What to know before trying an epilepsy drug for osteoarthritis

Gabapentin and pregabalin are not routine treatments for osteoarthritis. UK guidance recommends them as options for neuropathic (nerve) pain, a different pain category. If someone with osteoarthritis is offered one of these medicines, the important first step is to clarify what type of pain is being treated, whether another condition is present, what benefit is expected, and how the prescription will be reviewed and eventually tapered if necessary.

Do not start, stop, change, or switch a prescribed gabapentin or pregabalin regimen without speaking with the prescriber or a pharmacist. The recommendations below come from UK NICE and NHS guidance and do not diagnose an individual joint or determine whether a particular prescription is suitable.

Why an epilepsy medicine might be mentioned for joint pain

Gabapentin and pregabalin were developed as antiepileptic medicines. They are also used for some forms of neuropathic pain, which results from abnormal nerve signalling rather than only from damage or inflammation in a joint. Their use for nerve pain can create understandable confusion when a person also has osteoarthritis: a prescription may sound like an osteoarthritis treatment even when the clinician is targeting a different pain mechanism.

Osteoarthritis (OA) and neuropathic pain are clinical categories, not interchangeable labels. OA commonly involves a painful joint and reduced function, but not every symptom in a person who has OA is necessarily caused by that condition. A clinician may suspect a nerve-related component, another diagnosis, or two conditions at the same time. The medicine choice should follow that assessment rather than the medicine’s reputation as an “epilepsy drug.”

What NICE says about gabapentin, pregabalin and OA

NICE OA guideline NG226

NICE guideline NG226, published on 19 October 2022, recommends tailored therapeutic exercise for everyone with osteoarthritis. When medicines are needed, NICE says they should be used alongside non-drug treatment, at the lowest effective dose and for the shortest possible time.

For knee osteoarthritis, the exact recommendation is: “Offer a topical non-steroidal anti-inflammatory drug (NSAID) to people with knee osteoarthritis.” NICE also says to consider a topical NSAID for OA in other joints. This is a guideline-backed OA option; it is separate from treatment aimed at neuropathic pain.

NG226 does not recommend gabapentin or pregabalin as routine medicines for osteoarthritis. That absence matters: a medicine can be appropriate for one pain condition without being an established treatment for another.

NICE neuropathic-pain guideline CG173

NICE guideline CG173 recommends a choice of amitriptyline, duloxetine, gabapentin, or pregabalin as initial treatment for neuropathic pain in adults, except trigeminal neuralgia. If the first choice is ineffective or not tolerated, the guideline advises offering another option from that list.

CG173 was dated 20 November 2013 and was last updated on 22 September 2020 in the source record. Its recommendation applies to neuropathic pain; it does not turn gabapentin or pregabalin into routine OA medicines.

Chronic primary pain is a separate issue

NICE chronic-pain guidance says not to initiate antiepileptic drugs, including gabapentinoids, to manage chronic primary pain, except when gabapentinoids are offered as part of a clinical trial for complex regional pain syndrome. Where an underlying condition adequately accounts for pain, the guidance says to manage the pain in line with guidance for that condition.

This distinction is why the word “pain” alone is not enough to justify a prescription. The clinician should identify the suspected pain condition and explain how the proposed medicine fits it.

Questions to settle before taking the first dose

1. What pain is being treated?

Ask whether the target is ordinary OA-related joint pain, suspected neuropathic pain, or another diagnosis. Ask what findings led the clinician to that conclusion and whether the OA diagnosis itself has been reassessed. The cited guidance does not provide a way to diagnose yourself from symptoms, so a written label such as “nerve pain” should be explained in plain language.

2. Could more than one problem be present?

A person can have OA and a separate nerve-pain condition. They may also have symptoms that do not fit the original OA explanation. Ask whether another condition needs assessment and which symptoms would prompt a review. The aim is not to reject the prescription automatically, but to make sure the medicine is addressing the problem the clinician believes is present.

3. What outcome would count as a benefit?

Agree on a practical goal before treatment begins. That might involve a specific activity, sleep, walking tolerance, or another measure of function chosen with the clinician. A clear goal makes it easier to decide whether continuing the medicine is worthwhile instead of continuing indefinitely because it was once prescribed.

4. What are the relevant risks?

Tell the prescriber about other medicines, existing medical conditions, previous reactions, pregnancy or plans for pregnancy where relevant, and any history that could affect medicine choice. Ask which side effects require urgent advice, which are expected to settle, and what to do if they appear. The appropriate details depend on the individual prescription and cannot be safely replaced by a generic dose or internet checklist.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. How will the prescription be reviewed?

Ask when the benefit will be assessed, what evidence will be used, and what happens if the medicine is ineffective or not tolerated. A review should consider both symptom relief and day-to-day function, alongside adverse effects and the rest of the OA plan.

Evidence-based OA care that does not depend on an epilepsy medicine

Tailored therapeutic exercise

NICE recommends tailored therapeutic exercise for all people with OA. “Tailored” means the programme is adapted to the person’s joint problem, ability, and goals rather than treated as a single routine for everyone. Exercise is part of the core OA plan even when a medicine is prescribed for another type of pain.

Ask who will help design the programme, how progress will be judged, and what modification is appropriate if pain limits participation. A prescription for gabapentin or pregabalin should not displace the non-drug treatment recommended for OA.

