Recovery after laparoscopic endometrioma surgery usually includes abdominal soreness, bloating, tiredness and, for some people, temporary shoulder or upper-back pain caused by the gas used during laparoscopy. You may be sleepy or nauseated after anesthesia. Most people begin with rest and short, gentle walks, then increase activity only according to the operating team’s written instructions. There is no universal return-to-work or exercise date: recovery depends on whether the surgeon drained or removed the cyst, how much endometriosis was treated, whether other procedures were performed, and whether complications occurred.
This guide explains what is commonly expected, how to care for yourself at home, which activities require clearance, and which symptoms need prompt or emergency assessment. Your discharge sheet and surgeon’s advice take priority over any general timetable.

Why recovery differs after an endometrioma operation
“Laparoscopic endometrioma surgery” can describe operations of very different complexity. A limited procedure may involve treating one ovarian cyst, while a more extensive operation may include ovarian cystectomy (excision of the cyst), treatment of endometriosis elsewhere in the pelvis, adhesiolysis, or additional procedures discovered to be necessary during surgery. The size and location of the cyst, ovarian-sparing decisions, blood loss, anesthesia, and your general health all affect discharge and activity instructions.

National Institute for Health and Care Excellence (NICE) guidance describes ovarian cystectomy as surgical excision of an ovarian endometriotic cyst and recommends discussing benefits, risks, effects on symptoms and the possibility of future surgery. The European Society of Human Reproduction and Embryology (ESHRE) 2022 guideline recommends cystectomy rather than drainage and coagulation when surgery is undertaken for an ovarian endometrioma, citing lower recurrence and less endometriosis-associated pain. Those recommendations concern treatment choice before or during surgery; they do not create a personal recovery schedule.

The first hours after surgery
What you may feel in recovery
- Abdominal pain or tenderness: Small incisions and internal surgical work can make your abdomen sore, tight or bloated.
- Shoulder or upper-back pain: Carbon dioxide used to expand the abdomen can irritate the diaphragm and produce referred pain in the shoulder or upper back.
- Drowsiness or nausea: Anesthetic medicines commonly cause sleepiness, queasiness or light-headedness for several hours.
- Fatigue: The operation, anesthesia and your body’s healing response can leave you unusually tired.
- Vaginal spotting: Some people have light bleeding or spotting, depending on the operation and instruments used. Heavy or unusual bleeding is not something to ignore.
University College London Hospitals (UCLH) says movement and prescribed pain medicines can help gas-related shoulder discomfort, which generally settles within a few days. Johns Hopkins describes upper-back or shoulder pain as usually resolving within a day or two. These descriptions differ slightly, so treat them as typical ranges rather than a promise.
Going home
Whether you go home the same day or stay longer depends on the operation, your pain and nausea control, your ability to drink and urinate, and the surgeon’s assessment. Arrange an adult to take you home and, if advised, stay with you initially. Do not drive yourself after anesthesia or while taking sedating pain medicine.
Days two through seven: rest, movement and basic care
Use rest without becoming completely immobile
Rest is appropriate early on, but gentle movement helps you regain strength and reduces the effects of prolonged bed rest. Johns Hopkins advises planning to rest for about a week, arranging help with chores, and walking as able. Begin with brief, easy walks around your home; stop before pain or dizziness becomes significant. Increase distance gradually rather than trying to “make up” for inactivity in one session.

Johns Hopkins specifically advises avoiding lifting more than 10 pounds and avoiding strenuous housework until your clinician says it is safe. That limit is source-specific, not a universal rule. Your own discharge instructions may be more or less restrictive, particularly after extensive endometriosis treatment or additional procedures.
Managing pain and gas discomfort
Take prescribed or recommended medicines exactly as directed. Ask the surgical team or pharmacist before adding an over-the-counter medicine, especially if you have kidney disease, stomach-ulcer risk, take blood thinners, or were given a medication containing acetaminophen (paracetamol). Gentle walking may ease gas-related shoulder pain. Contact the team if pain is uncontrolled, steadily increasing rather than improving, or accompanied by fever, vomiting or increasing abdominal swelling.
Incision care
Follow the dressing and shower instructions on your discharge sheet. The Johns Hopkins discharge guidance says to wash the incision with mild soap and water, pat it dry, and avoid putting oils, powders or lotions on it. Do not pick at glue, strips or scabs unless the surgical team tells you to remove them. Watch each incision for spreading redness, increasing warmth, swelling, pus-like drainage or separation.

Preventing constipation
Anesthesia, opioid pain medicines, reduced activity and dehydration can slow your bowel. Unless your clinician has restricted fluids or fiber, drink regularly and eat foods you tolerate. Walking can help. Johns Hopkins advises preventing constipation and says stool softeners or laxatives should be used only with clinician approval. Severe abdominal pain, repeated vomiting or inability to pass stool or gas needs medical advice rather than self-treatment.

