What to Log and Monitor When AI Agents Access Company Systems

Log every AI-agent action as an attributable system event: the agent and delegated human or service identity, authentication and authorization decision, tool or API call, target resource, parameters, data returned or changed, approval status, time, trace identifiers, result, errors and state changes. Monitor those events centrally, correlate them with identity, cloud, application, database, endpoint and network records, and alert on privilege expansion, unusual access, policy violations, missing telemetry and high-impact external effects.

Why agent activity needs its own audit trail

Traditional application logs often show that an application endpoint was called, but not why an AI agent called it, which model-generated plan led to the call, what arguments it supplied, or which person authorized the resulting change. An agent can read several systems, invoke tools in sequence, retain memory and delegate work to another agent. An audit trail that captures only the final application request cannot reconstruct that chain.

Each deployment should therefore be treated as a distinct, least-privilege system actor. The record must connect machine activity to the human or service principal that delegated authority. This makes it possible to answer practical questions such as who approved a data export, whether the agent exceeded its scope, and which downstream systems were affected.

Government guidance is consistent on the basic principle. CISA describes logging as recording “who accessed what, when, and from where,” while its business guidance says, “Every time someone logs in, accesses a file, or makes a change to your system, it leaves a digital record.” A 2026 Singapore government addendum for agentic AI extends that idea to agent actions, API calls, inputs and outputs, internal state changes, errors, timestamps, duration and contextual identifiers.

The fields every agent event should contain

Use a versioned, structured event format such as JSON rather than free-form text. The exact schema can vary by platform, but the following fields give investigators enough context to reconstruct a run.

Event area Capture Investigation value
Identity and authority Agent ID; deployment and model version; tenant; session; initiating user or service principal; authentication method; granted scopes; authorization and policy decision Establishes which software instance acted and whose authority it used
Action and target Tool or API name and version; destination system; resource or object; operation; complete parameter set after safe redaction Shows precisely what the agent attempted and where
Data access Records, files or fields returned, created, modified or deleted; classification of the payload; volume and direction of transfer Supports privacy, breach and data-loss investigations without requiring unrestricted content retention
Approval and policy Required approval; approver identity; approval timestamp; human-in-the-loop decision; policy rule and evaluation result Demonstrates whether a high-risk operation was authorized before execution
Timing and correlation UTC timestamp; start and end time or duration; request, trace and workflow IDs; parent run ID; retry count Joins agent records to IAM, cloud, application, database, endpoint and network logs
Result and state Success or failure; HTTP or tool status; error class and exception context; output summary; model and tool versions; memory or knowledge-base changes Explains what actually happened, including partial success and persistent state changes
Security metadata Source network or workload identity; destination; encryption status; redaction or tokenization performed; event-schema version Helps detect misuse and verify that the record itself is trustworthy

Record inputs and outputs at the boundary where the agent invokes a tool or model, but do not automatically retain every prompt or returned document in clear text. Classify payloads, redact secrets and personal data, and hash or tokenize content when the full value is unnecessary. Keep a content reference or digest when investigators need to prove that a particular payload was used without exposing it to every log reader.

Instrument the complete agent workflow

Telemetry should be emitted at more than one layer. A useful architecture has four observing points:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Agent gateway: authenticate the caller, assign or validate the agent identity, enforce tenant boundaries and attach a trace ID before work begins.
  • Orchestrator: record the plan or workflow ID, parent-child runs, retries, routing decisions, approvals and hand-offs to other agents.
  • Model and tool layer: record model and tool versions, prompts or input references, tool selection, arguments, returned data, policy checks and output filtering.
  • Target systems: retain native IAM, cloud, application, database, endpoint and network events so the agent record can be checked against what the target actually accepted.

Carry one trace identifier through the workflow. Add a span or child ID for each model call, tool invocation and external request, and preserve the parent-child relationship when an agent launches another agent. Include the same identifier in outbound headers or metadata where the receiving system supports it. Correlation is what turns thousands of isolated records into one explainable transaction.

Monitor the agent’s memory, retrieval store and knowledge base as stateful systems. Log additions, edits, deletions, embedding or index updates, the actor that initiated them and the policy result. The Singapore addendum specifically calls for continuous monitoring of models, databases and files, memory, agents, tools, MCP interactions, agent communication and external actions.

Centralize and protect the records

Send a copy of events to an access-controlled log-management or SIEM system outside the agent’s control plane. If an agent can delete or rewrite the only copy of its own audit trail, the record is not evidence. CISA recommends centralization, and NIST SP 800-92 provides enterprise log-management planning guidance.

Protect the pipeline in transit and the repository at rest. Apply separate roles for writing, searching, administering and exporting logs; require strong authentication and record every access to the log store. Use append-oriented or immutable storage where appropriate, integrity checks or chained hashes to detect modification, and independent monitoring for deletion, retention-policy changes, clock drift and ingestion gaps.

