate.

Availability and regulatory approval vary by country, model, and software version. If a setting is missing, confirm those requirements before assuming the watch is malfunctioning.

Sleep

The Sleep app supports schedules, duration goals, and sleep history. Turn on a schedule if you want consistent reminders and overnight measurements. A schedule is most useful when you wear the watch to bed and keep it charged before sleep; without overnight wear, the watch cannot record the same sleep information.

Medications

The Medications app records medicines, vitamins, and supplements that you add through Health on the iPhone. Enable reminders only for items you genuinely want to log. The log is a record-keeping aid; it does not administer medication or replace instructions from a prescriber.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Cycle Tracking

Cycle Tracking records menstrual-cycle information and can use heart-rate and wrist-temperature data to produce estimates. Add the information consistently and treat predictions as estimates. Feature availability and the data used can differ by model and region.

Vitals and overnight measurements

Vitals can display overnight heart rate, respiratory rate, wrist temperature, blood oxygen, and sleep duration when supported. It is most useful for spotting changes from your usual range, which means regular overnight wear and the required Health permissions matter. Apple limits some measurements by model and region, so the Vitals screen may not contain every metric on every watch.

Use this decision guide before enabling every alert

Setting Primary purpose Default or choice What it needs
Medical ID Give responders and contacts essential information You enter and maintain it Health app setup; emergency sharing rules vary by country
Emergency SOS Call emergency services and alert contacts Available from the side button Usually cellular service or Wi-Fi calling
Fall Detection Respond to a detected hard fall Choose Always On or Only During Workouts Compatible model, Wrist Detection for automatic calling, connectivity for a call
Crash Detection Respond to a detected severe car crash On by default on supported models; verify Call After Severe Crash Compatible model and normally cellular service or Wi-Fi calling
Check In Confirm arrival, workout completion, or a time-based status You start each check-in A chosen contact and the information-sharing choices you select
Heart, sleep, medication, cycle, and Vitals features Track measurements or receive health prompts Optional and configurable Model, region, Health setup, and in some cases overnight wear

Check compatibility, connectivity, and privacy

Before troubleshooting, identify the exact Apple Watch model, iPhone pairing, watchOS version, and country or region. Safety and health features can be restricted by hardware, local regulations, or software. A setting that appears in Apple’s general safety guide may not appear on your device.

  • Connectivity: cellular service or Wi-Fi calling is the normal route for emergency calls. A GPS-only watch cannot create cellular service by itself.
  • Location and sharing: Medical ID sharing, Check In details, and health measurements involve different privacy choices. Review what a contact or emergency service can receive before relying on the feature.
  • Wrist fit and charging: loose wear can reduce sensor readings, while a flat battery removes all watch-based detection until it is charged.
  • False positives: vigorous, high-impact activity can resemble a fall or crash. Keep the watch accessible so you can cancel an accidental alert.

Review the setup after major changes

Recheck these settings after buying a new watch, restoring an iPhone, changing a phone number, moving country, updating watchOS, or changing a medical treatment. Confirm that emergency contacts still answer, Medical ID reflects current information, Fall Detection and Crash Detection show the intended state, and Check In contacts understand the plan. If an alert or call fails, first verify battery level, Wrist Detection, cellular or Wi-Fi calling, permissions, and regional availability. Do not assume that a missing notification means the watch detected nothing, or that a detected event guarantees a completed emergency call.
