Do not accept whichever result sounds more certain, and do not treat either a pathology classifier or an MRI classifier as a diagnosis on its own. Ask the treating team to reconcile the original images, tissue findings, reports, clinical history and each tool’s intended use. They should first confirm that both outputs concern the same lesion and adequate inputs, then document the discordance and decide whether specialist review, additional testing, repeat sampling or another step is appropriate.
First, identify what is actually disagreeing
“Pathology classifier” and “MRI classifier” can describe several different situations:
As an Amazon Associate I earn from qualifying purchases.
- An artificial-intelligence output from digitized pathology slides versus an artificial-intelligence output from MRI images.
- A radiologist’s interpretation of MRI versus a pathologist’s diagnosis from a biopsy.
- An AI result that conflicts with a clinician’s reading of the same study.
- Two tools that use different labels, probability scales or clinical targets.
These are not interchangeable disputes. A tool may be designed to flag a feature, assign a category or assist a professional, rather than make a final diagnosis. Before anyone interprets the mismatch, write down the exact wording, category and date of each result. The College of American Pathologists (CAP) advises understanding an AI system’s intended use and stated output; an output should not be stretched beyond that scope.
The reconciliation process, step by step
1. Ask the treating team to review the original evidence
Request review by the clinicians responsible for the case, not a decision based only on screenshots or an automated score. They should examine the original MRI images and sequences, the complete pathology material and report, prior studies, symptoms, examination findings and relevant laboratory or treatment information. The question is whether the tissue finding plausibly explains the imaging observation in this patient.

Reports can compress important details. A category in an MRI report may reflect probability or management risk, while a pathology label may describe only the fragment sampled. A disagreement therefore requires comparison of the underlying evidence, not a contest between numbers.
2. Confirm that both outputs refer to the same lesion
Patient identity, anatomic site and laterality should be checked against the imaging and specimen labels. The team should confirm that the biopsy needle or surgical specimen sampled the imaging target rather than an adjacent abnormality. Localization markers, procedure notes and post-biopsy imaging can help establish this.
Timing matters as well. A lesion can change between MRI and tissue acquisition, particularly when the interval is long. The American College of Radiology (ACR) advises identifying the pathology acquisition date when adjudicating discordant liver observations. The team should record which study came first, how much time elapsed and whether treatment or another procedure occurred between them.

3. Check image and specimen adequacy
For MRI, review whether the relevant sequences were obtained, whether motion or other artifacts limit interpretation, and whether contrast timing and image quality are sufficient for the clinical question. For pathology, confirm that the specimen is correctly identified, contains viable diagnostic tissue and is not too small, crushed, necrotic or otherwise unrepresentative.

A classifier can produce a confident-looking result from poor or incomplete input. Confidence is not proof that the input was appropriate. If an algorithm’s documented requirements do not match the actual images or slides, its output may be outside validated use.
4. Perform an explicit imaging–pathology concordance assessment
Concordance asks whether the pathology diagnosis reasonably accounts for the imaging appearance and level of concern. A benign result can be concordant when it explains the finding and sampling was demonstrably accurate. The same benign label can be discordant when the MRI remains suspicious, the lesion was not actually sampled or the tissue is inadequate.
ACR LI-RADS support guidance recommends multidisciplinary discussion with consensus review of histology, imaging and other clinical data when pathology conflicts with an imaging category. Although that advice is written for liver observations, the underlying practice—an explicit, documented concordance assessment—is useful more broadly. LI-RADS categories and procedures should not be applied to organs or cases outside their stated context.
5. Check each classifier’s intended use and validation
For every software output, the clinical team should establish:

- What input quality, scanner, sequence, staining or digitization conditions are required.
- Whether the output is a detection, measurement, category, risk estimate or diagnostic aid.
- How the system was clinically validated and whether relevant entities were represented.
- What known limitations, exclusions and patient-specific unknowns apply.
FDA clinical decision-support policy materials discuss these elements for software using medical images, including MRI and pathology images. FDA classification pages separately describe digital-pathology assistance devices and radiological image-processing software. A regulatory classification does not establish that a particular product fits this patient, resolves a discordance or replaces independent professional review. The exact product labeling and current intended use must be checked.
6. Select the next diagnostic step with the relevant specialists
There is no universal rule that every mismatch requires surgery, another biopsy or observation. The choice depends on the organ, clinical concern, adequacy and accuracy of sampling, lesion change over time, risks of another procedure and the expected diagnostic yield.
| What the review finds | Possible direction | Important limitation |
|---|---|---|
| The biopsy likely missed the imaging target or sampled an adjacent area | Repeat targeted percutaneous sampling may be considered | The decision depends on lesion accessibility, procedural risk and whether a repeat sample is likely to answer the question |
| The sample is adequate and accurately from the target, but pathology does not explain a concerning MRI finding | Additional tissue or another diagnostic pathway may be considered | The appropriate procedure is organ- and patient-specific |
| Breast lesion accurately sampled, with radiologic–pathologic discordance | Excisional biopsy is described as standard of care in an RSNA RadioGraphics review | This breast-specific recommendation must not be generalized to other organs |
| Breast lesion sampling was inaccurate | Repeat percutaneous biopsy may be attempted | Accuracy of targeting and the clinical setting must be established first |
The breast recommendations above come from a breast MRI review and are not a general protocol for MRI–pathology disagreement. In every organ, the treating specialists must balance the probability of important disease against procedure risks and the consequences of delay.
Why a multidisciplinary review can change the answer
Discordance often persists because each discipline sees a different part of the case. A radiologist can identify the exact imaging feature and whether the biopsy target matches it. A pathologist can reassess the slides, deeper levels, ancillary studies and whether the tissue is representative. The procedural clinician can explain targeting, needle position and technical limitations. The treating specialist can integrate symptoms, prior disease, treatment and patient preferences.

