PSMA PET is most useful when a prostate-cancer team has a specific decision to make: whether cancer has spread before treatment, where disease has returned after treatment, whether apparently nonmetastatic disease is visible on more sensitive imaging, or whether a patient’s tumors express enough PSMA to consider PSMA-directed radioligand therapy. It is not a universal screening test, a replacement for biopsy or PSA testing, or proof that cancer is absent when the scan is negative.
What PSMA PET is and what it can show
Prostate-specific membrane antigen (PSMA) is a cell-surface protein found at higher levels in many prostate-cancer cells. A PSMA PET scan injects a radioactive tracer designed to bind to that protein. The PET camera then detects where the tracer accumulates, usually as part of a PET/CT examination that also supplies an anatomic map.

The scan is therefore a molecular test rather than simply a picture of anatomy. It may reveal prostate-cancer deposits in lymph nodes, bone or other tissues that are too small or biologically inconspicuous for conventional imaging. However, PSMA expression varies among tumors, benign tissues can show uptake, and very small or low-PSMA lesions may remain below the method’s detection limit.

Interpretation depends on the tracer, the distribution of normal uptake in the body, the patient’s PSA and treatment history, pathology, and other imaging. A bright focus is not automatically metastatic cancer, and a scan without abnormal uptake does not establish that no cancer is present.

When clinicians routinely use PSMA PET
The 2023 joint procedure standard from the European Association of Nuclear Medicine and the Society of Nuclear Medicine and Molecular Imaging identifies four routine clinical scenarios. The practical question in each case is whether the result can change treatment or follow-up.
Initial staging when spread is a concern
After a prostate-cancer diagnosis, PSMA PET can help determine whether disease has reached lymph nodes or distant sites before surgery, radiation or another definitive plan. It is most relevant when the clinical and pathology findings create a meaningful suspicion of metastatic disease, not as an automatic step for every newly diagnosed patient.
Finding disease outside the prostate can alter the intent and scope of treatment. Conversely, a scan that shows no distant uptake does not guarantee organ-confined disease; microscopic deposits can be below PET resolution or may not express enough PSMA.

Localizing recurrent or persistent cancer
PSMA PET is commonly considered when cancer appears to persist after treatment or returns during follow-up. A rising PSA after surgery or radiation can indicate recurrent disease even when CT, bone scanning or other conventional tests are unrevealing. PET may help identify a prostate-bed recurrence, involved lymph node or distant lesion and thereby guide salvage radiation, surgery, systemic treatment or continued observation.
The appropriate timing depends on the PSA pattern, prior therapy and the decision under consideration. The scan is not a substitute for confirming what the PSA means clinically; it is an additional source of localization information.

Finding disease called nonmetastatic on conventional imaging
Some patients are classified as nonmetastatic because standard imaging shows no spread. PSMA PET may detect lesions that conventional imaging misses and can reclassify the extent of disease. This is particularly useful when the treatment plan would differ substantially if a small-volume metastasis were found.
A more sensitive scan can also create uncertainty: a lesion may be indeterminate, benign or too small to characterize confidently. Management should follow the entire clinical picture rather than a single isolated focus.
Staging before PSMA-directed radioligand therapy
PSMA PET can determine whether tumors express the target needed for a PSMA-directed radioligand treatment. In this setting, the scan is not merely looking for where cancer is; it is assessing whether the cancer displays enough targetable PSMA for a specific therapy pathway.
What US approvals cover
The National Cancer Institute’s prostate-cancer PDQ describes US-approved uses for gallium-68 gozetotide and fluorine-18 piflufolastat PET/CT in two broad situations: initial staging when metastatic disease is suspected, and evaluation of possible recurrence when PSA is elevated after treatment. The exact indication, age or disease criteria, and administration requirements come from the individual product label and the patient’s circumstances.
The US Food and Drug Administration’s Pylarify announcement similarly describes imaging patients with suspected metastasis who may still be candidates for potentially curative surgery or other treatment, and patients with suspected recurrence associated with an elevated PSA. Regulatory labeling and local availability can change, so a clinician or nuclear-medicine department must verify which tracer is licensed and appropriate in a particular region.

These approvals do not make PSMA PET a population-screening test. They also do not mean that every person with prostate cancer, every PSA rise or every treatment decision requires a scan.
Routine uses versus potential applications

“Potential application” does not mean useless; it means the guideline does not place that use on the same routine footing as the four established scenarios.
Who may receive little or no useful information
No source establishes a universal category of people who should never undergo PSMA PET. Benefit is limited when the scan cannot answer a defined management question, when the proposed use falls outside established indications, or when the likely result would not alter care.
People without a decision the scan could change
If treatment would be identical whether the scan is positive or negative, the information may not justify the procedure. Before ordering, ask what action would follow each plausible result. A scan should be connected to a real choice—such as local versus systemic treatment, a salvage target, or eligibility assessment for a named therapy.
People being scanned as a substitute for diagnosis
PSMA PET does not replace clinical examination, PSA measurement, prostate MRI when indicated, pathology or conventional imaging. A suspicious PET focus may require correlation or tissue sampling, while a negative scan cannot rule out cancer. Using PET as a stand-alone diagnostic shortcut can therefore mislead.

