Call emergency services for a potentially life-threatening reaction, severe impairment, or breathing trouble. Contact your prescriber promptly for persistent severe abdominal pain, ongoing vomiting or diarrhea, suspected gallbladder disease, vision changes, or other symptoms that are severe, worsening, or not settling. Nausea, constipation, diarrhea, vomiting and abdominal discomfort can occur with GLP-1 medicines, but “common” never means that a severe or persistent episode should be ignored. The correct response depends on the medicine, dose, other medicines, medical history and the product’s current FDA-approved instructions.
Use the right level of urgency
There is no single emergency checklist that applies identically to every GLP-1 medicine. Wegovy (semaglutide), Mounjaro (tirzepatide), Zepbound (tirzepatide) and other products have medicine-specific labels. A compounded or otherwise unapproved product can add a separate dosing and quality risk. Use this triage framework while arranging advice from your own clinician.

Persistent, severe abdominal pain
The MOUNJARO prescribing information describes pancreatitis pain as “persistent severe abdominal pain, sometimes radiating to the back and which may or may not be accompanied by vomiting.” The WEGOVY label gives a similar warning pattern. Pain that is intense, does not let up, or is accompanied by repeated vomiting warrants prompt assessment; do not wait for a routine follow-up appointment.

What the labels instruct
Wegovy’s patient information instructs patients to discontinue promptly and contact their physician if pancreatitis is suspected. Mounjaro labeling says to discontinue if pancreatitis is suspected and initiate appropriate management. Those are product-specific directions, not a universal instruction to stop every GLP-1 medicine. Identify the exact medicine and dose when you call, and follow the current label and clinician guidance.
When nausea, vomiting or diarrhea need a call
Common does not mean harmless at any severity
FDA lists nausea, diarrhea, vomiting, constipation, abdominal discomfort and abdominal pain among common Zepbound side effects. Mild, short-lived symptoms may be managed with the plan given by your prescriber. The threshold changes when symptoms are severe, keep returning, prevent normal drinking, or are getting worse instead of improving.
Dehydration and kidney risk
Prolonged vomiting or diarrhea can remove enough fluid to cause volume depletion. FDA product labeling links this dehydration setting with worsening kidney function. Contact your healthcare provider for severe or persistent gastrointestinal symptoms, as the Wegovy patient counseling information specifically instructs. If someone cannot keep fluids down, is severely weak or confused, faints, or is in immediate danger, use emergency services rather than waiting for a callback.

Information to have ready
- The product’s exact brand and active ingredient, if known.
- Your dose, the date and time of the last dose, and any recent dose increase.
- How long the vomiting or diarrhea has lasted and whether fluids stay down.
- Other medicines, especially insulin, insulin-secretagogue diabetes medicines, diuretics or medicines that affect kidney function.
- Any severe pain, fever, fainting, reduced urination or other new symptom.
Possible gallbladder disease
FDA labeling for GLP-1 products addresses acute gallbladder disease and recommends clinical follow-up when it is suspected. Do not assume gallbladder problems happen only after rapid or substantial weight loss, and do not try to diagnose them from pain location alone. Report new or persistent abdominal symptoms to your clinician, who can decide whether examination, blood tests or imaging is appropriate.
Severe allergic reactions: emergency action
Severe hypersensitivity reactions, including anaphylaxis and angioedema, are identified in Mounjaro labeling. FDA’s Zepbound information says patients should stop Zepbound immediately and seek medical help if a severe allergic reaction is suspected. Symptoms that threaten breathing, swallowing or circulation require local emergency services immediately; do not wait for a routine appointment or an online message response. Follow any personal emergency plan, such as instructions about an epinephrine auto-injector, if one has been prescribed.
Low blood sugar when GLP-1 treatment is combined with other diabetes medicines
The risk of hypoglycemia is particularly important when a GLP-1 medicine is used with insulin or a medicine that stimulates insulin secretion, sometimes called an insulin secretagogue. FDA labeling warns that these combinations can increase the risk, so your diabetes team should provide an individualized action plan.

