Misophonia support can include individualized psychological care—often using cognitive behavioral therapy (CBT)-based approaches—and practical accommodations for difficult sounds. No single treatment is established as best for everyone. Earplugs, headphones, quiet spaces, and background sound may help with coping, but they are not cures or stand-alone treatment.
What treatment for misophonia may involve
A clinician may tailor psychological care to the person’s triggers, distress, emotional reactions, avoidance patterns, and the effect of misophonia on daily life. CBT-family approaches are the most frequently described in the treatment literature, but there is no universal protocol.

CBT-based care
Depending on the person’s needs, a CBT-informed approach may include learning about misophonia, setting goals, identifying triggers and emotions, building emotion-regulation or distress-tolerance skills, trying behavioral experiments, and developing attention strategies. Some approaches may use carefully designed, graded exposure-related procedures. Exposure is not automatically appropriate or sufficient for everyone; the clinician should explain how it fits the individual plan.

A 2024 pilot study evaluated the Unified Protocol, a transdiagnostic CBT approach, with 8 patients in an initial feedback group and 10 in a revised-treatment group. The authors reported that it was acceptable and found preliminary improvement, particularly in emotional and behavioral responses. The small pilot does not establish definitive efficacy. 2024 Unified Protocol pilot study.
A PubMed-indexed randomized trial abstract describes 60 adults assigned either to individualized acceptance and commitment therapy (ACT) plus audiologically informed behavioral strategies or to education plus progressive relaxation training. The abstract does not provide the trial results, so it cannot support a claim that one approach performed better. Randomized trial abstract.

Medication and co-occurring needs
The sources reviewed do not establish medication as a specific treatment for misophonia. The American Psychiatric Association notes that medication or talk therapy may help with underlying mental-health issues. That is support for treating co-occurring concerns, not evidence that a medicine treats misophonia itself. American Psychiatric Association: Misophonia.

What the evidence can—and cannot—tell you
A 2023 systematic review described 33 treatment studies: one randomized controlled trial, one open-label trial, and 31 case studies. CBT incorporating different components was the most frequently used and was described as effective in the included reports. The review also emphasized limited rigor, small samples, few comparative studies, and limited replication. These findings suggest a developing evidence base, not proof that CBT works for everyone. A 2026 scoping review reported 30 publications with pre-to-post treatment measurement. It described varied CBT-family procedures as potentially helpful and deliverable, while noting uncontrolled designs, small samples, heterogeneous procedures, and inconsistent measures. The review did not pool effects, so it does not establish firm efficacy. 2026 scoping review.

Practical ways to manage trigger sounds
The APA suggests learning which sounds trigger reactions, finding or creating spaces where triggers are less likely, using earplugs or noise-cancelling headphones, and trying pleasant or distracting sounds. Mindfulness and relaxation may help manage stress and distress. These options may make situations more manageable; they should not be presented as cures. American Psychiatric Association: Misophonia.

The International OCD Foundation describes protective equipment and quiet zones as possible accommodations while a person develops adaptive strategies—not as stand-alone treatment. It cautions against relying on extensive avoidance, such as eating in isolation, because that may affect adaptive skills and social life. Support at home, school, and work
Support is most useful when it helps someone participate in ordinary activities rather than steadily narrowing them. A practical conversation can identify the situations that are hardest, what limited accommodations would make participation easier, and how the person can explain their needs. Options might include agreed access to a quieter space or permission to use sound protection in specific settings. Oxford Health NHS includes communication and accommodation planning among possible parts of CBT-informed support; arrangements should be tailored to the setting and individual.
How to choose a clinician or support plan
- Ask what the approach is based on. Find out whether the clinician is proposing CBT-family care, another approach, or coping support, and how strong the evidence is for the specific method.
- Look for an individualized plan. The proposed goals should reflect the person’s actual triggers, distress, behavior, and daily-life needs—not a one-size-fits-all protocol.
- Agree on how progress will be reviewed. Ask what changes the clinician will track and when the plan might be adjusted.
- Consider function as well as symptom relief. Accommodations may help, but a plan based only on avoiding more and more situations can carry costs. Discuss how to preserve relationships and everyday participation.