OWASP states: “The logging mechanisms and collected event data must be protected from mis-use such as tampering in transit, and unauthorized access, modification and deletion once stored.” Treat that protection as part of the agent’s security boundary, not as an afterthought.

Synchronize clocks to a trusted time source and store timestamps in UTC. A five-minute clock discrepancy can make a seemingly impossible sequence appear plausible or hide the ordering of an approval and a destructive call. Alert when a workload stops sending expected events, when event volume drops sharply, or when a collector reports rejected or malformed records.

Implement an auditable design step by step

  1. Inventory identities and capabilities. List every agent deployment, model and tool, its tenant, service account, delegated human or service principal, allowed scopes and target systems. Give separate deployments separate identities instead of sharing a broad credential.
  2. Define risk classes. Mark reads, writes, exports, account changes, code execution and external communications according to their impact. Require explicit human approval for the classes your policy identifies as high risk.
  3. Specify the event contract. Make identity, target, operation, parameters, data classification, decision, timing, trace IDs, result and error fields mandatory. Version the schema so parsers continue to work when fields evolve.
  4. Instrument enforcement points. Emit an event before authorization, after the policy decision, when the tool call is sent, when the target responds and when any persistent state changes. A denied call should be logged as carefully as a successful one.
  5. Join downstream evidence. Pass the trace or workflow ID to APIs and correlate it with cloud audit, IAM, database, endpoint and network records. Where a target cannot accept the ID, maintain a mapping based on request ID, workload identity and time window.
  6. Route copies to independent storage. Stream events to the central log platform over an authenticated channel, apply retention and access policies, and verify that the agent cannot alter the central copy.
  7. Build and test detections. Start with known benign and harmful workflows. Confirm that each rule fires, identifies an owner and provides enough context to decide whether to block, suspend credentials, require approval or investigate.
  8. Review the control regularly. Reconcile registered tools and scopes with observed calls, rotate credentials, test failover and restore, and sample traces to ensure payload redaction and correlation still work after model, tool or orchestration changes.

Cloud authorization should follow a documented model such as role-based or attribute-based access control. NIST SP 800-210 discusses access-control guidance across IaaS, PaaS and SaaS, including RBAC and ABAC. The model matters less than being able to show the effective decision for the specific resource at the specific time.

What to monitor and when to alert

Do not alert on every ordinary model response. Alert on combinations of identity, scope, target, volume, sequence and outcome that indicate risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Authority and policy changes

  • Privilege escalation, scope expansion, a new role assignment or use of credentials outside the approved policy.
  • An agent attempting a tool that is not registered for its deployment, or a call made after an approval expired or was revoked.
  • A human approval missing, arriving after execution, or being issued by an identity that is not authorized to approve that action.

Unusual access and impact

  • New destinations, unusual time-of-day activity, bulk reads, data-volume spikes or access to a tenant, database or file class the agent has not previously used.
  • Destructive writes, mass changes, account or security-setting modifications, code execution, outbound messages or transfers to an unapproved external service.
  • Repeated failed calls, rapid retries, loops, tool-sequence deviations and attempts to bypass a denied operation by changing parameters.

Agent-specific behavior

  • Prompt-injection indicators, instructions found in retrieved content that conflict with policy, unexpected agent-to-agent messages and unusual MCP tool interactions.
  • Memory or knowledge-base changes that are not linked to an approved workflow, or output filters that repeatedly block the same class of content.
  • Large divergence between the declared plan and the tools actually invoked, especially when a low-risk task ends with an external side effect.

Telemetry and log integrity

  • Logging stoppage, dropped or malformed events, collector backlogs, clock drift, tampering signals, deletion attempts or unauthorized log-store access.
  • Missing trace IDs, orphaned child runs, unexplained gaps between authorization and execution, or a target-system event with no corresponding agent event.

Route each high-risk alert to a named response team with a documented action: pause the run, revoke or narrow credentials, require a fresh approval, preserve related evidence and notify the system owner. CISA specifically recommends alerts for high-risk events such as failed logins and privilege escalation.

How to prove which user authorized an action

Authorization is a chain, not a username stored in one field. Preserve the initiating user’s authenticated session, the service principal or workload identity that executed the call, the agent deployment and version, the scopes granted, the policy rule evaluated, and the approval decision. Link these records with immutable request, trace and workflow identifiers.

For a disputed change, an investigator should be able to follow this sequence:

  1. Identify the user or upstream service that started the run and verify the authentication event.
  2. Show which agent deployment accepted the request and which scopes it received.
  3. Locate the policy evaluation and, where required, the named human approval before execution.
  4. Match the agent’s tool call and parameters to the target system’s accepted request and resulting state change.
  5. Check retries, child-agent calls, output filtering and errors to rule out an unrecorded alternate path.

If any link is absent, describe the evidence as incomplete rather than claiming certainty. Generic application logs may confirm that a change occurred while still failing to identify the model context, tool parameters or delegated authority that caused it.

