Try: “I’m not sure what to say, but I want you to know I care. I’m here for you.” Then pause and let them decide whether to talk, sit quietly, or change the subject. You do not need to find a perfect line or make their feelings go away; listening without judgment is often the most helpful response.

Start by acknowledging what they said

Keep your first response simple and honest. The American Cancer Society recommends listening and letting the person know you are available. It offers this example: “I’m not sure what to say, but I want you to know I care. I’m here for you.” (American Cancer Society: When Someone You Know Has Cancer)

You might also say, “It’s okay to feel how you’re feeling. I’m here for you no matter what.” Say it only if it feels sincere, then leave room for their response. The American Cancer Society notes that you do not always have to respond; sometimes listening is enough.

Let them choose what support looks like

People differ in what they want from a conversation. Ask rather than guessing, and accept the answer without pushing.

  • Listening or advice: “Would it help if I listened, or would you like me to help think through something?”
  • Company or quiet: “Would company help right now, or would you rather have some quiet?”
  • Cancer talk or ordinary conversation: “Do you want to talk about what’s going on, or would you rather talk about something else?”

The American Cancer Society advises asking whether someone wants advice or support. Macmillan Cancer Support also notes that a person may not be ready to talk, and that conversation does not always have to focus on cancer. Listen without trying to fix the feeling

Pay attention to what they say instead of planning a solution while they are speaking. A brief reflection can show you heard them: “It sounds like the uncertainty is exhausting.” You can also ask, “Did I understand that right?” Macmillan recommends repeating back what you heard to check your understanding.

If they mention fear or death, do not rush to reassure them or answer a question when you do not know the answer. You could say, “That sounds frightening. I don’t know what will happen, but I can stay with you.” The American Cancer Society says it is all right not to have an answer and not to respond to every difficult statement with a solution.

Offer practical help in a specific, low-pressure way

“Let me know if you need anything” can be hard to act on. Instead, offer one or two concrete options and make clear that declining is fine: “Can I bring dinner, pick something up, or take care of an errand?” The American Cancer Society suggests practical help such as meals, groceries, laundry, pet care, or a ride. If they say no, accept it and leave the invitation open rather than insisting.

Avoid phrases that can shut down an honest conversation

  • “Everything will be fine.” You cannot promise an outcome, and false reassurance can make it harder for someone to speak honestly. Macmillan advises against this kind of reassurance and against urging someone to stay positive.
  • “You’re so strong” or “You’re so brave.” Some people do not feel strong or brave; praise meant kindly may make it harder for them to say they feel scared, sad, angry, or lonely. The American Cancer Society suggests being careful with these compliments.
  • “You have to fight,” “win,” or “beat this.” Battle language motivates some people and hurts others, including some people with advanced or incurable cancer. Ask whether they are comfortable with those words rather than choosing them for the person.
  • “I know someone who had cancer and…” Another person’s experience may not match theirs. Cancer and people’s responses to it differ; do not use a comparison to tell them how they should feel.

When distress continues, raise professional support gently

If sadness, withdrawal, or other mental health symptoms continue, the American Cancer Society suggests offering help finding a counselor or therapist who has experience supporting people affected by cancer. You might say, “Would you like help finding someone to talk to?” This is a general support suggestion, not a diagnosis or a crisis assessment.

These communication suggestions are not emergency guidance. If you believe the person may be in immediate danger, seek urgent professional help rather than relying on conversation tips alone.
