Replay the disputed passage, correct the transcript text, repair the speaker label on every affected turn, and then recheck any summary, decision or action item that depended on it. Do not distribute the transcript until you have confirmed that you can edit it and that the intended recipients have the right viewing and download permissions.
Word recognition and speaker attribution are separate systems. A transcript can contain the right sentence under the wrong person, or the wrong words under the right person. The workflow below handles both problems while preserving an auditable path back to the audio.

Stop the error from spreading
As soon as someone reports a missing speaker or a wrong sentence, pause distribution of the transcript and any notes generated from it. Mark the passage under review so colleagues do not continue copying an unverified decision or assignment into other documents.

- Record the timestamp or section where the problem appears.
- Identify every summary, decision list, task list or follow-up message that used the disputed passage.
- Confirm that you have organizer or collaborator rights to edit the transcript before promising a correction.
Fluent wording is not proof of accuracy. Generated meeting text should be treated as a draft until it has been checked against the source audio.

A correction workflow that works across platforms
1. Locate the exact passage in the recording
Open the recording and jump to the timestamp associated with the questionable text. Use the platform’s playback controls, waveform or transcript-to-audio navigation when available. Listen to enough context to establish where the sentence starts and ends and whether another person began speaking at the same time.
Avoid reconstructing a sentence from memory when the recording exists. Overlapping speech, names and numbers are especially easy to misremember. Note the correct wording and the person who said it before changing anything.
2. Correct the words and punctuation
Edit the transcript passage itself, including punctuation and paragraph breaks when they change meaning. Preserve technical terms, names, dates and quantities exactly as they are heard. If a fragment cannot be understood from the audio, leave it clearly marked for human follow-up rather than inventing a plausible phrase.

Save the edit using the platform’s completion control. A correction that is visible only in an unsaved editing field has not fixed the shared transcript.
3. Repair speaker attribution at each affected turn
Correct the label on every turn touched by the error, not just the first paragraph. Check the transition before and after the disputed passage; a diarization mistake can continue for several turns after a speaker change.

Some services group related turns under one label, while an isolated paragraph may need its own correction. Review the whole exchange after saving so that a bulk label change has not assigned a name to the wrong person elsewhere.
4. Wait for processing to finish
Some editors realign the transcript with the audio after you save. Do not begin another edit until that process has completed. Otherwise, a second change can collide with the first or leave you unsure which version is current.

Read summaries, decisions, names, dates, commitments and action items that cite the corrected passage. A single changed pronoun can reverse who owns a task; a corrected number can alter a deadline or budget. Replace the downstream text rather than adding an unlinked footnote that leaves the original error visible.
6. Confirm sharing permissions before release
Check who can view, edit and download the corrected transcript. An organizer may control these settings, and an organization’s IT policy can limit which controls appear. Share the revised version only after verifying that recipients will not continue accessing an earlier copy.
Platform-specific editing paths
Microsoft Teams meeting recordings
For a meeting recorded in Microsoft Teams, the organizer can open the recording in the Microsoft 365 video player from OneDrive or SharePoint, select the transcript, edit the passage and choose Done. The exact controls can vary with administrator policy, so a missing edit command may indicate a permission or policy restriction rather than a damaged transcript.

After saving, reopen the passage from the beginning of the sentence and play it against the recording. Then inspect adjacent turns and any generated meeting notes that quote or summarize the same exchange. Organizers can manage who is allowed to view, edit and download the transcript; transcripts are stored with the recording in OneDrive or SharePoint.
Otter transcripts
In Otter, a collaborator with editing permission opens the Transcript tab and can change words, punctuation, paragraph breaks or a speaker assignment. Select Done to save. Otter performs an audio-realignment process after an edit, so wait for that process to finish before making another change.
The documented workflow does not support editing a live transcript in place. Make corrections after processing has completed. If the same person appears under several labels, correct the relevant turns and then inspect the rest of the conversation; tagging one speaker can update grouped labels, but a wrongly tagged paragraph elsewhere may still require a separate correction.
Why speaker names go missing or land on the wrong person
Speaker attribution is a separate task from recognizing words. In a connected video-conferencing meeting, a notetaker may match participant names to detected speaker turns. When the match fails, the transcript can retain generic labels such as Speaker 1 or Speaker 2, or show an unknown speaker.
Uploaded or ad-hoc audio can behave differently. Automatic participant names may not be available, leaving generic labels even when the words are readable. A rematching command cannot resolve an unknown person unless the service has a participant identity or a confirmed voice reference to use.

