What to Eat While Taking Antibiotics to Support Gut Health

There is no single food proven to restore gut health during antibiotics. Follow the food instructions for your specific medicine first: some antibiotics should be taken with food, while others need an empty stomach. For general support, choose a varied diet you can tolerate, and treat probiotic foods or supplements as optional—not as a guaranteed way to prevent diarrhea or rebuild the microbiome.

Start with your antibiotic’s food instructions

Antibiotics do not all have the same food rules. The NHS notes that some need to be taken with food and others on an empty stomach. Read the current patient information leaflet for your medicine; if it is unclear, ask a pharmacist or prescriber whether food, dairy, or other products affect how you should take it. The NHS guidance is on its Interactions – Antibiotics page.

Do not skip, stop, share, save, or change a prescribed antibiotic because of a food choice. The CDC advises taking antibiotics exactly as prescribed and contacting a healthcare professional about side effects.

What foods make sense during a course?

There is no established “gut-restoring” menu for an antibiotic course. A practical approach is to eat a varied diet that you tolerate, while following any medication-specific instructions. You do not need to add a special food to make the antibiotic work or to replace its effects.

Choose foods you can comfortably eat

If your appetite or digestion is unsettled, keep meals simple and drink fluids as needed. These are comfort measures, not proven ways to prevent antibiotic-associated diarrhea. If dairy does not agree with you, there is no need to eat yogurt for gut health.

Yogurt and fermented foods are optional

Yogurt and other fermented foods can contain live microorganisms, but that fact alone does not show that a particular grocery product prevents diarrhea during antibiotics. Studies of probiotics have tested particular preparations and populations; their findings cannot automatically be applied to ordinary yogurt, kefir, or every fermented food.

What the evidence says about probiotics

Antibiotic-associated diarrhea is a recognized side effect. The NIH Office of Dietary Supplements (ODS) says that up to 30% of antibiotic users experience it, a literature-based estimate that does not apply uniformly to every medicine or person. Some probiotic preparations may reduce the risk for certain groups, but results depend on the formulation, strain, timing, age, baseline risk, and care setting.

Results are promising in some studies, not a personal guarantee

NCCIH describes a 2017 review of 17 studies involving 3,631 non-hospitalized participants: people taking probiotics with antibiotics had about half the likelihood of antibiotic-associated diarrhea. NCCIH characterizes this finding as tentative because study quality was moderate. It is a relative association, not a promise that an individual will avoid diarrhea.

A 2021 systematic review and meta-analysis in BMJ Open Gastroenterology combined 42 randomized trials involving 11,305 adults and estimated a 37% lower pooled risk (RR 0.63; 95% CI 0.54–0.73), with moderate GRADE certainty. The authors cautioned that the result may not apply where baseline risk is low. When studies at high or unclear risk of bias were removed in a sensitivity analysis, the estimate was no longer statistically significant.

Timing and product matter

ODS reports that some studied probiotic preparations, including those using Lactobacillus rhamnosus GG (LGG) or Saccharomyces boulardii, may reduce antibiotic-associated diarrhea in particular contexts. Evidence suggests a benefit may be more relevant when certain products are started within two days of the first antibiotic dose. This does not establish a universal product, dose, or schedule, or show that any yogurt has the same effect.

NCCIH also summarizes a 2017 analysis of 31 studies and 8,672 patients that found moderate-certainty evidence probiotics can reduce the risk of C. difficile-associated diarrhea among adults and children receiving antibiotics. Most included studies involved hospital patients; whether probiotics help people who are very weak or immunocompromised is uncertain. NCCIH says effective probiotic types, duration, and doses remain unclear in several contexts.

When to ask before using a probiotic supplement

Probiotic supplements are optional. Ask a healthcare provider or pharmacist before taking one if you have significant health problems, a weakened immune system, are seriously ill, or take other medicines. NCCIH advises particular caution and close monitoring for people with serious underlying conditions; it also reports severe or fatal infections in premature infants given probiotics. Do not give a probiotic supplement to a premature infant based on general food advice.

If you are considering a supplement, discuss the specific product with a clinician or pharmacist rather than assuming that a label claim, strain name, or dose applies to your situation. The evidence does not identify one best probiotic for everyone.

Know when diarrhea needs medical advice

Diarrhea can occur while taking antibiotics, and some cases may be serious, including C. difficile infection. Symptoms alone cannot establish the cause. Contact a healthcare professional about side effects, especially if diarrhea is severe, persistent, or otherwise concerning. The CDC’s Healthy Habits: Antibiotic Do’s and Don’ts advises taking antibiotics exactly as prescribed and seeking professional advice about side effects.