How long does recovery take?
There is no single guaranteed timetable for laparoscopic endometrioma surgery. Endometriosis UK states that weakness and tiredness for about 10 days after a laparoscopy is normal, and that fatigue may last longer when surgical treatment has been performed. This is general patient guidance, not a prediction for a particular cystectomy or complex endometriosis operation.
A straightforward laparoscopy may allow an earlier return to routine than extensive surgery, but the apparent size of the skin incisions does not reveal how much work was done internally. A longer hospital stay, treatment of bowel or bladder endometriosis, significant adhesiolysis, complications, or an unexpected second procedure can lengthen recovery. Do not use another patient’s timeline as a target.

Returning to everyday activities
Use symptoms and the surgeon’s restrictions together. An activity that feels easy one day can cause a setback if increased too quickly. Ask at follow-up for explicit clearance rather than assuming that a small incision means every activity is safe.
| Activity | What the evidence supports | How to decide safely |
|---|---|---|
| Walking | Johns Hopkins advises walking as able. | Start with short, gentle walks and stop for significant pain, dizziness or breathlessness. |
| Work | No universal return-to-work date is established; Endometriosis UK notes fatigue for about 10 days and longer after treatment. | Base timing on the operation, commute, ability to sit or stand, lifting demands and your surgeon’s written advice. Discuss phased return if needed. |
| Exercise | Strenuous activity should wait for clinician clearance; the cited guidance does not provide one fixed date. | Increase activity in stages, beginning with ordinary walking. Ask when running, abdominal exercises, cycling, swimming or weight training are permitted. |
| Lifting and housework | Johns Hopkins uses a 10-pound lifting limit and advises avoiding strenuous housework until cleared. | Treat this as that service’s instruction, not a universal limit. Follow your own discharge sheet. |
| Driving | The cited sources do not establish a fixed number of days. | Wait until you are off sedating medicines, can sit comfortably, operate controls and perform an emergency stop without pain, and your clinician or insurer’s requirements are met. |
| Sex, tampons and douching | Johns Hopkins advises no sex and no tampons or douches until the doctor says it is safe. | Obtain direct clearance; do not substitute an internet day-count. |
When symptoms are normal and when they are not
Usually expected during uncomplicated healing
- Mild to moderate abdominal soreness that gradually improves.
- Temporary bloating or gas-related shoulder discomfort that eases over several days.
- Tiredness, reduced stamina and a need for daytime rest.
- Mild incision tenderness or itching as skin heals.
- Reduced appetite or mild nausea that improves as anesthesia wears off.
“Expected” does not mean you must tolerate severe symptoms. If you are unsure whether a symptom is within your surgeon’s expected range, use the postoperative contact number supplied at discharge.
Call the surgical team promptly
- Fever or chills.
- Increasing or uncontrolled abdominal pain.
- Worsening abdominal swelling.
- Unusual, heavy or persistent bleeding.
- Redness, increasing swelling, warmth or drainage from an incision.
- Fainting, persistent nausea or vomiting.
- Difficulty or inability to pass urine.
Describe when the symptom began, whether it is worsening, your temperature if measured, the medicines you have taken and any change in the wound. Follow the local contact pathway on your discharge paperwork rather than waiting for a routine appointment.

Seek emergency help
UCLH advises emergency assessment for difficulty breathing, chest pain, or swelling, redness or tenderness in the lower legs. Severe breathlessness, sudden chest pain, collapse, heavy bleeding or rapidly worsening pain should be treated as an emergency according to local emergency instructions. Do not drive yourself if you may be seriously unwell.
Follow-up and longer-term expectations
Your follow-up appointment is the place to review pathology, wound healing, persistent pain, menstrual symptoms, fertility goals and whether additional treatment is needed. Ask what was found and treated, whether the ovarian cyst was excised, and whether any endometriosis remained untreated because of safety or fertility considerations.
Recovery from the operation is separate from the long-term course of endometriosis. UCLH reports that symptoms return in two to four out of every 10 women within five years of surgery. This is a population statement about symptom recurrence, not a measure of immediate healing and not an individual prediction. Ongoing or returning pain should be discussed with a gynecology team; it does not automatically mean the operation failed.

A practical discharge checklist
- Know which number to call during office hours and overnight.
- Confirm which medicines to take, maximum doses and which medicines to avoid.
- Ask when dressings, glue or strips should be left in place or removed.
- Confirm showering, bathing and swimming restrictions.
- Write down the lifting limit and when housework or exercise can resume.
- Ask when driving, work and sex are likely to be reconsidered.
- Arrange help with meals, shopping, children, pets and chores during the first several days.
- Check how constipation should be prevented and when a stool softener is appropriate.
- Keep your follow-up appointment and bring a list of symptoms and medicines.
Questions to ask before leaving hospital
- Exactly what procedures were performed in addition to treating the endometrioma?
- What symptoms should prompt a same-day call, and where should I go after hours?
- What are my specific limits for lifting, stairs, exercise, bathing, driving, work and sex?
- When should I expect pathology results and follow-up?
- Which pain medicines can be combined safely, and what is the constipation plan?
- What wound changes are expected, and what drainage or redness is abnormal?
The safest recovery plan is individualized: rest initially, walk gently as tolerated, protect the incisions, take medicines as directed, and progress only when your operating team says the extent of your surgery makes it safe.
