When Constipation Needs Medical Attention: Warning Signs to Know (Adults)

Constipation needs medical attention when it comes with red-flag symptoms or does not settle with appropriate self-care. Seek immediate medical care for severe abdominal pain or a combination of pain, vomiting, abdominal swelling, and inability to pass stool or gas. Contact a clinician promptly for rectal bleeding, blood or black stools, constant abdominal pain, fever, lower-back pain, unexplained weight loss, or repeated vomiting. Arrange an appointment when constipation is persistent, recurrent, worsening, unusually severe, or linked to a sudden change in bowel habits.

What counts as constipation?

Constipation is more than simply having fewer bowel movements than someone else. NIDDK describes it as a pattern that may include fewer than three bowel movements a week, hard, dry, or lumpy stools, painful or difficult passage, or a feeling that the bowel has not emptied completely. Normal bowel frequency varies widely, so a meaningful change from your own usual pattern matters.

Constipation can occur by itself, but it can also accompany another medical problem. The warning signs below do not identify a specific diagnosis; they indicate when a healthcare professional should assess you.

NIDDK reports that about 16 out of 100 adults have constipation symptoms, citing a 2013 American Gastroenterological Association medical position statement. The estimate rises to about 33 out of 100 adults aged 60 and older. NIDDK’s page was last reviewed in May 2018, so these figures provide context rather than a newly collected 2026 prevalence estimate, and they are not thresholds for deciding whether symptoms are dangerous.

Get immediate medical care for severe pain or possible obstruction

Severe abdominal pain is the clearest reason to treat constipation as an immediate-care concern. Mayo Clinic advises immediate medical care for severe pain or other symptoms that could indicate an intestinal obstruction.

Constipation accompanied by several of the following symptoms can signal a serious blockage or another urgent condition:

  • Severe or rapidly worsening abdominal pain
  • Repeated vomiting
  • An abdomen that is noticeably swollen or distended
  • Inability to have a bowel movement
  • Inability to pass gas

These symptoms can have causes other than obstruction, and a symptom list cannot diagnose one. Do not wait for a laxative to work if severe pain, vomiting, swelling, or inability to pass gas is present. Use the emergency pathway appropriate to your location—such as emergency services or an emergency department—and tell the clinician when the constipation and associated symptoms began.

If you feel faint, confused, severely weak, or unable to keep fluids down, treat that deterioration as an emergency as well. Local services differ, so follow your region’s instructions rather than relying on a universal number or rule.

Warning signs that call for prompt clinician contact

NIDDK advises seeing a doctor right away when constipation occurs with rectal bleeding, blood in the stool, constant abdominal pain, inability to pass gas, vomiting, fever, lower-back pain, or unintentional weight loss. Mayo Clinic and NHS guidance identify additional circumstances that warrant medical review. The exact route—same-day assessment, an urgent clinic, or a routine appointment—depends on severity, speed of change, and your local healthcare system.

Symptom or pattern Why it matters Action
Rectal bleeding, red blood in stool, or black stools Bleeding is not explained safely by constipation alone. Mayo Clinic specifically lists black stools as a reason to consult. Contact a clinician promptly. Seek emergency care if bleeding is heavy, you feel faint, or it occurs with severe pain or weakness.
Constant or severe abdominal pain Persistent pain is a NIDDK warning sign; severe pain can occur with conditions requiring immediate assessment. Severe pain: immediate medical care. Constant but not severe pain: prompt clinician contact.
Vomiting Vomiting with constipation can accompany a serious blockage, especially when gas or stool cannot pass. Immediate assessment when vomiting is repeated, accompanied by severe pain, swelling, or inability to pass gas.
Fever Fever with bowel symptoms may indicate an illness that needs examination rather than more self-treatment. Contact a clinician promptly; use emergency care if fever accompanies severe pain, confusion, or rapid deterioration.
Inability to pass gas NIDDK lists this as a right-away warning sign, and Mayo Clinic includes it among possible obstruction symptoms. Seek urgent assessment, immediately if pain, vomiting, or abdominal swelling is also present.
Lower-back pain NIDDK includes lower-back pain among symptoms that should be assessed with constipation. Arrange prompt medical evaluation, especially if the pain is new, intense, or accompanied by urinary or neurological symptoms.
Unintentional weight loss Unexpected weight loss can signal an underlying illness and should not be attributed to constipation without assessment. Contact a clinician promptly.
Regular bloating, persistent tiredness, or a sudden bowel-habit change NHS adult guidance lists these patterns as reasons to see a GP. Arrange a medical appointment rather than repeatedly treating the symptom yourself.
Alternating constipation and diarrhea, unusual stool changes, or symptoms that interfere with daily activities Mayo Clinic identifies these changes as reasons for consultation. Book a clinician visit; seek faster care if severe pain, bleeding, vomiting, or dehydration develops.

