When Breast Pain or a Lump Needs Medical Attention: Symptoms Not to Ignore

A new breast or armpit lump, an unusual breast or nipple change, or breast pain that persists should be assessed by a healthcare professional. Pain by itself is often not a sign of cancer, and many breast changes have benign causes—but symptoms alone cannot establish what is causing a change.

Which breast changes should be checked?

Contact a healthcare professional about a new lump in the breast or armpit, unusual thickening or firmness, or another change that is new for you. Other changes to report include:

  • Skin dimpling, puckering, redness, or scaling.
  • A new nipple turning inward, a nipple rash, or discharge that is not breast milk—especially if it is bloody.
  • A change in the size or shape of a breast.

These signs are reasons to seek assessment, not proof of cancer. A lump’s feel, whether it moves, and whether it hurts cannot reliably determine its cause. Many lumps are benign, including cysts and fibroadenomas, but a clinician should assess a new lump rather than relying on touch to judge it. The NHS explains when to seek help for a breast lump; the National Cancer Institute (NCI) and CDC also list breast changes to discuss with a healthcare professional.

When is breast pain a reason to seek care?

Persistent or unexplained pain

Breast pain alone is usually not a sign of most breast cancers. Pain can have many other causes, including hormonal changes, pregnancy, menopause, certain medicines, injury or strain in the neck, shoulder, or back, and infection. The NHS describes period-related pain that may start up to two weeks before a period, worsen, and ease when the period ends; it may affect both breasts and spread toward the armpit. That pattern can be useful context, but it is not a way to diagnose or rule out a cause.

Arrange medical advice if pain persists or is not improving, painkillers are not helping, or you have a family history of breast cancer. The American Cancer Society notes that cancer-associated pain, when it occurs, is more often constant and noncyclical and may occur alongside other symptoms; this pattern is not a rule-out test. As the NCI puts it, “If you have breast pain that persists, it’s important to see a doctor.” Read the NHS guidance on breast pain and the American Cancer Society’s explanation of breast pain and cancer.

Pain with signs of infection or feeling unwell

Seek urgent advice if breast pain comes with a very high temperature or feeling hot, cold, or shivery, or if an area of the breast becomes red, hot, or swollen. Infection, including mastitis or a breast abscess, is among the possible causes of breast pain, but a clinician can advise on the appropriate assessment.

How quickly should you get help?

For a new lump, an unusual breast or nipple change, or persistent pain, contact a healthcare professional for assessment. Under NHS guidance in the UK, seek an urgent GP appointment or call NHS 111 for breast pain with a very high temperature or feeling hot, cold, or shivery; a red, hot, or swollen area; a hard lump that does not move; discharge that may be blood-streaked; a change in breast shape; skin dimpling; or a nipple rash or newly sunken nipple. These are UK care routes; outside the UK, contact your local healthcare service for advice on urgency.

A recent normal mammogram does not settle a new symptom. The NCI advises following up a new breast change even after a normal mammogram.

What may happen at an assessment?

A clinician may examine your breasts and, if the cause is unclear, refer you to a breast clinic. NHS guidance describes possible clinic assessment with a breast examination, mammogram or ultrasound, and a needle biopsy. The tests depend on the situation; the NHS says they are often done during the same visit, while biopsy results take longer and may take about a week. This is not a guarantee of a particular test or same-day result. See the NHS overview of breast lump assessment.

Why symptoms alone cannot settle the cause

Breast changes are common and most are not cancer, but symptoms can have different causes and cannot be safely diagnosed by appearance, feel, or pain level alone. Do not ignore a new lump because it does not hurt, or assume pain is required for a concerning change. An assessment is the way to investigate what is happening.
