There is no single confirmed cause. U.S. studies find that diagnoses increased for some cancers and age groups under 50, while other cancers declined and the overall cancer incidence rate did not rise in the main 2010–2019 analysis. Researchers are examining changes in underlying risk, screening and detection, and the way cancers are diagnosed and coded. A newer report found that the overall cancer incidence rate for adolescents and young adults ages 15–39 was stable from 2018 through 2022, even as particular cancer types rose over longer or different periods.
“Younger adults” is not one standard age group
Any answer depends on the population and years being measured. The National Cancer Institute’s 2025 analysis divided people younger than 50 into three groups: ages 15–29, 30–39, and 40–49. NCI’s adolescent and young adult (AYA) statistics generally cover ages 15–39. Its Early-Onset Cancer Initiative describes cancers in people ages 18–49 that are more commonly diagnosed later in life.

These categories should not be confused with the age distribution of cancer overall. The median age at a cancer diagnosis in NCI’s SEER data through 2022 was 67. The current concern is therefore not that cancer has become more common than aging-related disease among young people. It is that selected cancers appear to be occurring more often, or being identified more often, in certain younger age bands.

What the main U.S. trend study found
An NCI-led study examined 33 cancer types in U.S. data. From 2010 through 2019, incidence increased for 14 types in at least one age group under 50. Incidence for 19 other cancer types, including lung and prostate cancer, decreased in at least one under-50 age group.

When all cancers were combined, the total incidence rate did not increase in younger age groups or in older groups during that analysis. The total cancer death rate also did not increase. Nine of the 14 cancer types that rose in at least one younger age group also rose in at least one age group over 50. That pattern argues against a single explanation that applies only to young adults.
The largest estimated additions in 2019
NCI estimated how many more early-onset diagnoses occurred in 2019 than would have been expected if the 2010 rates had continued. These are modeled extra case counts for one year, not percentage increases and not evidence that a particular factor caused them.
| Cancer type | Estimated additional cases in 2019 |
|---|---|
| Female breast cancer | About 4,800 |
| Colorectal cancer | About 2,100 |
| Kidney cancer | About 1,800 |
| Uterine cancer | About 1,200 |
| Pancreatic cancer | About 500 |
The estimates compare one observed year with a modeled continuation of an earlier rate. They should not be read as a count of cases caused by a newly emerged exposure, nor as a forecast of future diagnoses.

Incidence is the number of new diagnoses in a population over a specified period. Mortality is the number of deaths. A report can show rising incidence for one cancer while overall incidence is flat and death rates are falling.
NCI’s AYA page reports average annual incidence growth of 0.3% for ages 15–39 from 2013 through 2022, with death rates falling from 2014 through 2023. The NCI Annual Report to the Nation, published in September 2026, says overall incidence among AYAs ages 15–39 was stable from 2018 through 2022. It also reports that cancer death rates in this age group declined by about 1% per year on average from 2004 through 2023.

Those findings use different incidence windows and statistical summaries. They do not cancel one another. Before comparing a headline with another headline, check four details:
- the exact age range, such as 15–39 or 40–49;
- the calendar years included;
- whether the result covers all cancers or a particular site; and
- whether the measure is incidence, mortality, or an average annual change.
The principal trend study covered U.S. incidence through 2019 and mortality through 2022. The newer AYA report extends the aggregate incidence window to 2022, but its summary does not establish causes for individual cancer trends.

What might be driving the increases?
The evidence supports several categories of explanation, not one proven cause. Meredith Shiels, Ph.D., the NCI study’s lead investigator, described the uncertainty this way: “The causes of these increases are likely to be cancer specific, including cancer risk factors becoming more common at younger ages, changes in cancer screening or detection, and updates to clinical diagnosis or coding of cancers.”
Risk factors may be appearing earlier in life
One possibility is that exposures or conditions associated with a particular cancer have become more common, or have begun earlier, in younger birth cohorts. This could affect the biology of a cancer before age 50. The same factor might influence one organ strongly and have little measurable effect on another, which is why a cancer-specific approach is essential.
NCI discussions have included obesity and heavier alcohol consumption as possible contributors for some cancers. They also mention environmental exposures and changes in the microbiome as hypotheses under investigation. These are candidate mechanisms, not established explanations for the overall pattern. For many proposed individual links, the evidence is still limited or indirect.