Topical NSAID

For knee OA, NICE says to offer a topical NSAID. For OA in other joints, it says to consider one. Product suitability, labeling, contraindications, and availability vary, so a pharmacist or clinician should confirm whether a topical NSAID is appropriate for you and how it should be used.

This category-level recommendation is not an instruction for every person to buy or apply a particular product. It is the OA-specific medicine option named in NG226, in contrast to gabapentin and pregabalin’s role in neuropathic pain.

Walking aids for lower-limb OA

For lower-limb OA, NICE says to consider walking aids such as walking sticks. The decision depends on mobility, balance, the affected joint, and safe fitting. Ask a clinician or therapist to check whether an aid improves movement and function and how to use it safely.

Braces, insoles, tape and supports

NICE does not recommend routine use of braces, insoles, tape, splints, or supports. It says they may be considered when there is joint instability or abnormal biomechanical loading, exercise is ineffective or unsuitable without an aid, and the aid is likely to improve movement and function. That conditional wording is important: a support is not automatically appropriate simply because a joint hurts.

How the main options differ

Option Pain or function it addresses Position in the cited UK guidance Key discussion point
Gabapentin Neuropathic pain; also an antiepileptic medicine One initial-treatment option for neuropathic pain, not routine OA treatment Clarify why nerve pain is suspected and how benefit will be reviewed
Pregabalin Neuropathic pain; also used for epilepsy One initial-treatment option for neuropathic pain, not routine OA treatment Confirm the target diagnosis, risks, interactions, and tapering plan
Tailored therapeutic exercise OA-related function and symptoms NICE says to offer it to all people with OA Programme and goals should be individualized
Topical NSAID OA-related joint pain Offer for knee OA; consider for OA in other joints Check product suitability and labeling with a professional
Walking aid Mobility in lower-limb OA NICE says to consider walking aids such as sticks Fit and use should support safe movement and function
Brace, insole, tape, splint or support Specific instability or biomechanical problems Not for routine use; consider only under stated functional conditions Use requires a clear reason and likely functional benefit

If you already take gabapentin or pregabalin

Do not stop abruptly on your own

NICE neuropathic-pain guidance says withdrawal or switching should be tapered to account for the dose and possible discontinuation symptoms. That means a person who is already taking gabapentin or pregabalin should contact the prescriber before reducing, stopping, or changing it. The safe schedule depends on the prescribed regimen and the individual situation.

Ask for a switching plan if it is not helping

If treatment is ineffective or not tolerated, CG173 advises offering another listed option for neuropathic pain. A switch is not a reason to improvise with missed doses or overlapping medicines. Ask which medicine is being stopped, which is being started, how the transition will be staged, and who to contact if symptoms worsen.

Keep the original indication visible

Record whether the medicine was prescribed for epilepsy, neuropathic pain, or another reason. The same drug name can appear in different treatment plans, and the tapering or review requirements may differ. Bring an up-to-date medicine list to every review, including non-prescription products.

A practical appointment checklist

  1. Describe the pain precisely. Note the joint or area involved, when it occurs, what limits you, and whether the symptoms match the OA explanation you were given.
  2. Ask for the working diagnosis. Use the words “OA pain,” “neuropathic pain,” or another term only after the clinician explains what is meant in your case.
  3. Confirm the goal. Agree on a functional or symptom outcome that can be reviewed.
  4. Review your medicines and conditions. Include prescribed drugs, over-the-counter medicines, supplements, allergies, and relevant medical history.
  5. Discuss adverse effects and warning signs. Ask what to monitor, what requires prompt advice, and whom to contact outside normal hours.
  6. Set a review date. Decide when to judge benefit, tolerability, and whether the medicine remains necessary.
  7. Plan any reduction in advance. If the medicine is stopped or switched, ask how tapering will account for dose and possible discontinuation symptoms.

Common mistakes to avoid

  • Assuming the drug treats OA because it treats epilepsy. The indication for epilepsy does not establish effectiveness for OA.
  • Using “pain” as a single category. OA-related joint pain, neuropathic pain, and chronic primary pain have different guidance.
  • Replacing exercise with a prescription. NICE’s OA recommendation for tailored therapeutic exercise still applies when another medicine is used.
  • Stopping after reading that it is not an OA medicine. If it was prescribed for epilepsy or suspected nerve pain, stopping without a tapering plan can be unsafe.
  • Buying a support without a functional reason. NICE limits routine braces, insoles, tape, splints, and supports to specific circumstances.
  • Expecting a universal answer from UK guidance. Recommendations, licensing, and product labeling may differ in other countries.

How to interpret a clinician’s recommendation

A recommendation can be reasonable when the clinician has identified neuropathic pain alongside OA, explained the expected benefit, checked relevant conditions and medicines, and arranged monitoring. It deserves clarification when the only explanation is that the drug is “good for pain,” when no OA and neuropathic-pain distinction is made, or when there is no plan for review or tapering.

The most useful conversation is therefore not “Is gabapentin an OA drug?” but “What pain mechanism are we treating, what improvement should I expect, what are the risks for me, and how will we decide whether to continue?”

Scope of this advice

The recommendations summarized here are from UK NICE and NHS sources. They do not provide a diagnosis, a dose, a prediction of effectiveness for an individual, or a substitute for a prescriber’s instructions. People outside the UK should check their own national guidance and the labeling supplied with their medicine. Anyone considering a change to gabapentin or pregabalin should speak with the prescribing clinician or a pharmacist first.
