A single reading cannot determine whether you need a medication change. Bring a record of properly taken home readings and discuss the options with your healthcare team; do not start, stop, or change prescription medicine on your own.
This guidance reflects U.S. adult recommendations. Pregnancy and some other circumstances call for individualized advice, so ask your own clinician how general adult guidance applies to you.

When is a new treatment worth discussing?
Make an appointment if your readings repeatedly exceed the range or target your clinician has set, or if you have questions about whether your current plan is controlling your blood pressure. A treatment conversation is also appropriate when another health condition, medication, side effect, or practical barrier may affect your care. The decision depends on the pattern of confirmed readings, cardiovascular risk, other health conditions, your response to current treatment, and your preferences—not one number in isolation.

How the 2025 U.S. adult guideline frames blood pressure
The 2025 AHA/ACC adult high blood pressure guideline replaces the 2017 guideline. It retains these familiar categories, measured in millimeters of mercury (mm Hg):
| Category | Systolic and diastolic reading |
|---|---|
| Normal | Below 120 and below 80 |
| Elevated | 120–129 and below 80 |
| Stage 1 hypertension | 130–139 or 80–89 |
| Stage 2 hypertension | At least 140 or at least 90 |
The systolic value is the first, higher number; the diastolic value is the second. When either number falls in a higher category, a healthcare professional interprets the result in context and confirms whether hypertension is present. These categories are not a self-diagnosis or an instruction to change medication.

The updated guideline includes PREVENT risk assessment, recommendations for specific groups, home monitoring, team-based care, and updated medication guidance. For stage 1 hypertension, its framework uses the PREVENT-CVD equations to estimate 10-year total cardiovascular disease risk. The cited AHA summary describes a 7.5% or greater 10-year risk threshold for initiating drug therapy in stage 1 hypertension. Clinicians also consider individual clinical factors, social drivers, and patient preferences—particularly when a risk estimate is near that threshold. The calculation informs a clinician’s decision; it is not a self-prescribing tool. 2025 AHA/ACC guideline summary and AHA summary of key points.

What to bring to the appointment
- Your home readings: Bring the dates, times, and values you have recorded, along with any notes that help explain the circumstances. Ask whether your measurement routine and monitor are suitable for making decisions. The guideline supports home monitoring, but the sources cited here do not establish a particular monitor model’s validation or accuracy.
- A complete medication list: Include prescriptions, over-the-counter (OTC) medicines, vitamins, supplements, and anything else you take. Some OTC medicines or supplements can interact with treatment or raise blood pressure.
- Your experience with the current plan: Note side effects, missed doses or difficulties following the schedule, and whether cost or access has made treatment harder to use.
- Your questions and priorities: Tell the clinician what matters to you, such as avoiding a particular side effect, simplifying dosing, or making treatment affordable.
Questions to ask before choosing an option
If your clinician recommends a new medicine or another change, ask how each option fits your readings, risk, health conditions, and preferences. When there is more than one reasonable choice, compare the points that affect both safety and whether the plan will work in your daily life.

| Ask about | Why it matters |
|---|---|
| Expected blood pressure and cardiovascular benefit | Clarifies what the treatment is intended to improve and how success will be assessed. |
| Side effects and contraindications | Helps you weigh likely downsides against the expected benefit and identify reasons an option may not suit you. |
| Interactions with current medicines or supplements | Lets the care team review the full list rather than considering a prescription in isolation. |
| Dosing and possible combinations | Shows how complex the schedule may be and whether a combination approach is appropriate. |
| Affordability and access | Surfaces cost, coverage, or availability barriers that could affect your ability to follow the plan. |
| Monitoring and follow-up | Establishes what readings to track and when the team will review your response or adjust the plan. |
Useful questions include: “What pattern do my home readings show?” “How do my overall risk and other conditions affect this choice?” “What benefits and side effects should I expect?” “Could any medicine or supplement I take affect the plan?” and “When should we review how it is working?”
If your blood pressure is still high or the medicine is causing problems
Contact your doctor, nurse, pharmacist, or another member of your care team if readings remain high, side effects concern you, or a prescription is difficult to obtain or afford. Finding a helpful dose or combination can take time, and more than one prescription may be used. Explain what is happening and ask what options are available rather than changing the regimen yourself.

The AHA advises raising medication concerns with a healthcare professional and says not to stop a prescribed medicine without talking to one. It also cautions that products promoted as natural remedies may not work as advertised and can affect other medicines or raise blood pressure; do not use a supplement or non-prescription remedy as a substitute for prescribed treatment. As the AHA puts it on its Managing High Blood Pressure Medications page: “The important thing is to talk with them and to follow their treatment plan.”
