A clinician places thin leads in the epidural space near the spinal cord and connects them to a pulse generator under the skin. The system delivers electrical signals intended to change how pain is processed; it does not repair the original injury or guarantee pain relief.
The usual pathway is specialist assessment, education and often a temporary trial, followed—only if the patient and team judge the trial worthwhile—by a permanent implant. Expect separate decisions about medicines, anesthesia, wound care, programming, activity restrictions and follow-up. Exact instructions differ by hospital, procedure and device, so your surgical team’s written plan takes priority over any general timeline.

What the treatment involves
SCS is considered for selected people with persistent pain after a specialist assessment. The implant normally has three functional parts:
- Leads: insulated wires positioned in the epidural space alongside the spinal cord.
- Pulse generator: a battery-powered unit placed beneath the skin, often in the buttock or abdomen.
- Controller: a handheld programmer used to turn therapy on or off and, when permitted, adjust settings.
The device is not a universal treatment for every pain condition. Suitability depends on your diagnosis, previous treatments, physical health, medication plan, ability to manage the equipment and realistic expectations. The team should explain what improvement they are targeting—such as less pain, better sleep, walking farther or reducing rescue medication—rather than promising complete pain elimination.

Before an implant: assessment and preparation
Specialist and multidisciplinary review
Expect a consultation with a pain specialist, surgeon or neuromodulation team. Some services use a multidisciplinary review that may include pain medicine, surgery, psychology, physiotherapy or nursing. They may examine your strength, sensation, reflexes and pain pattern, review scans and previous treatments, and discuss whether an implant is safer and more useful than other options.
Preparation can include blood tests, imaging, an anesthesia assessment and other preoperative checks. Bring a complete list of prescription medicines, over-the-counter products, vitamins and supplements, plus allergies and relevant conditions such as diabetes, immune problems, bleeding disorders or previous infections.
Medication instructions
Do not stop an anticoagulant, antiplatelet drug or any other medicine on your own. The Royal National Orthopaedic Hospital (RNOH) patient guidance says blood-thinning medicines must be stopped only after discussion with the clinician and responsible prescriber. Your surgical and prescribing teams should give the exact drug, dose and timing instructions, including when to restart treatment.

Arrange transport if sedation or anesthesia is planned; you may not be allowed to drive afterward. Ask whether you need an adult with you overnight, how dressings should be protected, and which telephone number handles wound or device questions. Wear loose clothing that will not rub the generator site, and plan help with shopping, childcare, pets and lifting while movement is restricted.
Why a temporary trial is often used
Effectiveness varies from person to person, so many centers test stimulation before implanting a permanent generator. Cambridge University Hospitals describes the reason plainly: “A trial of spinal cord stimulation is carried out before considering a permanent implant because the effectiveness of spinal cord stimulation varies from patient to patient.”

In a staged trial, the clinician places one or more leads near the spinal cord and connects them to an external generator. You use a controller at home and record pain, sleep, movement and daily function. The leads usually exit through a small dressing, so you receive specific instructions about keeping the area dry and avoiding accidental pulling.
How long the trial lasts
There is no single worldwide duration. Guy’s and St Thomas’ NHS Foundation Trust describes a usual trial of about 14 days, while another NHS patient source describes roughly one to two weeks. These are examples of local practice, not a rule that applies to every device or patient.

What counts as success
Cleveland Clinic patient information describes a usual threshold of at least a 50% reduction in pain. That figure is a decision convention used by that service, not a guaranteed outcome rate for everyone receiving SCS. Your team may also require meaningful functional change—for example, being able to stand longer, sleep better or reduce medication—because a numerical pain score alone may not reflect whether the treatment is worthwhile.
Single-stage and on-table approaches
Some services, including Cambridge University Hospitals, describe an on-table test and selected single-stage pathways in which a permanent system may be placed without a separate home trial. Eligibility and local protocol determine whether this is offered. Ask which pathway is planned and what evidence the team will use to decide.
What happens on the procedure day
Trial procedure
You are positioned so the clinician can access the spine. After skin preparation and local anesthesia, with sedation or another anesthetic plan as appropriate, the lead is guided into the epidural space. The lead is connected to an external generator and the team may test stimulation while you provide feedback. A dressing protects the exit site.

Permanent implantation
For a permanent system, the lead or leads are positioned and then tunneled under the skin to the generator pocket. The generator is commonly placed in the buttock or abdomen, depending on your body, comfort, lead path and device design. The system is checked and may be programmed before you leave or at a later clinic visit.