What the obesity evidence does and does not show
NCI’s obesity fact sheet cites a 2024 study of worldwide trends from 2000 through 2012. Six of nine obesity-related cancer types increased among young adults in that analysis. Increases in body weight were associated with increases in colon, rectal, pancreatic, and kidney cancers.
This is consistent with obesity contributing to risk for some cancer types. It does not demonstrate that obesity explains every early-onset increase, prove that weight change caused an individual diagnosis, or account for cancers with different risk profiles. The study also covers an earlier global period than the U.S. analyses, so its results should not be treated as a direct measurement of current U.S. incidence.
Screening and detection can change the numbers
A rise in recorded diagnoses can occur when a disease is found more often, earlier, or after a change in who is offered testing. Better imaging, broader use of diagnostic procedures, increased awareness, and changes in clinical practice can all alter incidence statistics even when the underlying occurrence changes less.
Detection can also shift the timing of a diagnosis. Finding a tumor earlier may move a case into a younger age band or an earlier calendar year. That does not mean detection is the only explanation for the trends; it means incidence data alone cannot separate a true change in disease occurrence from a change in when and how cases are identified.
Diagnosis and coding changes matter
Medical records and cancer registries rely on diagnostic definitions and coding systems. Updates to clinical classification or coding can affect which cases are counted and how they are grouped. NCI investigators therefore include coding changes among the possible contributors, particularly when a trend is not accompanied by comparable evidence from other data sources.

A coding explanation does not make the diagnoses unreal. It qualifies what a trend means: a higher recorded rate may reflect a combination of biology, clinical recognition, and measurement.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to weigh the competing explanations
No current evidence supports ranking one proposed factor as the confirmed cause of the overall rise. A useful way to evaluate a claim is to ask four questions:
| Question | Why it matters |
|---|---|
| Which cancer and age group? | A factor associated with colorectal cancer in ages 30–49 may not explain breast, pancreatic, or uterine trends, or a pattern in teenagers. |
| How direct is the evidence? | Registry correlations, international studies, biological mechanisms, and prospective exposure studies answer different questions and carry different levels of uncertainty. |
| Does it change risk or only timing? | An exposure may increase the chance of developing a tumor; screening or coding may primarily change when a case is recorded. |
| Where and when was it measured? | A global 2000–2012 association cannot automatically be applied to every U.S. age group in 2026. |
Claims that treat one association as a universal cause fail these checks. The NCI results instead point toward multiple, cancer-specific processes that may overlap.
What the findings mean for people under 50
The data do not show a uniform increase in every cancer among younger adults. They also do not show that a young adult’s individual risk can be calculated from a population trend alone. Personal risk depends on age, family history, inherited variants, previous medical treatment, exposures, and other factors that population studies may not measure in the same way.

The appropriate evaluation depends on the symptom and medical history. Population statistics cannot determine whether a specific symptom is cancer, and they should not be used to justify unproven supplements, home diagnostic products, or self-treatment.
Best Value
Likewise, a diagnosis in a younger adult does not by itself identify its cause. A registry trend can reveal that rates changed; it cannot reconstruct all of the exposures, biology, and clinical decisions behind one person’s case.
What researchers still need to resolve
Future work must connect cancer-registry trends with detailed information about exposures, body weight over time, alcohol use, medications, infections, environmental conditions, family history, screening, and diagnostic pathways. Researchers also need analyses that distinguish birth-cohort effects from period effects: whether people born in particular years carry different risks, or whether something changed for everyone during a specific period.
Longer follow-up will show whether the site-specific increases continue after 2019 and whether the stable aggregate AYA incidence reported for 2018–2022 persists. More detailed comparisons can also test whether a rise is concentrated in one age band, stage, or subtype. Mortality trends provide an additional check, but falling death rates can coexist with rising incidence when detection and treatment improve.

Some cancers are being diagnosed more often in selected younger age groups, but the United States has not experienced a blanket increase in all cancer incidence among people under 50. The largest modeled additions in the NCI analysis were in female breast, colorectal, kidney, uterine, and pancreatic cancers. Several cancers that increased in younger adults also increased in at least some older groups.
Researchers are testing explanations involving changing risk factors, obesity and alcohol patterns for certain cancers, environmental and microbiome influences, screening and detection, and diagnostic or coding practices. The available evidence does not establish one overall cause. The most accurate interpretation is therefore cancer-specific and time-specific: identify the cancer, age band, geography, years, and measure before deciding what a reported “rise” means.
