Cortisol testing is considered when your symptoms, examination, medicines, or an adrenal finding suggest either Cushing syndrome (too much cortisol) or adrenal insufficiency (too little cortisol). A clinician should select the test and interpret it, because pregnancy, timing, and steroid medicines can substantially change results.
Cortisol testing is not routine fertility screening
Standard infertility evaluation is guided by your history, menstrual and ovulation pattern, examination, and targeted hormone tests. The American Society for Reproductive Medicine’s 2021 fertility-evaluation guidance focuses on tests such as thyroid testing, prolactin, androgen assessment, and ovulatory evaluation when those questions arise; it does not recommend broad cortisol screening for everyone who is having difficulty conceiving.

That distinction matters because cortisol varies across the day and responds to illness, sleep disruption, medications, and pregnancy. A single result cannot diagnose “stress,” explain every irregular cycle, or predict whether you will become pregnant. Testing becomes useful when a clinician is investigating a specific adrenal disorder or a finding that changes pregnancy care.
When fertility concerns may justify asking about cortisol
- You have several progressive features that could indicate Cushing syndrome, such as easy bruising, facial fullness, reddish-purple stretch marks, weakness in the muscles close to your hips or shoulders, unusual osteoporosis, or hypertension at an unusually young age.
- You have symptoms or laboratory findings that could indicate adrenal insufficiency, including unexplained dehydration or low blood pressure, low sodium, high potassium, low blood sugar, hyperpigmentation, severe weakness, abdominal pain, fever, or persistent vomiting.
- An imaging study found an adrenal mass (an adrenal incidentaloma), which may produce hormones even when it was discovered for another reason.
- You use glucocorticoid medicines in any form—tablets, injections, inhalers, creams, eye preparations, or other products—and your clinician needs to determine whether the exposure is affecting cortisol production or causing Cushing-like effects.
- You are pregnant or planning pregnancy and an endocrine disorder is suspected. Coordination between your obstetric or reproductive clinician and an endocrinologist helps avoid a test that is difficult to interpret at your stage of pregnancy.
Difficulty conceiving by itself is not evidence of high cortisol. If your only concern is everyday stress, a consumer cortisol kit or random blood draw is not a validated fertility assessment.

What doctors are looking for: excess versus deficiency
The appropriate test depends first on whether the concern is cortisol excess or cortisol deficiency. These are different disorders with different symptoms, sample types, and follow-up tests.
Possible cortisol excess: Cushing syndrome
Cushing syndrome is prolonged exposure to excess cortisol. The Endocrine Society recommends testing people with multiple progressive features, features that are unusual for their age (for example, early osteoporosis or hypertension), or an adrenal incidentaloma. It recommends against widespread testing in people without a clinical reason because false-positive results can lead to unnecessary scans and procedures.
Before biochemical testing, the guideline says clinicians should take a thorough drug history to identify excessive exogenous glucocorticoid exposure. Steroids supplied by prescription or contained in several products can suppress the body’s own cortisol production or create Cushing-like changes. Do not stop a steroid medicine abruptly; the prescriber must decide whether and how it can be tapered.

Adrenal insufficiency occurs when the body cannot produce enough cortisol. Primary adrenal insufficiency can also affect aldosterone, producing dehydration and electrolyte abnormalities. The Endocrine Society’s guidance identifies otherwise unexplained volume depletion, hypotension, hyponatremia (low sodium), hyperkalemia (high potassium), fever, abdominal pain, hyperpigmentation, and hypoglycemia as reasons to investigate.

Symptoms such as fatigue, nausea, vomiting, dizziness, or weakness are common in pregnancy and many other conditions, so they do not establish adrenal insufficiency on their own. The combination of symptoms, blood pressure, electrolytes, medication history, and examination determines whether testing is warranted.
Which cortisol test is used?
There is no single “best cortisol test” for every situation. The suspected disorder, pregnancy status, trimester, medicines, collection method, and need for confirmation all matter.

Urinary free cortisol in pregnancy
For suspected Cushing syndrome in a pregnant patient, the Endocrine Society recommends UFC and recommends against dexamethasone testing for the initial evaluation. Circulating cortisol concentrations increase approximately two- to three-fold during pregnancy because cortisol-binding globulin rises. Urinary cortisol also normally increases, so the laboratory and clinician must account for trimester and the degree of elevation.
The guideline notes that UFC values above three times the upper reference limit in the second or third trimester are concerning. A value below that level does not automatically exclude disease, and a result near the reference range must be interpreted with symptoms and repeat or confirmatory testing when appropriate. Follow the collection instructions exactly: an incomplete 24-hour collection can make a result appear falsely low.

