Seek professional help when anxiety or low mood is hard to cope with, lasts or worsens, or begins interfering with ordinary life. You do not need a diagnosis, a particular number of symptoms, or a complete inability to function before contacting a clinician. Severe symptoms lasting about two weeks are a useful National Institute of Mental Health (NIMH) signal, but severe, rapidly escalating, or unsafe symptoms warrant help sooner. If there is immediate danger or a risk of self-harm, use emergency or crisis services now.
Get urgent help if safety is in question
Thoughts of suicide, urges to hurt yourself or someone else, an inability to stay safe, or a mental-health crisis that feels life-threatening should not be managed by waiting for a routine appointment. Tell someone nearby, move away from anything you could use to cause harm, and contact the appropriate emergency service.

When routine professional help is the right next step
Routine help can come from a primary-care clinician, general practitioner (GP), psychologist, psychiatrist, therapist, or another qualified mental-health professional. Consider making an appointment when any of the following is true.
Your daily life is being disrupted
Anxiety or depression deserves attention when it affects work, school, relationships, parenting, sleep, eating, personal care, finances, or other responsibilities. NIMH specifically identifies problems at school, work, or with friends and family as a reason to seek help for anxiety. You do not have to be unable to leave home or completely off work for the effect to count. Repeatedly cancelling plans, missing deadlines, avoiding places, or needing substantial effort to complete basic tasks are meaningful changes.

NIMH uses two weeks as a practical signal for severe symptoms and advises contacting a provider if milder symptoms do not improve or get worse despite self-care. This is guidance, not a diagnostic test. A fixed number of days should never be used to justify waiting through severe, escalating, or dangerous symptoms. Recurrent episodes also merit an assessment even when each episode eventually passes.

Self-help is not helping
For mild symptoms lasting less than two weeks, NIMH lists measures such as regular movement, adequate sleep, healthy eating, social connection, meditation or breathing exercises, and low-stress activities. If these steps do not help, or if symptoms intensify, move from self-care to professional advice rather than continually adding more techniques.
You are worried and want an assessment
You do not need to prove that symptoms are “bad enough.” NHS guidance says not to self-diagnose anxiety and recommends speaking with a GP if you are worried. A clinician can consider anxiety, depression, physical health, medication effects, substance use, sleep, and other possible explanations.
How to tell when anxiety needs attention
Anxiety becomes a professional-help issue when fear, worry, or panic is difficult to control, causes significant distress, or changes what you do. Examples include avoiding school, work, travel, appointments, social contact, or ordinary errands; repeatedly seeking reassurance; being unable to concentrate; or spending large parts of the day anticipating danger.

Anxiety can involve a racing heart, shaking, sweating, stomach symptoms, dizziness, muscle tension, breathlessness, headaches, or trouble sleeping. Physical symptoms can also arise from other health conditions or medicines. Do not assume they are anxiety solely because you feel worried; arrange a medical assessment for new, recurring, severe, or unexplained symptoms.

Panic and avoidance are not a test you must pass
A single stressful day does not automatically indicate a disorder, and you do not need to experience panic attacks to seek help. If fear is narrowing your life, causing repeated absences, or making ordinary tasks feel unmanageable, that impact is enough reason to talk with a professional.
How to tell when depression or low mood needs attention
NIMH describes severe symptoms lasting two weeks or more that can include persistent sadness or hopelessness, irritability, low energy, loss of interest, difficulty concentrating or making simple decisions, trouble completing tasks, and changes in appetite, weight, sleep, or sex drive. You do not need every symptom to ask for help.
Look for a meaningful change from your usual self
Depression may show up as withdrawing from people, losing pleasure in activities, moving or speaking more slowly, feeling worthless or excessively guilty, struggling to start routine tasks, or sleeping far more or less than usual. Some people mainly notice irritability, exhaustion, physical complaints, or reduced motivation rather than sadness.

Low mood does not have to be constant
Symptoms can fluctuate during a day or week and still deserve care. NHS guidance for England recommends seeing a GP when low mood has lasted more than two weeks, is hard to cope with, or self-help has not helped. Seek help sooner if symptoms are severe, rapidly worsening, or connected with thoughts that life is not worth living.