Follow that plan for checking glucose and taking the prescribed rescue carbohydrate or other treatment. If the person is severely impaired, cannot safely self-treat, cannot swallow safely, or is in immediate danger, use emergency services. Do not give food or drink to someone who cannot safely swallow. Tell emergency clinicians every diabetes medicine and the time of the last doses.
Wegovy-specific symptoms that should be reported
Vision changes
For a person taking Wegovy, new vision changes—particularly in someone with type 2 diabetes—should prompt contact with the physician. Do not assume the change is an ordinary digestive side effect or wait for the next routine eye appointment.
Palpitations or a racing heartbeat at rest
Wegovy information tells patients to report palpitations or a racing heartbeat while at rest to a healthcare provider. Record when the episodes occur, how long they last and whether they accompany fainting, chest discomfort or shortness of breath; those additional symptoms can change the urgency and should be reported immediately.

Before surgery, anesthesia or deep sedation
Tell the surgeon, anesthesiologist and procedural team that you use a GLP-1 medicine before any planned procedure involving general anesthesia or deep sedation. Wegovy delays stomach emptying, and its label discusses pulmonary aspiration risk during anesthesia or deep sedation. Do not stop or skip a dose on your own to prepare for a procedure. The procedural and prescribing clinicians must decide timing after considering the medicine, indication, dose schedule and your individual risk.

Why the product source matters
FDA says unapproved GLP-1 versions are not reviewed for safety, effectiveness or quality before marketing. FDA advises obtaining a prescription from a doctor and filling it at a state-licensed pharmacy. A vial, concentration or syringe instruction that differs from an FDA-approved pen can make a dose error easier.

What FDA has reported
As of May 31, 2026, FDA reported 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide. FDA cautions that these reports do not establish that the product caused each event and are likely underreported; the figures are not incidence rates. Reported effects in the semaglutide dosing-error information included gastrointestinal symptoms, fainting, dehydration, pancreatitis and gallstones, with some patients seeking medical attention or hospitalization.
If you may have taken the wrong amount
Do not try to calculate an unfamiliar vial dose from memory or internet advice. Call the prescriber or pharmacist promptly with the vial label, concentration, syringe markings, amount taken and time taken. If severe pain, repeated vomiting, fainting, severe weakness, breathing problems or another life-threatening symptom occurs, contact emergency services and bring the container or a photo of its label.
How to make a safer call to your care team
- Write down the exact product name, active ingredient, dose and formulation (pen, vial or other form).
- Describe the symptom in plain terms: when it began, whether it is constant, how severe it is and whether it is worsening.
- Report associated symptoms such as vomiting, diarrhea, pain to the back, inability to drink, vision changes, palpitations, rash, swelling or impaired awareness.
- List insulin, insulin secretagogues and every prescription, over-the-counter medicine and supplement you use.
- Ask whether you should be seen today, what warning signs require emergency services, and what to do about the next scheduled dose. Follow the clinician’s answer rather than making an independent dose change.
What the 2026 FDA suicidal-thoughts update means
On January 13, 2026, FDA said, “FDA’s evaluation did not identify an increased risk of suicidal ideation or behavior with the use of GLP-1 RA medications.” The agency’s analysis covered 91 placebo-controlled trials and 107,910 patients and found no increased risk of suicidal ideation or behavior or several other psychiatric adverse events. FDA requested removal of the suicidal-behavior warning from certain labels, so older websites or package inserts may contain outdated wording.
This finding does not make an individual mental-health crisis safe to ignore. If someone has thoughts of self-harm, cannot stay safe or is in immediate danger, contact local emergency services or an appropriate crisis service now, regardless of the suspected cause. For non-urgent mood changes, contact the prescribing clinician.
Questions to ask when warnings differ by product
When you read a leaflet or speak with a pharmacist, verify five details: the active ingredient and brand; the indication and formulation; the exact warning wording and action for that product; whether insulin or an insulin secretagogue is being used; and whether the medicine is FDA-approved or compounded. Never transfer a stop, restart or dose instruction from Wegovy, Mounjaro or Zepbound to another product without clinician confirmation.
A practical decision rule
Use emergency services for signs of a severe allergic reaction, severe hypoglycemia with inability to self-treat, or any symptom that puts breathing, consciousness or immediate safety at risk. Call the prescriber promptly for persistent severe abdominal pain, continuing vomiting or diarrhea, suspected gallbladder disease, Wegovy-related vision changes or resting palpitations. Mild digestive symptoms can be common, but they deserve reassessment whenever they become severe, persistent, dehydrating or accompanied by a new warning sign.