Retention, privacy and payload detail

There is no universal retention period for AI-agent logs in the cited guidance. Set retention from legal, regulatory, contractual and incident-response requirements, then document the rationale for each event class. Keep security metadata and authorization evidence long enough to investigate incidents and satisfy applicable obligations; apply shorter periods or stronger minimization to sensitive prompts and returned records when full content is not needed.

Separate searchable metadata from restricted payload storage. Encrypt both, restrict access by job function, and record every export. Redaction should happen before events leave the agent or tool boundary, with a controlled mechanism for authorized investigators to retrieve original content when a case requires it. Test that secrets in error messages, headers, prompts and tool responses are not copied into multiple systems.

Retention policies must cover backups, replicas and exported investigations. When records expire, delete them consistently and retain a deletion event that identifies the policy and date without preserving the sensitive content itself.

Comparison axis Questions to ask
Identity fidelity Can it preserve agent, deployment, model, workload and delegated-user identities separately?
Coverage Does it capture tools, APIs, databases, files, memory, MCP interactions and agent-to-agent communication?
Correlation Can one trace join model spans, retries, approvals, cloud events and target-system request IDs?
Detection quality Can rules detect scope changes, unusual sequences, missing telemetry and data-volume anomalies without excessive noise?
Policy integration Are authorization decisions and human approvals recorded before execution, with an enforceable block or pause?
Tamper resistance Are logs copied outside the agent’s control, integrity-checked and protected from unauthorized deletion?
Retention and privacy Can you apply different retention, redaction, tokenization and access rules to metadata and payloads?
Response workflow Does an alert reach a named owner with runbook actions, evidence preservation and escalation?
Deployment and cost What integration effort, data volume, operating staffing and ongoing storage cost are required?

A platform that shows attractive model timings but omits tool arguments, memory changes or delegated authority is not an audit solution. Conversely, a central SIEM may provide strong retention and response workflows while requiring custom instrumentation for agent-specific spans.

Common implementation failures and fixes

Logging only the final API request

Why it fails: The record omits the model decision, tool parameters, approval and intermediate calls. Fix: instrument the gateway, orchestrator, model/tool layer and target system, then carry a shared trace ID.

Giving every agent one shared service account

Why it fails: Investigators cannot distinguish deployments or delegated users, and compromise grants excessive reach. Fix: assign distinct workload identities and narrowly scoped credentials; record the human or service principal that delegated authority.

Keeping the only logs beside the agent

Why it fails: A compromised runtime can erase evidence. Fix: stream an independent copy to centralized, access-controlled storage and monitor ingestion health.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Storing every prompt and document indefinitely

Why it fails: It expands privacy and breach impact without improving every investigation. Fix: classify data, redact secrets and personal data, tokenize or hash unnecessary content, and set retention by risk and obligation.

Alerting without an owner or response action

Why it fails: A notification does not stop a destructive workflow. Fix: assign each alert class to a team and define pause, credential-revocation, approval and evidence-preservation steps.

Operational checklist

  • Every deployment has a unique identity, version and documented owner.
  • Every run records the delegated human or service principal and effective scopes.
  • Every tool and API call includes target, operation, parameters, data classification, approval and trace IDs.
  • Inputs, outputs, errors, retries, memory changes and policy results are captured with safe redaction.
  • Agent, IAM, cloud, application, database, endpoint and network records can be correlated.
  • Copies are centralized outside the agent’s control plane and protected against tampering and unauthorized access.
  • Rules alert on privilege changes, unusual access, policy violations, high-impact effects and missing telemetry.
  • Retention, deletion, payload access and incident escalation are documented and tested.

Frequently Asked Questions

What should I log when an AI agent uses our APIs?

Record the agent and delegated identity, authentication method, scopes, API and resource, operation and parameters, data returned or changed, approval and policy result, UTC time, duration, trace IDs, status, errors and any persistent state change. Redact secrets and unnecessary personal data.

How do I audit agent tool calls?

Instrument the gateway, orchestrator and tool layer, assign a shared trace ID with child spans for each call, and correlate those events with the target system’s native audit record. Store an independent copy in centralized protected storage.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I prove which user authorized an agent action?

Yes, when the identity chain is preserved: the user’s authenticated session, delegated service or workload identity, agent deployment and scopes, policy evaluation, required human approval, tool request and target-system result must share linked request or trace identifiers.

What alerts catch an agent going rogue?

Prioritize privilege or scope expansion, unapproved tools, new destinations, bulk reads, destructive writes, unusual time or volume, repeated failures, sequence deviations, prompt-injection indicators, unexpected external actions and missing or tampered telemetry.

How long should AI-agent logs be retained?

No single period is established by the cited guidance. Set retention by legal, regulatory, contractual and incident-response needs, and minimize or separately restrict sensitive prompt and document content when full payloads are not required.