A conference or consensus review allows these facts to be compared in one discussion. It is not an appeal to the most senior-sounding score; it is an adjudication of the complete case. A second opinion from an appropriate subspecialist may be useful when the diagnosis is unusual, the tissue is limited or the imaging pattern remains unexplained.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What should be documented
The record should make the unresolved issue visible to everyone who may act on it. ACR guidance for discordant LI-RADS cases recommends documenting:
- That imaging and pathology are discordant.
- The imaging category and the pathology diagnosis.
- The date the pathology specimen was acquired.
- A brief explanation of why the findings appear discordant.
- Whether multidisciplinary adjudication or consensus review was performed or requested.
- The agreed follow-up, responsible clinician and conditions that would trigger reassessment.
CAP’s active 2026 Interpretive Diagnostic Error Reduction update, listed online ahead of print on May 27, 2026, says cases may be reviewed prospectively or retrospectively, but “timely” review must occur before management is initiated when the review is intended to detect potential diagnostic errors. CAP’s recommendation is that anatomic pathologists develop procedures to review cases for disagreements and potential interpretive errors and to improve patient care.

- Are the MRI finding and the biopsy specimen definitely from the same lesion and location?
- What were the exact MRI category, pathology diagnosis and classifier outputs?
- Was the specimen adequate, and is there evidence that the target was sampled accurately?
- How long was the interval between MRI and tissue acquisition, and could the lesion have changed?
- What was each classifier designed to do, and does this case meet its intended-use and input requirements?
- Does an imaging–pathology concordance assessment explain the mismatch?
- Would review by a subspecialty radiologist, pathologist or multidisciplinary conference add information?
- If another procedure is proposed, what question will it answer, what is its expected yield and what are its risks?
- What is the follow-up plan if no immediate procedure is recommended?
What not to do
- Do not choose the result with the higher confidence score, stronger wording or more alarming category without reviewing the evidence.
- Do not assume a benign biopsy settles a suspicious MRI when sampling accuracy or adequacy is uncertain.
- Do not assume discordance automatically means cancer, a missed diagnosis or an error by one clinician.
- Do not order a repeat biopsy or surgery solely because two software outputs differ.
- Do not generalize a liver LI-RADS or breast MRI recommendation to another organ.
- Do not treat regulatory status or the presence of an AI label as proof that the tool is suitable for this case.
What is—and is not—known about disagreement rates
No general numeric rate for disagreement between pathology and MRI classifiers is established by the cited guidance. CAP’s earlier interpretive-error guideline reported that evidence was inadequate to determine a precise prevalence of discrepancy or error. Rates from a particular disease, scanner, algorithm or study population should not be transferred to a different patient or organ.
Best Value
Similarly, there is no single best resolution method or universal waiting period for every discordant case. The defensible approach is case-specific reconciliation of target, timing, adequacy, clinical concern and tool limitations, followed by a documented plan.
How to interpret the term “classifier”
A pathology classifier may evaluate digitized slides to assist a professional in determining a diagnosis. A radiological classifier may process MRI or other medical images to detect, segment, measure or categorize findings. Their labels may answer different questions, so apparent disagreement can sometimes be a mismatch of task rather than a true contradiction.
For example, one system may estimate that an image resembles a high-risk pattern while pathology identifies the tissue actually present in the sampled fragment. Neither output, considered alone, establishes what is happening in the entire lesion. The treating team must determine whether the outputs are comparable and clinically applicable before using them in management.

Pause management decisions long enough to reconcile the original MRI, tissue, reports and clinical context. Confirm lesion identity, sampling accuracy, timing, input quality and each classifier’s intended use. Then have the relevant specialists document the discordance and choose any repeat sampling, additional testing, specialist review or observation plan for this specific case. The correct next step cannot be selected from an isolated score.