Ask in advance whether you will be awake enough to give feedback, what type of anesthesia is planned, where the generator will sit, and whether the operation is expected to be a day case or require an overnight stay. The answer varies with the procedure and your health.
Leaving hospital: checks and instructions
Before discharge, staff may check your pain, ability to eat and drink, urination, wound dressing and neurologic status. They should demonstrate the controller, explain charging if your system requires it, and provide written contact details. Confirm whether stimulation is active immediately or will be switched on after the incision begins to heal.
Do not leave without knowing:
- How to take prescribed pain medicines and which regular medicines to restart.
- How to keep the dressing clean and dry and when it will be changed.
- When the first wound and programming appointment is scheduled.
- Which symptoms require a same-day call and which require emergency assessment.
- What to do if stimulation suddenly stops, becomes painful or feels different.
Recovery: a practical timeline
| Period | What you may notice | Typical priorities |
|---|---|---|
| First 24 hours | Soreness at the incision or generator pocket, bruising, fatigue and restricted movement. | Rest, take medicines as directed, protect dressings and arrange supervision if instructed. |
| First one to two weeks | Incisions continue healing; trial leads, if present, remain vulnerable to traction. | Keep appointments, log pain and function, and follow your center’s bathing and movement rules. |
| Up to six weeks | The pocket and lead path gradually settle. | Increase activity only as cleared; avoid lifting, bending, twisting or stretching beyond your written limits. |
| Later follow-up | Settings may be refined as activity and pain change. | Report loss of benefit, uncomfortable stimulation or wound concerns rather than changing settings repeatedly on your own. |
These periods are a framework, not a universal prescription. Your operation, lead position, wound condition and hospital protocol control the actual timeline.
Movement, bathing, driving and work restrictions
Restrictions differ substantially between centers, which is why combining instructions from different leaflets can be unsafe. One Cambridge University Hospitals leaflet advises avoiding arm elevation, twisting, stretching, bending and lifting more than 2.5 kg for six weeks after an implant. RNOH guidance advises avoiding sudden, jerky spinal movements during a trial and for the first six months after implantation, and gives center-specific bathing advice for trial leads.

Before going home, ask for a written answer to each activity question:
- When may I shower, take a bath or swim?
- When may I drive, and do I need medical or insurance clearance?
- What weight may I lift, and when can I resume exercise or physiotherapy?
- When can I return to work, and are modified duties needed?
- Which movements could pull or shift the leads?
Do not assume that a small incision means the internal leads are ready for unrestricted activity. Early lead movement can reduce coverage or require revision.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Programming and judging whether it helps
Pain relief may not be immediate or may require several programming sessions. Keep a simple record of pain intensity, sleep, walking or sitting tolerance, medication use and unwanted sensations. Compare ordinary activities with your pre-trial baseline, not just a single good or bad hour.
Stimulation should feel acceptable. Depending on the system, you might feel tingling, pulsing or no sensation at all. Contact the device team if stimulation becomes painful, spreads unexpectedly, interferes with movement or no longer covers the painful area. Do not open the incision or attempt to manipulate the hardware.
Risks and warning signs
Patient materials list several possible complications:
Best Value
- Infection, wound separation, redness or drainage.
- Bleeding, bruising or a collection near the lead.
- Lead migration, fracture, disconnection or electrical failure.
- Insufficient pain relief or loss of benefit after an initial response.
- Unpleasant stimulation or pain over the generator pocket.
- Allergic reaction to materials or medicines.
- Rare nerve or spinal-cord injury.
Cambridge aftercare information warns that pus or a blood clot near an electrode can place pressure on the spinal cord. RNOH material gives a local estimate of “2 patients per million” for paralysis. That number is from a hospital leaflet and should not be treated as a pooled risk estimate for every center, device or patient.
When to call promptly
Contact your implant team promptly for fever, increasing wound pain, spreading redness, drainage, a new lump, loss or sudden change of stimulation, or a dressing that becomes soaked. Follow the discharge number even if symptoms seem minor.
When to seek emergency help
Severe or rapidly worsening weakness, new numbness, loss of bladder or bowel control, severe back pain with neurologic change, collapse, breathing difficulty or other alarming symptoms need urgent assessment through your local emergency pathway. Tell clinicians that you have an SCS system and show your implant identification information if available.
Troubleshooting common problems
The device suddenly stops helping
- Check whether the controller is on and the battery is charged.
- Confirm that you have not accidentally selected a different program.
- Stop strenuous activity and note when the change began.
- Call the implant team if benefit does not return or if the change follows a fall, twist or impact.
The wound looks different
Do not apply creams, remove glue or re-dress the incision unless instructed. Photographing a change can help the clinical team, but a photo does not replace an examination when redness, drainage, fever or rapidly increasing pain is present.
The trial lead dressing loosens
Keep the area as still and dry as your instructions require and call the trial team immediately. Do not push a lead back in or attempt to secure an exposed wire yourself.
Imaging and future medical care
Carry your implant identification details and tell every clinician, dentist and imaging department that you have an SCS system. MRI and other imaging rules are device-specific: some systems permit particular scans under defined conditions, while others impose restrictions. The imaging department must verify the exact implanted components and follow the manufacturer and hospital protocol; never assume that a scan is safe because someone else with an implant had one.
Keep follow-up appointments even when pain is improved. The team may adjust programs, monitor the generator pocket and discuss battery or hardware management over time.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteQuestions to take to your clinical team
- Is my plan a temporary trial, an on-table test or a permanent implant in one stage?
- What specific change in pain or daily function would count as success for me?
- Which medicines should I take or pause, and which prescriber coordinates that decision?
- Where will the generator be placed, and what clothing or sitting positions may irritate it?
- When will therapy be activated or programmed?
- What are my exact limits for bathing, driving, lifting, bending, exercise and work?
- What device-specific rules apply to MRI or other imaging?
- Who do I call about a wound, a changed stimulation pattern or a dead controller?
- Which symptoms require a same-day call, and which require emergency care?
What the evidence can—and cannot—tell you
Hospital patient leaflets are useful for explaining pathways and local aftercare, but their timings and restrictions are not universal. The available patient information does not establish one cross-center percentage for pain relief or complications, and the 50% trial threshold is not an outcome statistic. Your expected benefit and risk should therefore be discussed using your diagnosis, previous treatment response, health conditions and the exact system proposed.