Room-based meetings add another complication. In a configured meeting room, voice profiles can support in-room identification. Without that recognition, audio may be attributed to the room itself. Participants can identify themselves in the transcript, and a person who was recognized incorrectly can be marked as misrecognized. Configuration and organizational policy determine which of these controls are available.
Always compare a speaker label with the recording when identity changes the meaning of a decision, promise or task. The sentence may be accurate while the accountability is not.

You can read the transcript but cannot edit it
- Check whether you are the meeting organizer or an authorized collaborator.
- Ask the organizer to grant editing access or make the correction themselves.
- In a managed workplace, check whether an administrator policy suppresses transcript editing.
- Do not download, alter and re-upload a private copy unless the team has agreed which file is authoritative.
The label is still “unknown” after you rematch speakers
Unknown is not the same as a generic numbered label. If the service has no participant identity or voice reference for that turn, rematching may not resolve it. Listen to the recording, identify the speaker from the meeting context, and apply the appropriate label manually when your permissions allow. If identity cannot be established reliably, retain a clearly qualified label instead of guessing.
A second edit appears to undo the first
Stop editing and wait for audio realignment or other post-save processing to finish. Reopen the passage, confirm the saved wording, and then make one change at a time. Record the final timestamp and corrected text in the team’s working notes so another editor does not reintroduce an earlier version.
The transcript is correct but the summary is wrong
Generated summaries are separate outputs. Re-read the corrected passage and manually replace the affected statement, owner, date or action item in the summary. Do not assume that changing the transcript automatically regenerates every downstream note.
The recording is unavailable
Do not present an inferred sentence as a verified quotation. Ask the organizer or recording owner whether the source file is stored with the meeting in OneDrive or SharePoint, or whether a permitted collaborator can provide access. Until the audio is available, label the passage as unverified and avoid assigning consequential work from it.
How to check a corrected transcript before sharing
- Play the entire corrected passage. Include the sentence before and after it to catch speaker changes and clipped words.
- Check proper nouns and numbers. Names, dates, times, quantities and technical identifiers deserve a second listen.
- Inspect every changed speaker turn. Verify that the label matches the voice and that grouped labels did not alter unrelated paragraphs.
- Trace derived content. Find each summary sentence, decision, task and deadline based on the passage and update it.
- Save and reopen. Confirm that the final text remains after processing completes.
- Review access. Ensure the intended audience can view the corrected version and that obsolete copies are not still being circulated.
What to evaluate when choosing an AI notetaker
If correction work is frequent, compare products by workflow rather than by a single accuracy claim. The available documentation does not establish a universal accuracy percentage or a comparative test result, so ask practical questions before adopting a service.
- Source-audio access: Does the transcript remain connected to an accessible recording, with timestamps or playback controls?
- Text editing: Can an authorized user correct words, punctuation and paragraph breaks after processing?
- Speaker editing: Can you change a label on one turn, a group of turns, or future conversations?
- Processing behavior: Does the service require a wait for audio realignment after each edit? Is live-transcript editing supported?
- Permission model: Can the organizer or a named collaborator edit, view and download the corrected file?
- Unknown-speaker handling: Does the interface distinguish an unresolved unknown from a generic numbered label, and can participants identify themselves?
- Room recognition: If meetings occur in shared rooms, are voice profiles available and governed by your organization’s policy?
Feature availability can depend on account setup, recording type and administrator settings. Treat those conditions as part of the product decision, not as edge cases discovered after an important meeting.

A team policy that prevents repeat errors
Define a simple owner for transcript quality. The organizer or assigned collaborator should review passages that contain decisions, commitments or sensitive names before the transcript is distributed. Keep the recording and transcript together, use one authoritative corrected copy, and tell recipients when a revision replaces an earlier version.
For recurring meetings, make speaker identification easier at the source: use the supported participant or room-identification features, introduce attendees when the recording begins, and speak one person at a time when possible. These practices do not guarantee perfect attribution, but they give the editor more evidence when a label is wrong.
The essential rule is straightforward: verify against audio, edit both words and labels, wait for processing, repair downstream notes, and check permissions before sharing. That sequence turns an unreliable first draft into a transcript the team can responsibly use.
