If a Claude citation does not support the answer, treat the claim as unverified—not as proven. Open the cited source, find the exact passage, compare it with the claim’s wording and scope, and then qualify, replace, or remove the claim. A URL, document pointer, or confident explanation is only a route to evidence.
Anthropic warns that Claude can produce incorrect or misleading responses, including authoritative-sounding quotations that are not grounded in fact. Its Help Center says users should review cited sources and original websites, and should not rely on Claude as a singular source of truth for high-stakes advice. Read Anthropic’s guidance.

Start with the claim, not the citation
Separate Claude’s answer into individual factual claims. A single sentence may contain several propositions: a number, a date, a description of who is affected, and a conclusion about what to do. One source may support one part but not the rest.

Preserve qualifiers such as “always,” “typically,” “as of 2026,” “for all users,” or “causes.” These words determine how much evidence is required. Then open the linked page or document yourself. Do not accept a source merely because its title, publisher, or URL looks relevant.

Six-step source check
1. Confirm the destination
- Open the citation in a new tab.
- Check the publisher, page title, publication date, and whether the page is an original source or a secondary account.
- Make sure the link resolves to the document Claude appears to have used, rather than a homepage, search result, cached copy, or similarly titled page.
For web-search answers, Anthropic specifically recommends reviewing the cited links and the original websites because Claude’s synthesis can omit important context. Anthropic Help Center.
2. Locate the evidence
Find the sentence, table, footnote, definition, or data point that Claude appears to have relied on. Search the page for distinctive terms, then read beyond the matching line. Continue far enough to see exceptions, eligibility rules, geographic limits, dates, definitions, and uncertainty statements.
A short excerpt can make a source appear to say more than it does. The original page may limit a result to a particular population, product version, jurisdiction, or study condition. Those limits belong in your assessment even if Claude left them out.
3. Decompose and compare
Compare each part of the answer with the passage. Ask five questions:

| Scope | Does it cover the same people, product, jurisdiction, circumstances, and exclusions? |
| Currency | Does the evidence cover the same time period, and is it current enough for this claim? |
| Context | Do surrounding passages add qualifications, exceptions, uncertainty, or a different definition? |
| Authority and provenance | Is this the original authority or a secondary retelling, and is the publisher suitable for the claim? |
These checks turn “the citation looks wrong” into a precise finding. They are a practical way to apply Anthropic’s advice to inspect original content and account for omitted context; they are not presented as an official Anthropic consumer checklist.

4. Label the result
Use a label that describes the relationship between the claim and the cited passage:
- Supported: the passage directly establishes the claim at the stated scope and certainty.
- Partly supported or missing context: the source supports a narrower or qualified version.
- Contradicted: the passage says something materially different.
- Not established by this source: the citation is relevant or nearby, but the evidence does not show the proposition.
Do not call a claim false solely because one citation is inadequate. “Not established by this source” means you need different evidence or a narrower statement; another authoritative source could still support the underlying proposition.

Choose the repair that matches your label:
- For a supported claim, retain the citation and quote or paraphrase the relevant passage accurately.
- For a partly supported claim, rewrite it to the source’s actual scope and include the omitted qualification.
- For a contradicted claim, correct it using the source or find a stronger original authority.
- For a claim not established by the citation, remove it or mark it as unverified until you locate direct evidence.
Keep the source, the original wording, the relevant passage, and your assessment together when another person must review the work. This creates an auditable record instead of silently changing the answer.
6. Escalate consequential claims
For medical, legal, financial, safety, employment, security, or other high-stakes advice, check the relevant original authority and consult a qualified professional when appropriate. Anthropic’s Help Center states: “Users should not rely on Claude as a singular source of truth and should carefully scrutinize any high-stakes advice given by Claude.” Source and date: March 16, 2026.
Why a credible-looking citation can still fail
Out-of-date information
Claude identifies outdated knowledge as one reason a response can be incorrect or misleading. A page can be genuine yet no longer describe the current policy, software version, price, regulation, or product behavior. Check the page’s date and any “last updated” notice against the time period in the claim.

