“T-cell immunity test” is not one standardized test or score. The report might show T-cell counts (such as CD3, CD4, or CD8), a laboratory stimulation and proliferation response, a CD8-specific functional assay, or a tuberculosis (TB) interferon-gamma release assay (IGRA). Each measures a different biological question.

Start with the exact test name and its laboratory reference interval. A low value can indicate an abnormality in the function or cell population measured, but it does not by itself diagnose immunodeficiency. A result inside the interval does not prove that the entire immune system is normal or that you are protected from every infection.

Why the exact test name matters

T cells are a family of lymphocytes with different jobs. Some coordinate immune responses, some kill infected cells, and others form memory after exposure. A laboratory can measure how many T cells are present, whether they respond when stimulated, or whether they react to antigens from one infection. Those are related questions, not interchangeable measurements.

Before interpreting a number, locate the assay name on the order or report. Terms such as “lymphocyte subsets,” “T-cell proliferation,” “CD8 function,” “QuantiFERON-TB,” and “T-SPOT.TB” point to different tests. Never compare a percentage from flow cytometry with a proliferation index or an interferon-gamma value as though they were the same score.

What the main T-cell tests measure

Test type What the laboratory measures What a result can and cannot tell you
T-cell subsets or counts Flow cytometry identifies populations such as CD3, CD4 and CD8, reporting absolute counts, percentages or both. Shows the size and proportion of measured populations. It does not directly show whether those cells function normally.
Lymphocyte proliferation Patient cells are exposed to mitogens (broad stimulants) or specific antigens, and the laboratory assesses their proliferative response. An abnormal response can support concern about T-cell function in context. Profound T-cell lymphopenia, specimen timing and handling can complicate interpretation.
CD8 functional competence A specialized assay measures interferon-gamma response and CD107a/b expression in CD8 T cells. A value below that laboratory’s interval is consistent with impaired CD8 function, often in the setting of excessive immunosuppression. It does not establish overall T-cell competence.
TB interferon-gamma release assay (IGRA) Blood is stimulated with TB-related antigens and the immune response is measured. QuantiFERON-TB Gold Plus (QFT-Plus) and T-SPOT.TB use different formats. Addresses likelihood of infection with TB bacteria, not general immune strength. Non-informative results need clinical follow-up.

T-cell subset counts: numbers, not performance

Flow cytometry labels cells according to surface markers. CD3 represents the broad T-cell population; CD4 and CD8 identify major subsets. Reports may include an absolute count (cells per microliter), a percentage of lymphocytes, or a ratio. Counts naturally vary with age, time of day, recent illness and other biological factors. For serial monitoring, Mayo Clinic Laboratories emphasizes collecting specimens at a consistent time because timing can affect comparisons.

A low CD4 or CD8 count means that fewer cells were measured in that sample. It does not prove that the remaining cells are defective, and a normal count does not prove that they respond normally. The laboratory’s age-appropriate interval—not a value copied from a different assay or age group—must be used.

Lymphocyte proliferation to mitogens or antigens

In a proliferation assay, living lymphocytes are stimulated in the laboratory. Mitogens provide a broad, non-specific stimulus; antigens test response to selected targets. The output reflects how strongly the cells divide or otherwise respond under the assay conditions.

Mayo Clinic Laboratories notes that abnormal mitogen or antigen responses may indicate impaired T-cell function when interpreted with the clinical picture. However, a very low number of T cells can make a response appear poor simply because fewer responding cells were available. Delays, temperature problems or other specimen-handling issues can also affect viable-cell assays. A proliferation result therefore requires clinical correlation and is not a stand-alone diagnosis of clinically significant immunodeficiency.

CD8-specific functional testing

A global CD8 functional assay evaluates selected activities of CD8 cells: interferon-gamma production and expression of the degranulation markers CD107a and CD107b. It is narrower than a test of all T cells. Mayo’s catalog states explicitly that the assay is specific only for CD8 T cells and does not provide information about overall T-cell competence.

Values below the laboratory reference interval are consistent with impaired CD8 function, often in people receiving substantial immunosuppression. Values above the interval are described as unlikely to have clinical significance. The published 95% confidence-interval reference values were derived from 102 healthy adult donors, so the interval belongs to that defined reference population and may not apply unchanged to children, older adults or people with particular illnesses.

TB IGRAs: a focused infection test

QFT-Plus and T-SPOT.TB expose blood cells to antigens associated with TB bacteria and measure the resulting interferon-gamma response. They answer a specific question: is the immune response compatible with TB infection? They do not measure total T-cell numbers, broad cellular immunity or protection from unrelated infections.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

QFT-Plus reports positive, negative or indeterminate. T-SPOT.TB reports positive, borderline, negative or invalid. The Centers for Disease Control and Prevention (CDC) states that a borderline, indeterminate or invalid result did not provide useful information about the likelihood of TB infection. The result should be discussed with the ordering clinician together with exposure risk, symptoms and other clinical findings.

How to read a result without overinterpreting it

1. Identify the measured target

Write down the exact assay name, specimen type and measured output. “CD8 percentage,” “mitogen stimulation,” and “TB antigen minus nil” are not interchangeable labels. If the report contains several panels, determine which line prompted the question.

2. Use that laboratory’s reference interval

Look for the stated interval, units and any age- or sex-specific notes. “Low” means below that laboratory’s interval under its validated conditions. A result from another laboratory, a general internet range or a different assay platform may not be comparable.

