Check the romper itself first: its lining, opacity, fit while moving and whether it has built-in shorts. Add a comfortable layer only if the child prefers it for coverage, warmth or comfort; an off-the-shoulder neckline alone does not mean she needs a bra or specialty underwear.
Start with the romper’s construction
Rompers are designed as one-piece outfits, and a U.S. Customs and Border Protection apparel publication defines a romper as “An abbreviated one piece garment designed to be worn alone as informal attire for casual wear use.” That general definition does not guarantee that every individual romper is opaque, lined or comfortable for every wearer.

- Lining: Check whether the bodice—or the whole romper—is lined.
- Opacity: In ordinary light, see whether the fabric provides the coverage the child wants.
- Built-in shorts: If they are included, separate shorts may be unnecessary.
- Fit and movement: Have the child sit, reach and move around. Notice whether the neckline stays comfortably in place and whether the garment shifts or pinches.
Construction varies by product. For example, an Aeropostale off-the-shoulder romper listing describes a lined front bodice and built-in shorts, while Marlo Kids’ Angelina Lace Romper listing describes lining throughout. Those are examples, not rules for all rompers; listing details can change.

Choose an optional layer by comfort
If the child wants an extra layer, familiar everyday underwear is a reasonable starting point. The garment listings do not establish that a particular fabric, cut or specialty product is necessary. For a younger child, make the choice with a caregiver and let comfort guide it.
Consider how any extra clothing will work with the romper: a layer may show at the neckline or arm openings, bunch beneath the fabric, or add warmth. A camisole or short-sleeved top can be worn for personal preference or warmth, but it may change the off-shoulder look. It is not a default requirement.

If coverage is the concern
First check whether the romper itself is sufficiently opaque and fits well during normal movement. If the child still wants more coverage, choose a comfortable layer that stays in place and does not interfere with moving. Built-in shorts, when present, may already provide the separate bottom layer someone might otherwise consider.