Confident but ungrounded quotations
Anthropic also warns that Claude can produce quotations that look authoritative but are not grounded in fact. A quotation mark is not evidence. Search for the exact wording in the original source, verify the speaker and date, and read the surrounding passage. If you cannot locate it, do not publish it as a quotation.
Synthesis that drops qualifications
Web search gives Claude material to summarize; it does not guarantee that every exception survives the summary. A statement that was limited to one country, trial group, subscription tier, or date may emerge as a universal claim. The source passage—not Claude’s fluent wording—sets the boundaries.
A follow-up prompt that helps, but does not verify
You can ask Claude to expose its reasoning about the citation with this prompt:
Re-check each factual claim against the cited source. For each claim, quote the exact passage you believe supports it and explain whether it supports the full claim, only part of it, or none of it. Flag any passage you cannot locate, any missing context, and any claim that needs a different source. Do not fill gaps by guessing.Use the response as a checklist for your own inspection, not as independent confirmation. The same model that produced the unsupported statement may repeat it, invent a passage, or overlook the same qualification. Open the source and verify every quoted location yourself.
Web-search citations and API citations are different
Do not assume that a citation displayed in Claude’s consumer interface works like a citation returned by the Anthropic API.

The Anthropic API citation documentation describes zero-based indices for document and text or block positions. Plain-text and PDF sources are chunked into sentences by default. Scanned PDFs without extractable text are not citable through this mechanism, and image citations are not supported. Those are format and pointer constraints, not proof of semantic support.
Anthropic’s March 14, 2024 Citations cookbook likewise describes references to documents and locations supplied to the request. It notes that the feature will not return references to documents or locations that were not provided as valid sources. Its examples may have aged, so use the current API documentation for implementation details.

The link is relevant but does not say the quoted sentence
Mark the quotation unverified. Search the source for the exact words and inspect nearby sections. If the wording cannot be found, remove quotation marks and either paraphrase only what the page establishes or find the original statement.
The source supports a narrower claim
Rewrite the answer with the source’s population, location, date, product, or condition. For example, replace an unrestricted statement with one that names the study group or jurisdiction actually covered.

Check whether Claude cited a secondary summary while the answer describes a primary report. Locate the original authority and compare it directly. If you cannot find a source that supports the full claim, remove the unsupported portion.
The API pointer is precise, but the inference is too broad
Inspect the cited characters, sentences, page range, or content block and then apply the directness and scope checks. Pointer precision improves traceability; it does not make an interpretation automatically valid.
Several sources disagree
Record each source’s date, authority, population, method, and conclusion. Explain the disagreement rather than selecting the most convenient citation. A current regulator, standard-setting body, original dataset, or product documentation may carry more weight than a later commentary, depending on the claim.
What Anthropic’s citation figures do—and do not—show
In its June 23, 2025 announcement, Anthropic said internal evaluations found its built-in citation capability increased recall accuracy by up to 15% compared with most custom implementations. This is an Anthropic-reported internal evaluation, not an independent benchmark and not a guarantee that an individual citation is correct. Read the announcement.

The same announcement reports customer figures from Tarun Amasa, CEO of Endex: source hallucinations and formatting issues fell from “10% to 0%,” with a “20% increase” in references per response. Those are named customer-reported results presented by Anthropic, not universal outcomes or an expected error rate.
Anthropic describes the feature as providing “detailed references to the exact sentences and passages it uses to generate responses, leading to more verifiable, trustworthy outputs.” That product description explains the traceability goal; the human reader still has to decide whether the passage supports the claim.
A record you can hand to a reviewer
For important work, keep a compact verification record:
- The exact Claude claim, split into separate propositions.
- The citation URL or API location, including document and page or character details where available.
- The exact supporting passage and enough surrounding context to show qualifications.
- The source publisher, title, publication or update date, and whether it is primary or secondary.
- Your label: supported, partly supported or missing context, contradicted, or not established by this source.
- The repaired wording, replacement source, or reason for removing the claim.
- The reviewer or authority consulted when the consequence warrants escalation.
This record makes it clear what was checked and prevents a citation from being mistaken for a conclusion.

Bottom line: citations improve traceability, not truth by themselves
When Claude cites a source that does not support its answer, stop treating the answer as established. Open the source, locate the evidence, compare the exact claim with its scope and context, label the mismatch, and repair the wording or source. Web links and API pointers make inspection easier, but neither replaces independent judgment—especially when the advice could materially affect someone’s health, rights, money, safety, or security.