Cosyntropin stimulation for suspected deficiency
The standard corticotropin stimulation test evaluates whether the adrenal glands can increase cortisol after synthetic ACTH (cosyntropin). The usual protocol measures cortisol at baseline and at 30 or 60 minutes after administration. The 2016 Endocrine Society guideline states that peak cortisol levels below 500 nmol/L (18 micrograms/dL) at 30 or 60 minutes indicate adrenal insufficiency, while emphasizing that the cutoff depends on the assay used by the laboratory.
If stimulation testing cannot be performed, a morning cortisol below 140 nmol/L (5 micrograms/dL), interpreted with ACTH, is suggestive but not a complete diagnosis. A clinician may repeat testing, arrange stimulation testing, or investigate pituitary and medication-related causes. Do not diagnose yourself from a morning result printed on a home report.
How pregnancy changes the decision
Pregnancy changes both normal cortisol physiology and the reliability of certain tests. Rising cortisol-binding globulin increases total circulating cortisol, and normal pregnancy symptoms can overlap with adrenal disease. The test that is reasonable before conception may therefore be inappropriate or difficult to interpret after pregnancy begins.

If Cushing syndrome is suspected
- Tell the clinician how far along the pregnancy is and provide a complete list of prescription, over-the-counter, inhaled, topical, injected, and complementary products.
- Expect a discussion of urinary free cortisol rather than an initial dexamethasone suppression test.
- Complete the urine collection precisely and report missed samples, unusual fluid intake, or an acute illness that occurred during collection.
- Review the result with the treating clinician. A high value is not interpreted without considering trimester-specific physiology, the laboratory’s reference range, symptoms, and whether repeat testing is needed.
If adrenal insufficiency is suspected
When feasible, cosyntropin stimulation is the preferred confirmatory test. Pregnancy can make fatigue, nausea, and vomiting less specific, so clinicians rely more heavily on blood pressure, hydration, sodium, potassium, glucose, pigmentation changes, medication exposure, and the overall trajectory of illness. Obstetric and endocrine teams should agree on testing and treatment rather than relying on an over-the-counter kit.

What to do before the appointment
- Write down every glucocorticoid exposure, including dose, route, frequency, start date, last dose, and whether you recently stopped it.
- Record symptoms and their progression: blood-pressure readings if available, fainting or dizziness, vomiting, weight and body-shape changes, bruising, stretch marks, muscle weakness, skin darkening, and episodes of low blood sugar.
- Bring prior hormone results and imaging reports, especially any report mentioning an adrenal nodule or mass.
- Tell the clinician whether you are trying to conceive, could be pregnant, or are breastfeeding, and state the first day of your last menstrual period when relevant.
- Ask which sample is required, when it should be collected, whether normal medicines should be taken that day, and how a missed urine collection should be handled.
Do not alter steroid treatment, deliberately change sleep or food intake, or take supplements marketed as “cortisol regulators” to prepare for testing. Those changes can create risk or make the result harder to interpret.
Why random cortisol and home kits can mislead
Cortisol follows a daily rhythm and changes with acute illness. A random blood cortisol therefore cannot reliably screen for Cushing syndrome, and random cortisol is also not recommended for diagnosing central adrenal insufficiency. A result that looks high or low outside the correct clinical context may trigger unnecessary anxiety without answering the fertility question.
Home saliva or urine products may measure cortisol, but they are not equivalent to guideline-directed evaluation. They may lack pregnancy-adjusted reference ranges, validated collection controls, or a clinician who can connect the result to medication exposure and symptoms. If a consumer result worries you, bring the complete report to a licensed clinician instead of repeating tests on your own.

Seek emergency medical care for severe weakness, fainting, confusion, very low blood pressure, persistent vomiting, inability to keep fluids down, severe abdominal pain, fever with collapse, or suspected adrenal crisis—particularly if you have known adrenal disease or recently reduced long-term steroid treatment. The Endocrine Society recommends immediate hydrocortisone treatment when adrenal crisis is suspected; clinicians should not delay lifesaving treatment while waiting for cortisol results.
Pregnancy adds urgency to significant dehydration, repeated vomiting, fainting, or rapidly worsening symptoms. Tell emergency staff about pregnancy status and all steroid medicines so they can coordinate obstetric and endocrine care.
How results fit into fertility and pregnancy care
An abnormal cortisol result is a starting point, not a fertility verdict. For suspected Cushing syndrome, clinicians usually confirm biochemical excess before determining its source. For suspected adrenal insufficiency, stimulation testing, ACTH, electrolytes, and medication history help distinguish primary disease from pituitary suppression or exogenous steroid effects.
Treatment decisions are individualized around conception plans and gestational age. If an adrenal disorder is confirmed, the endocrine condition is managed while reproductive clinicians address ovulation, pregnancy planning, or prenatal monitoring. If testing is normal, the fertility evaluation returns to the more common causes identified by your history and reproductive assessment rather than assigning infertility to ordinary stress.
