Wearable health trackers can record movement and, depending on the model, estimate selected signals such as heart rate, sleep patterns, or blood-oxygen saturation. Some offer an ECG feature for a narrowly defined heart-rhythm assessment. What they cannot do is turn every reading into a diagnosis: capabilities and accuracy vary by metric, device, intended use, and conditions.
What can a fitness tracker measure?
Most watches, rings, and bands collect movement data. Some use optical sensors to estimate heart rate and related patterns; specific models also offer features such as an ECG, oxygen-saturation estimate, temperature measurement, or sleep analysis. Those capabilities are not standard across wearables, and two devices that show similarly named metrics may use different sensors or algorithms.

It helps to distinguish what the device does from what its screen calls the result. A sensor can record a signal; software may then estimate a value or classify a pattern from it. A daily score or trend is not, by itself, a clinical diagnosis. Use “detects” or “diagnoses” only for a feature and intended use supported by its evidence and regulatory status.

The FDA maintains a list of medical devices that incorporate sensor-based digital health technology, including qualifying wearables. The list is not comprehensive, and inclusion relates to the particular device and its intended use—not blanket approval of every health feature on a watch, ring, or band.

How accurate are smartwatches for health?
There is no single accuracy rating that applies to every metric or wearable. A 2026 living systematic review in npj Digital Medicine assessed Apple Watch measurement studies published through September 24, 2025. It found that performance varied by measurement: heart-rate results had a small average bias but moderate variability; oxygen-saturation readings had low average bias but wide limits of agreement; sleep and step-count accuracy was moderate; and energy-expenditure errors were inconsistent and often large. These findings describe the reviewed Apple Watch studies, not every brand or current model.
| Metric | What the evidence says | Practical reading |
|---|---|---|
| Heart rate | The Apple Watch review found a small average bias but moderate measurement variability. | An average close to a reference does not mean every reading is close; treat individual results in context. |
| Blood-oxygen saturation | The review found low average bias but wide limits of agreement. | A result can differ meaningfully from a reference, so do not use a consumer reading alone to make a health decision. |
| Sleep and steps | The review found moderate accuracy. | These outputs may help follow patterns, but they are not equivalent to a clinical sleep assessment or a precise count in every situation. |
| Energy expenditure | The review found errors that were inconsistent and often large. | Do not assume a calorie estimate is an exact measure of energy burned. |
Fit and skin contact, activity, the measurement environment, and the person wearing the device can all affect results. A trend against your own baseline may be useful for awareness, but it is not proof that a change is medically significant. Check the exact model, the metric-specific evidence, and whether the feature is intended for wellness or for a defined medical use. Regulatory status and feature availability can also differ by model and location.
Apple’s September 2026 announcement described more frequent heart-rate and heart-rate-variability sampling and a readiness score for Apple Watch Series 12 and Ultra 4. Apple also described a comparison study with more than 1,000 participants and claimed its new heart-rate sensing had the “highest accuracy” among leading wearables. That is a company-reported claim, not an independent ranking established by the systematic review.

Can a smartwatch measure blood sugar?
Not independently in a way consumers should rely on. In a February 21, 2024 safety communication, the FDA said: “Do not buy or use smartwatches or smart rings that claim to measure blood glucose levels.” The agency said it had not authorized, cleared, or approved a smartwatch or ring intended to measure or estimate glucose on its own.
A watch app that displays readings from an authorized glucose-monitoring device is a different use: the watch is showing data collected by that monitor, not measuring glucose itself. If you need glucose monitoring, ask a health professional about an appropriate authorized device rather than substituting a watch or ring that claims to read glucose without piercing the skin.
Can an Apple Watch ECG detect a heart attack?
No. Apple says its ECG app, available on Apple Watch Series 4 or later and Apple Watch Ultra models, produces a waveform similar to a single-lead ECG. It can provide heart-rate and rhythm information and classify certain recordings as atrial fibrillation or sinus rhythm; it is not a full diagnostic workup. Apple’s support page states: “The ECG app cannot detect a heart attack.” It also cannot detect a stroke, blood clots, high blood pressure, high cholesterol, congestive heart failure, or every form of arrhythmia.

Apple reports that a clinical trial of approximately 600 subjects found 99.6% specificity for sinus-rhythm classification and 98.3% sensitivity for atrial-fibrillation classification among recordings that could be classified. Those are feature-specific results from a controlled trial, not a general measure of smartwatch accuracy. Apple warns that real-world recordings may be inconclusive or unclassifiable.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Are wearable oxygen readings dependable?
Blood-oxygen features on consumer wearables provide estimates, and oxygen measurement has known accuracy limitations. FDA materials identify skin pigmentation as one factor that can affect pulse-oximeter accuracy. The agency’s consumer guidance also lists poor circulation, skin thickness, skin temperature, tobacco use, and nail polish as possible influences.
The FDA’s January 2025 pulse-oximeter guidance is a draft, non-binding document—not a finalized rule. If you are concerned about symptoms or a reading, consider how you feel and contact a health care provider rather than relying on the number alone.
Quick Recap
How should you compare wearable health features?
- Start with the metric. Evidence for step counts does not establish accuracy for sleep, oxygen, ECG, or another output.
- Check the exact model and intended use. Features and regulatory status can vary by generation and geography. An FDA listing or authorization applies to a particular device and intended use, not to every feature in its product category; the FDA’s sensor-based device list is not comprehensive.
- Look for metric-specific validation. Separate independent evidence for the relevant model and population from a manufacturer’s own study or marketing claim.
- Ask what the number represents. A direct sensor output, an algorithmic estimate, and a classification are different kinds of information.
- Consider measurement conditions. Fit, contact, activity, skin characteristics, and environment can affect what a wearable records or estimates.
- Decide how you will use it. A personal trend can support awareness or a conversation with a clinician. It should not replace a medical device or professional assessment when one is needed.