3. Ask what the result is intended to answer

  • A subset count asks how many cells of a type were present.
  • A proliferation assay asks how cells responded to a laboratory stimulus.
  • A CD8 functional assay asks about selected CD8 activities.
  • An IGRA asks about immune recognition of TB antigens.

4. Check factors that can change the measurement

Review medicines, especially immunosuppressive treatment; recent or ongoing illness; underlying conditions; and whether the sample was collected, transported and processed as required. For repeat lymphocyte-subset monitoring, use a consistent collection time when the laboratory recommends it. For proliferation testing, specimen viability and timing deserve particular attention.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Decide whether confirmation is needed

An unexpected result may need repetition, comparison with earlier results or evaluation alongside other immune tests. Mayo Clinic Laboratories cautions that no single laboratory test can identify or define impaired cellular immunity on its own. The clinician who ordered the test can decide whether the finding is persistent and clinically meaningful.

What a low result can mean

The word “low” has meaning only in relation to the assay. A low subset count indicates fewer measured cells in that sample. A low proliferation response suggests a weak response under the specific stimulation conditions, but profound lymphopenia or a compromised specimen can contribute. A low CD8 functional value is consistent with impaired CD8 activity, not proof that every arm of cellular immunity is impaired.

Medication effects, an underlying disease, temporary illness, biological variation and technical factors can all contribute. The appropriate next step is to determine whether the result fits the person’s history and whether it persists, not to label the person “immunodeficient” from one number.

What a normal or in-range result means

In-range values are reassuring only for the measurement performed under the assay’s conditions. A normal CD3, CD4 or CD8 count does not establish normal cell function. A normal proliferation response to one stimulant does not test every antigen or immune pathway. A normal CD8 assay does not evaluate non-CD8 T cells, antibodies or innate defenses.

Mayo notes that further studies are needed to connect in-range values from its CD8 assay with differences in infection risk. More broadly, no single laboratory result defines whether a person has clinically adequate cellular immunity. Clinical history, examination and, when appropriate, multiple complementary tests remain important.

How to interpret an IGRA result

Positive

A positive QFT-Plus or T-SPOT.TB indicates an immune response compatible with TB infection. It does not by itself distinguish latent infection from active TB disease. The ordering clinician must evaluate the person for TB disease and determine appropriate management.

Negative

A negative result lowers the likelihood of TB infection under the test’s conditions, but it must be considered with exposure risk and the clinical situation. A negative result is not a general statement that T-cell immunity is normal.

Borderline, indeterminate or invalid

These categories are non-informative rather than reassuring or positive. CDC guidance says they do not provide useful information about the likelihood of TB infection. The clinician may consider the person’s risk, symptoms, controls reported by the assay and whether repeat testing or another evaluation is appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Numerical values

IGRA reports can include antigen responses and control responses. CDC states that specific guidance is not available for interpreting quantitative IGRA values on their own. Do not apply a universal cutoff to a number without using the assay’s reported interpretation and the clinical context.

Questions to ask the ordering clinician

  • What exact cells, response or infection target did this test measure?
  • Is the result outside the laboratory’s age-appropriate reference interval?
  • Could my medication, illness, specimen handling or collection timing have affected it?
  • Does the result need confirmation or comparison with an earlier sample?
  • Which additional findings would change the interpretation?
  • If this is an IGRA, how do my TB exposure risk and clinical findings affect the result?

Common interpretation mistakes

  • Calling every test a “T-cell immunity score.” Counts, functional assays and IGRAs measure different constructs.
  • Treating a low value as a diagnosis. A low result supports a question for clinical evaluation; it does not independently establish broad immunodeficiency.
  • Treating a normal result as total protection. An in-range result covers only the tested population or function.
  • Using numbers across laboratories. Units, platforms, reference populations and validation methods differ.
  • Reading an IGRA as a general immune test. It is focused on TB-antigen response.
  • Ignoring specimen conditions. Delayed processing or inconsistent timing can affect cellular assays.
  • Interpreting an indeterminate, borderline or invalid IGRA as negative. CDC classifies these as results that did not provide useful information about TB infection likelihood.

Why these tests require professional laboratory handling

Subset testing uses flow cytometry, while proliferation and functional assays depend on viable cells and controlled stimulation. TB IGRAs require the correct collection tubes, controls and processing steps. These are laboratory procedures with assay-specific instructions and interpretation; an over-the-counter supplement, home “immune score” or generic laboratory supply cannot substitute for the ordered test or its clinical interpretation.

A practical workflow from report to next step

  1. Copy the exact test name. Include the laboratory, specimen date, units and reported reference interval.
  2. Classify the purpose. Mark it as a count, proliferation test, CD8-specific function test or TB IGRA.
  3. Review context. Note medicines, recent illness, immune conditions, exposure history and whether the collection followed laboratory instructions.
  4. Discuss unexpected findings. Ask whether the result should be repeated or paired with other tests rather than acting on the number alone.
  5. For TB results, follow the category-specific plan. Positive results require medical evaluation for TB disease; non-informative categories require discussion of risk and possible next testing.

The most reliable interpretation is therefore specific rather than global: identify what was measured, use that assay’s interval and intended purpose, account for timing and specimen quality, and interpret the finding with the clinician who knows the patient’s history.