People with disease below detection limits or low PSMA expression
Very small deposits, lesions affected by scan resolution, and tumors that express little or no PSMA may not be visible. Some prostate cancers are biologically heterogeneous, so one lesion can be PSMA-avid while another is not. The risk of a false-negative interpretation is one reason PSA trends, pathology and other imaging remain important.
People whose result would be difficult to interpret
Normal organs and some noncancerous conditions can take up PSMA tracers. Prior treatment can alter anatomy and tracer distribution. An indeterminate focus may lead to additional imaging or follow-up rather than an immediate treatment change. Patients should understand that a scan can add information without producing a definitive answer.
People with patient-specific restrictions
The cited sources do not provide one complete contraindication list covering every PSMA tracer. The ordering clinician and the imaging service must review the selected product label, medical history and local protocol. Restrictions, precautions and availability can differ among tracers and jurisdictions; they should not be inferred from another product’s instructions.

Yes. A negative result means that no abnormal PSMA uptake was identified under the scan’s technical and biological limits; it does not prove that cancer is absent. Microscopic disease, low-PSMA tumors, lesions obscured by normal tracer activity and very small deposits can all produce a negative or equivocal study.

The FDA reported that, among a selected subset of patients with positive Pylarify readings and available corroboration from tissue pathology, conventional imaging or serial PSA, recurrence or metastasis was confirmed in an estimated 85% to 87% of cases, depending on the reader. This figure applies to that study-specific subset and confirmation method. It is not a universal sensitivity, specificity, accuracy rate or personal probability for every patient.
Positive findings also require context. A focus can represent prostate cancer, another malignancy, benign tissue or an imaging artifact. Clinicians integrate the location and intensity of uptake with PSA, treatment history, pathology and other tests before deciding whether to biopsy, treat or observe.
How tracers differ—and what cannot be claimed
Commonly used agents include gallium-68 gozetotide and fluorine-18 piflufolastat in the United States, along with other PSMA radioligands used under local approvals. The joint EANM/SNMMI guideline notes differences in radionuclide, radiochemistry, physiologic biodistribution and interpretation pitfalls.

Differences between tracers should not be turned into a universal ranking. The most suitable option is the one that is approved, available and interpretable for the patient’s clinical question.

PSMA PET has a distinct role when a clinician is considering PSMA-directed radioligand therapy. In a July 31, 2026 FDA announcement, the agency stated that patients with metastatic androgen-pathway-modulation-naive or androgen-pathway-modulation-sensitive prostate cancer should be selected for Pluvicto using Locametz or another approved PSMA PET product, based on PSMA expression in tumors. A March 28, 2025 FDA announcement also described PSMA PET selection in the expanded metastatic castration-resistant prostate-cancer indication.
These are treatment-specific contexts, not a recommendation that every patient receive PSMA PET or radioligand therapy. Eligibility also depends on the current label, disease state, previous treatments, laboratory findings and the treating team’s assessment. Because indications evolve, the clinician should confirm the latest labeling at the time treatment is considered.
Questions to ask before scheduling the scan
- What decision will this scan change? Ask what the team would do after a positive, negative or indeterminate result.
- Which indication applies? Clarify whether the purpose is initial staging, recurrence localization, reclassification after conventional imaging or therapy selection.
- Which tracer will be used, and is it approved here? Ask about the product name, local availability and tracer-specific interpretation issues.
- What are the scan’s limits in my case? Discuss PSA level and trend, prior treatment, pathology, lesion size and the possibility of low PSMA expression.
- What happens if the result is unclear? Establish whether follow-up imaging, biopsy, PSA monitoring or multidisciplinary review would be needed.
- Will other tests still be required? A PSMA PET result is interpreted alongside examination, PSA, pathology, MRI, CT, bone imaging or other studies when clinically indicated.
Bottom line
PSMA PET is a targeted problem-solving scan, not a universal prostate-cancer test. It is routinely used for selected staging, recurrence or persistence, detection of disease missed by conventional imaging and assessment of PSMA expression before specific radioligand therapies. Patients may gain little when no management decision depends on the result, when the use is outside established indications, or when disease biology and scan limitations make a false-negative or indeterminate result plausible. The safest interpretation combines the scan with PSA, pathology, prior treatment, other imaging and the exact decision being made.