“I can still function”—should I talk to someone?
Yes. Continuing to work, attend classes, care for children, or appear composed does not rule out significant anxiety or depression. People often compensate by sleeping less, using all their energy to complete essentials, avoiding enjoyable activities, or hiding symptoms. Tell a clinician what functioning costs you, not only what you manage to finish. Early support may prevent a more serious disruption.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What professional help can involve
A first appointment usually focuses on what you have noticed, how long it has been happening, how it affects your life, safety, physical health, medicines, substances, sleep, and previous episodes. The professional may suggest monitoring, therapy, medication, a physical-health review, or a combination. NIMH identifies therapy and medication as common options, but neither is automatically required.
Therapy
Therapy can help with patterns of worry, avoidance, low mood, coping skills, relationships, and problem-solving. The type, length, format, and frequency depend on the assessment and your preferences. Ask what the proposed approach is intended to treat, how progress will be reviewed, and what to do if it is not helping.
Medication
Medication is a decision to discuss with a qualified prescriber. Ask about expected benefits, common side effects, interactions, how long a trial may take, what monitoring is needed, and how it would be stopped safely. Do not start, stop, or change prescribed medicine without medical advice.
Best Value
Combined or stepped care
Some people benefit from therapy alone, some from medication, and some from both. A clinician may begin with lower-intensity support and increase care if symptoms persist, or recommend specialist assessment when symptoms are severe, complex, or unsafe. Treatment should be adjusted to your symptoms, health history, risks, access, and goals.
Where to start, depending on location
| Situation | United States | England | Other locations |
|---|---|---|---|
| Routine concern | Primary-care clinician or licensed mental-health professional | GP, or NHS Talking Therapies where eligible | Local primary-care or mental-health service |
| How to access therapy | Ask a clinician about referrals, insurance, community clinics, or licensed providers | Eligible people may self-refer to NHS Talking Therapies or ask a GP to refer them | Follow local referral and eligibility rules |
| Urgent but not immediately life-threatening | Contact a clinician, local crisis service, or 988 for guidance | NHS 111 online or call 111 | Use your local urgent mental-health route |
| Immediate danger | 988; call 911 for a life-threatening emergency | Call 999 or go to A&E | Call the local emergency number or go to an emergency department |
NHS Talking Therapies access, eligibility, waiting times, and service arrangements vary by age and local area. If you are unsure whether the service fits your situation, NHS guidance says you can ask a GP. NHS service instructions cited here apply to England, not automatically to Scotland, Wales, Northern Ireland, or other countries.
Prepare for an appointment
A short record can make it easier to explain what is happening. Bring or write down:
- When symptoms began, whether they are constant or episodic, and what has changed recently.
- Effects on sleep, appetite, concentration, work, study, relationships, and self-care.
- Panic episodes, avoidance, physical symptoms, alcohol or drug use, and any medicines or supplements.
- What you have tried, what helped briefly, and what did not help.
- Any thoughts of self-harm, suicide, harming someone else, or inability to stay safe. State these plainly; they change the urgency of care.
- Questions about therapy, medication, confidentiality, costs, waiting times, and follow-up.
You can ask someone you trust to attend, help make the appointment, or write down the plan. If speaking feels difficult, show the clinician your notes or send a message through the service’s approved system.
What to do while waiting for care
Keep routines realistic: regular meals, sleep at roughly consistent times, gentle activity, contact with supportive people, and a smaller list of essential tasks. Breathing or meditation exercises and low-stress activities may help mild symptoms, but they are not substitutes for assessment when symptoms persist, worsen, impair functioning, or create safety concerns. Reduce alcohol and non-prescribed drugs, which can intensify anxiety, worsen sleep, or complicate treatment. Make a safety plan with a trusted person if you have had thoughts of self-harm, and use urgent services if risk increases.
If you are concerned about someone else
Choose a private moment, describe specific changes without diagnosing, and ask directly how they are coping. Listen without arguing or promising secrecy about imminent danger. Encourage a GP, primary-care clinician, therapist, or other qualified professional, and offer practical help such as arranging transport or sitting with them while they call. If they may hurt themselves or cannot stay safe, contact emergency or crisis services rather than leaving them alone to manage the situation.
A simple decision check
- Immediate danger or self-harm thoughts: use crisis or emergency services now.
- Severe symptoms, rapid worsening, or major disruption: contact a professional promptly; do not wait two weeks.
- Symptoms lasting around two weeks or more: arrange an assessment, especially when sadness, hopelessness, loss of interest, anxiety, sleep or appetite changes, or concentration problems persist.
- Milder symptoms under two weeks: try sensible self-care while monitoring closely, and contact a provider if there is no improvement or symptoms worsen.
- Uncertainty: ask a qualified health professional. You do not need to settle on an anxiety or depression label first.
The practical question is not whether you can endure symptoms for a little longer. It is whether they are unsafe, worsening, persistent, difficult to cope with, or taking over parts of your life. Any of those reasons is enough to start a professional conversation.