When persistent or recurring constipation needs an appointment

Even without an emergency symptom, arrange medical review when constipation does not improve with self-care or recommended treatment, keeps returning, persists, or becomes worse. NHS guidance says to see a GP when constipation is not improving with treatment or is regular or recurrent. Mayo Clinic advises consultation when it is more severe or lasts longer than usual, and its symptom guidance includes symptoms lasting longer than three weeks.

There is no single number of days that safely applies to everyone. “Longer than usual” depends on your baseline, and a sudden change can matter even before a fixed time period has passed. Do not postpone an appointment simply because you have not reached three weeks, and do not assume that three weeks is an emergency threshold; the sources use different measures because urgency depends on the whole symptom pattern.

Arrange review particularly soon when constipation begins suddenly in an adult who normally has a regular pattern, alternates with diarrhea, or is accompanied by ongoing stomach or rectal pain, bleeding, unexplained weight loss, or persistent fatigue. NIDDK also advises medical discussion when symptoms do not go away with self-care or when there is a family history of colon or rectal cancer.

If a prescribed medicine may be contributing, tell the prescriber or GP. NHS guidance specifically lists medicines that may cause constipation as a reason for review. Do not stop a prescribed medicine on your own; the clinician can decide whether the dose, timing, or treatment needs to change.

A practical decision path for adults

  1. Check for immediate danger. If pain is severe, the abdomen is swelling, vomiting is repeated, or you cannot pass stool or gas, use emergency medical care now. Do not try to establish the cause at home.
  2. Look for right-away warning signs. Blood in the stool or from the rectum, constant abdominal pain, fever, lower-back pain, unexplained weight loss, vomiting, or inability to pass gas warrants prompt contact with a doctor. Escalate to emergency care if symptoms are severe or rapidly worsening.
  3. Assess the course. Note whether the problem is new, persists beyond your normal pattern, recurs, alternates with diarrhea, or is not improving with treatment. Any of these patterns supports arranging an appointment.
  4. Consider contributing medicines and history. Make a list of prescribed and over-the-counter medicines, supplements, recent illnesses, and any family history of colon or rectal cancer. Share it rather than stopping treatment without advice.
  5. Use self-care only while it is clearly safe. If no warning sign is present and symptoms are mild, follow the self-care or treatment instructions you have been given. Stop self-treatment and seek advice if symptoms worsen or a red flag appears.

What to tell the clinician

Useful details help a clinician judge urgency. Record when the change began, your usual bowel pattern, stool appearance and consistency, how often you pass stool, whether you can pass gas, and whether pain is constant or comes in waves. Note vomiting, fever, bleeding, black stools, bloating, back pain, fatigue, weight change, and any alternating diarrhea.

Bring the names and doses of medicines and supplements, including products bought without a prescription. Mention recent changes, medicines known to slow the bowel, previous episodes, and any family history of colon or rectal cancer. Do not delay urgent care in order to complete a diary; these details are for an appointment when you are stable.

Adults and children have different guidance

This guidance is for adults. NHS constipation information states that children have separate advice, and a child with constipation should be assessed using pediatric guidance rather than adult thresholds. For an infant or child with severe pain, vomiting, swelling, blood, fever, or inability to pass stool or gas, contact an appropriate pediatric or emergency service.

How the major guidance sources frame urgency

NIDDK provides a direct list of symptoms for which adults should see a doctor right away and advises review when symptoms do not resolve with self-care. NHS adult guidance emphasizes GP review for treatment-resistant, regular, or recurrent constipation and for associated bloating, bleeding, weight loss, tiredness, medicine effects, sudden bowel-habit changes, or tummy pain. Mayo Clinic adds unusually severe or prolonged symptoms, alternating diarrhea, black stools, unusual stool changes, and difficulty with daily activities. Its intestinal-obstruction guidance supports immediate care for severe pain or a cluster of pain, vomiting, swelling, and inability to pass stool or gas.

Because these authorities use slightly different wording and healthcare pathways, apply the most urgent reasonable route to your symptom severity and local system. None of the lists proves cancer, obstruction, or another diagnosis; they identify situations in which examination is safer than continued self-treatment.
