After a hair transplant, the scalp heals over days to weeks, transplanted hair shafts often shed in the first several weeks, and visible regrowth takes months. Many patients see results in 6–9 months, though some take 12 months; your surgeon’s aftercare instructions take priority because timing varies by procedure and person.

What happens in the first days and weeks?
First days: healing, swelling and tenderness
The scalp is recovering from surgery. ISAPS lists temporary swelling, bruising, pain or tenderness, and numbness among possible effects. It says swelling often reaches its peak around days 3–5. Its general guidance says gentle soaking and shampooing may usually begin after 48 hours, but your surgeon’s specific instructions should govern when and how you wash.

First week or two: small scabs
Small scabs can form around the grafts and, according to ISAPS, fall off within about a week. Healing and timing vary. Avoid picking or rubbing the area, and do not adopt a washing routine that conflicts with the clinic’s directions.
Why transplanted hair sheds before it grows
It is common for transplanted hair shafts to shed in the first weeks. AAD gives a broad window of 2–8 weeks, while ISAPS gives 2–4 weeks. These overlapping ranges describe typical guidance, not a precise date for an individual patient. The early loss of visible shafts can be discouraging, but it is part of the expected process described by both organizations; it does not, by itself, tell you what the final result will look like.

Do not assume that every shed hair or every graft is guaranteed to regrow. If the pattern or timing worries you, contact your surgeon rather than trying to judge the outcome from shedding alone.
When does new growth appear, and when can you judge results?
Visible growth is delayed. ISAPS places the start of regrowth at around 3 months. ASPS describes a further 5–6 weeks before growth resumes after the shedding phase. These milestones are expressed differently, so they should not be treated as a single exact schedule for every patient.

Recovery is not one fixed calendar. ASPS says the return to routine depends on the operation’s length, complexity and type. The milestone also matters when comparing timelines: skin healing, hair-shaft shedding, the first new growth and a mature cosmetic result are different stages, not competing estimates of one event.

Does a transplant stop further hair loss?
No. Hair loss can continue in untreated or native hair after surgery. AAD notes that a dermatologist may recommend medication to slow or prevent further loss. Whether ongoing treatment is appropriate depends on the individual; discuss it with your dermatologist or surgeon rather than starting or changing medication on your own.
Aftercare and follow-up
Use the instructions from the team that performed your procedure for washing, activity and care of the graft area. ISAPS says gentle shampooing may usually begin after 48 hours, but that general guidance does not replace your clinic’s directions. ASPS says the surgeon may schedule visits during the first month to check healing; attend the appointments arranged for you and contact the clinic if you are uncertain about a symptom or instruction.
